DOT1L — the target where excellent chemistry met an indirect interaction

also called DOT1L · KMT4 · histone-lysine N-methyltransferase DOT1L · H3K79 methyltransferase

DOT1L is the enzyme that puts a specific methyl mark on histones and keeps KMT2A-fusion leukemia genes switched on. Someone made an 80-picomolar inhibitor for it, and the drug still underperformed in patients — which is the most useful failure in this whole field.

The same thing, for a scientist

DOT1L (Q8TEK3, 1,537 aa) is the sole H3K79 methyltransferase. KMT2A fusions do not bind it directly; recruitment runs through the AHD domains of ENL/AF9-family partners or through the AF10/MLLT10 coiled coil. Aberrant H3K79me2 marks KMT2A-fusion target loci and the leukemia is Dot1l-dependent in vivo. Chemically this is the best-validated target in the interactome after menin: pinometostat/EPZ-5676 has a measured Ki of 80 pM with 37,000-fold selectivity over other methyltransferases. DepMap places it ninth on lineage-controlled selectivity (-0.166) and not common-essential (mean Chronos -0.148), so the therapeutic index is genuinely favourable. The clinical shortfall therefore cannot be blamed on potency, selectivity or essentiality, and understanding why is directly informative for the menin programme.

The AHD is intrinsically disordered and folds only upon binding, and AFF1, DOT1L, BCOR and CBX8 use it mutually exclusively.

Mutants that partially reduce DOT1L recruitment give partially reduced H3K79me2/me3 and partially reduced transformation - the response is graded, not switch-like.

For KMT2A::AFF1 the chain is fusion to AFF1 to the ENL/AF9 AHD to DOT1L, so DOT1L is reached through a competitive intermediate and is therefore buffered.

This is a prediction, not a measurement. No one has seen this protein in this shape. The colours say how much the software trusts itself, and that is worth reading before anything else — the orange stretches are places the model is effectively saying this part has no fixed shape.

I have never read one of these pictures before

A protein is one long chain of amino acids that folds up. The ribbon traces that chain from one end to the other; it is not a wire, it is a simplification of where the backbone runs. Drag to rotate, scroll to zoom.

Domains are the stretches of chain that do a particular job, and they are what the coloured blocks above the picture mark out. Click one and the view flies to it. In this project the important question about any domain is usually whether the fusion still has it — the labels say lost where it does not.

Anything in yellow sticks is a bound small molecule, which is usually the drug.

Go deeper
The recruitment is indirect, and that is the first thing to get right1 deeper

KMT2A::AFF1 does not contain a DOT1L-binding surface. The fusion retains the KMT2A N-terminus (AT-hooks, CXXC, the menin-binding motif) and the AFF1 C-terminus; the routes to DOT1L that have solved structures are:

  • Through the AF9/ENL AHD. 2MV7 is an NMR structure of DOT1L 877-900 bound to the MLLT3/AF9 AHD (residues 500-568) 1. Crucially, the degree of DOT1L recruitment to MLL-AF9 is graded: mutants that partially reduce recruitment give partially reduced H3K79me2/me3 and partially reduced transformation potential 2.
  • Through AF10/MLLT10. 6JN2 (3.6 Å) resolves the DOT1L coiled coil (470-550) with the AF10 leucine zipper (MLLT10 699-782); 7EDP (2.2 Å) captures an AF10 fragment with DOT1L 610-649. The higher-order configuration of these coiled coils drives tetramerisation into a DOT1L-AF10 octamer required for leukemogenesis 3.

For KMT2A::AFF1 specifically, neither route is direct. AFF1's own verified structural partner is the AF9 AHD (2LM0, AFF1 738-779 with MLLT3 490-568), so the chain is fusion → AFF1 → ENL/AF9 AHD → DOT1L, with each link occupying the AHD mutually exclusively 4. That means the AHD cannot simultaneously hold AFF1 and DOT1L, which is a real puzzle for the linear-chain picture and is discussed in interface-mllt3-ahd-competition-hub.

The gradedness is the most reusable finding in this node

Partial reduction of DOT1L recruitment gives partial loss of H3K79 methylation and partial loss of transformation 2. Dose-response is smooth, not switch-like.

That is good news for combination strategies and bad news for monotherapy. It predicts that a DOT1L inhibitor at achievable exposure produces a partial phenotype rather than collapse — which is roughly what happened clinically — and it predicts that DOT1L inhibition should combine additively with anything else that lowers fusion output, including menin inhibition. The same logic should be applied to the menin arm: if the menin-KMT2A-LEDGF clamp is also graded rather than switch-like, complete responses require either complete occupancy or a second hit.

The dependency numbers are good, which is why the clinical result is instructive1 deeper

HPAtissue enhanced, detected in all; not detected in immune cells

Two things stand out 5.

First, the naive selectivity score (-0.417) is two and a half times the lineage-controlled one (-0.166). DOT1L is substantially a leukemia gene, not specifically a KMT2A-rearrangement gene. Restricting the comparison to leukemia lines cuts the apparent selectivity sharply, and the same correction should be applied to every partner before a target is chosen.

Second, HPA reports DOT1L as not detected in immune cells, which is unusual in this dossier — almost every other partner is 'low immune cell specificity, detected in all'. If DOT1L protein really is low in mature immune compartments, the expected haematological toxicity profile is better than the gene's ubiquity suggests.

So: not common-essential, favourable expression, ninth on selectivity, and an 80 pM highly selective inhibitor. The programme still underdelivered. The lesson for this project is that potency, selectivity and a clean essentiality profile are jointly insufficient. What they do not address is whether the node is load-bearing enough, and whether the residual activity left by partial inhibition is enough for the cell.

Why menin inhibition worked where DOT1L inhibition disappointed

This is our reading rather than a published comparison, and it is stated as inference. Three differences are structural rather than pharmacological.

Mechanism of action on chromatin. Blocking menin evicts the fusion from chromatin; blocking DOT1L leaves the fusion bound and removes one downstream mark. Eviction is a more complete intervention than enzyme inhibition at the same level of target engagement.

Position in the chain. DOT1L is recruited indirectly, through an AHD that four proteins compete for. Anything reached through a competitive intermediate is buffered. Menin binds the fusion directly at a motif retained in every breakpoint.

Pharmacokinetics. Pinometostat required continuous intravenous infusion to achieve sustained regressions in the preclinical rat model 6. A target that needs continuous infusion to work is a different clinical proposition from an oral agent, independent of how good the molecule is.

Chemical matter: rich, and oddly uncurated

ChEMBL target CHEMBL1795117 carries 193 activity rows at pChEMBL ≥ 6, best Ki 0.46 nM (CHEMBL2171169), dominated by one SAM/SAH-mimetic series with S-adenosylhomocysteine (CHEMBL418052) as the reference compound. Notably there is no curated mechanism-of-action record for pinometostat on this target, which is a curation gap rather than a biological statement and a reminder that ChEMBL mechanism records under-report clinical matter 7.

Structurally the target is as well characterised as anything in this beat: the catalytic domain with SAM (1NW3 2.5 Å, 3QOW 2.1 Å), with SAH (3QOX 2.3 Å), with a selective inhibitor (3SR4 2.5 Å), and a whole series of cryo-EM structures on H2B-ubiquitinated nucleosomes capturing active and poised states (6NJ9 2.96 Å, 6NQA 3.54 Å, 6NOG 3.90 Å, 7XCR 2.57 Å) 1. If the project ever needs a positive control for a co-folding or docking pipeline, this is the system with the best structural coverage.

How this could bite us back
  • Indirect recruitment means buffering: DOT1L is reached through an AHD that four proteins compete for, so removing DOT1L may simply let another AHD ligand take its place.
  • Delivery: the preclinical regimen that produced sustained regressions was continuous intravenous infusion. For an infant population that is a serious practical barrier independent of target biology.
  • Partial response breeds resistance: because the recruitment-to-output relationship is graded rather than switch-like, achievable inhibition gives partial suppression of the leukemic programme, which is the selection pressure under which resistance arises.
  • H3K79 methylation has roles outside the fusion programme, including in DNA damage response; combining a DOT1L inhibitor with genotoxic chemotherapy is not obviously safe.
  • Compensation through the second axis: KMT2A fusions also drive their programme through menin-LEDGF chromatin tethering and SEC-mediated elongation, so removing the H3K79 mark leaves the fusion bound and the elongation machinery in place.
Still open
  1. Why did an 80 pM, 37,000-fold selective, non-common-essential target underdeliver clinically? Separating pharmacokinetics (continuous infusion needed), indirectness of recruitment (through a competitive AHD) and gradedness of the phenotype would tell the menin programme which of its own assumptions to stress-test.
  2. Is DOT1L inhibition synergistic with menin inhibition, and is the synergy additive on fusion output or genuinely mechanistic? The gradedness result predicts additivity, and a matrix in SEM and MV4-11 with H3K79me2 and HOXA9 readouts would test it.
  3. For KMT2A::AFF1 specifically, which AHD-bearing partner delivers DOT1L - ENL or AF9? The DepMap split between MLLT1 (selectivity -0.132) and MLLT3 (+0.059) says ENL, but nobody has shown that DOT1L recruitment in a KMT2A::AFF1 line runs through ENL rather than AF9.
  4. If AFF1, DOT1L, BCOR and CBX8 occupy the AHD mutually exclusively, how does a single fusion complex use more than one of them? Either multiple ENL/AF9 copies are present per complex or the partners exchange dynamically, and the two models predict different drug behaviour.
  5. Is the HPA 'not detected in immune cells' call real at the protein level in haematopoietic stem and progenitor cells? If so it materially improves the predicted toxicity profile and nobody has checked it directly.
  6. This entry summarises DOT1L recruitment as the AHD of ENL/AF9-family partners or the AF10/MLLT10 coiled coil. Both are correct and neither is present in KMT2A::AFF1, which leaves the index fusion with no stated route to DOT1L. Reported by the Q3b beat as an omission rather than an error - no citation settles it - and the rule it implies is worth holding: AHD binds AF9/ENL, the OM-LZ octamer binds AF10, and KMT2A::AFF1 has neither.

    What this evidence cannot show

Identifiers, methods, resolutions and residue ranges were read from the RCSB API in this session rather than recalled, but residue ranges are deposited construct ranges annotated against UniProt, not ordered regions in the maps. None of these structures contains KMT2A or AFF1, which is the point: there is no structure of DOT1L with a KMT2A fusion protein.

Evidence that argues against this (1)
    • Counting PDB entries measures how tractable a protein was to CRYSTALLISE, which correlates with being small, ordered and well-behaved rather than with being druggable. What decides design value is whether the deposited density covers the site that matters, at a resolution that supports modelling, in the relevant state — and this graph has repeatedly found structures described in the direction that overstates what is resolved. DOT1L's problem was never structural. The compound reached patients, demethylated on target, and produced 2 complete remissions in 51 — so a dossier built on structural coverage is measuring the axis that did not fail. Blood 2018
  2. E2strong confidence 4 of 5: Replicated in peer-reviewed work by more than one group.paper

The degree of DOT1L recruitment to MLL-AF9 determines the level of H3K79 di- and tri-methylation on target genes and the transformation potential, with graded mutants giving graded loss of both; the NMR structure of a DOT1L-AF9 AHD complex identifies three AF9 binding sites on DOT1L.

    What this evidence cannot show

The system is MLL-AF9, not KMT2A::AFF1, and the transformation readout is a murine model. The abstract reports no numeric affinity, so no Kd should be attributed. Graded recruitment by mutation is not identical to graded occupancy by a drug, which also competes with substrate.

Evidence that argues against this (1)
    • Graded recruitment giving graded methylation and graded transformation is an elegant dose-response and it is measured in MLL-AF9, a murine myeloid model, with engineered mutants rather than pharmacological occupancy. Non-canonical H3K79me2-dependent pathways promote survival of KMT2A-rearranged leukaemia, so removing the mark and removing the requirement are not the same operation. eLife 2021

The AF10/MLLT10 leucine zipper binds the DOT1L coiled-coil domain, and the higher-order configuration of these heterodimeric coiled coils drives tetramerisation into a DOT1L-AF10 octamer that is required for MLL-AF10 leukemogenic activity.

    What this evidence cannot show

This is MLL-AF10 biology and does not transfer directly to KMT2A::AFF1, where AF10 is not part of the fusion. The oligomerisation requirement is shown for leukemogenic activity of the AF10 fusion, and whether the same octamer is needed for DOT1L function recruited via ENL or AF9 is not established here. No affinity is reported in the abstract.

Evidence that argues against this (1)
    • Octamerisation of DOT1L-AF10 is required in MLL-AF10, where AF10 is the partner. KMT2A::AFF1 does not contain AF10, so the tetramerisation requirement is a property of one fusion rather than of DOT1L, and importing it into an AFF1 dossier crosses a partner boundary. Leukemia 2023

AFF1/AF4 binds the AF9/MLLT3 AHD (structure 2LM0, AFF1 738-779 with MLLT3 490-568), and binding of AF4-family proteins, DOT1L, BCOR and CBX8 to the AHD is mutually exclusive, so the fusion cannot engage AFF1 and DOT1L on the same AHD simultaneously.

BCOR binding to MLL-AF9 is essential for leukemia via altered EYA1, SIX, and MYC activity · Schmidt CR, Achille NJ, Kuntimaddi A, et al. · Blood Cancer Discovery 2020
    What this evidence cannot show

Mutual exclusivity is demonstrated for the isolated AHD with isolated partner peptides; in a cell, multiple ENL and AF9 molecules are present and different copies could hold different partners, so exclusivity at the level of one domain does not mean exclusivity at the level of the complex. The structural work is on MLL-AF9, not KMT2A::AFF1.

Evidence that argues against this (1)
    • Mutual exclusivity of AFF1, DOT1L, BCOR and CBX8 on the AF9 AHD was established in vitro with isolated domains, where competition reflects the concentrations the experimenter supplied. In a nucleus the same surfaces can be occupied by different partners on different molecules at one locus, so the constraint applies to a single AF9 chain and not to the complex at a gene. Nature 2012

DOT1L is not DepMap common-essential (mean Chronos -0.148 across 1,258 lines, 27.4% of lines depleted) and falls to -0.565 across 10 KMT2A-rearranged lines and -0.546 across 4 KMT2A::AFF1 lines, giving a lineage-controlled selectivity of -0.166 (rank 9 of 41) against a naive selectivity of -0.417; gnomAD gives pLI 1.0, LOEUF 0.288, 33 observed versus 153.3 expected loss-of-function variants; HPA classifies DOT1L as tissue enhanced, detected in all tissues, and not detected in immune cells.

    What this evidence cannot show

Means were computed by us from per-line API values, not read from a DepMap release table, and the release version is not stated by the API. Only 10 KMT2A-rearranged lines (4 KMT2A::AFF1) contribute. The HPA 'not detected in immune cells' classification is an RNA-level blood-cell call and should not be read as absence of DOT1L protein from haematopoietic progenitors, which is where toxicity would arise.

Evidence that argues against this (1)
    • Cell-line gene effect cannot see the in vivo requirement. The in vivo statement that MLL-rearranged leukemia is Dot1l-dependent, with little global effect of Dot1l inactivation outside direct target loci, comes from mouse genetics rather than from DepMap. Bernt KM et al. 2011

Pinometostat (EPZ-5676) inhibits DOT1L histone methyltransferase activity with a measured inhibition constant Ki of 80 pM and 37,000-fold selectivity over all other methyltransferases tested, and continuous intravenous infusion produced complete, sustained tumour regressions in a rat KMT2A-rearranged xenograft.

Potent inhibition of DOT1L as treatment of MLL-fusion leukemia · Daigle SR, Olhava EJ, Therkelsen CA, et al. · Blood 2013
    What this evidence cannot show

This is the only numeric affinity we verified anywhere in this beat, and it is a biochemical methyltransferase assay, not a cellular or clinical potency. The requirement for continuous intravenous infusion to achieve sustained regressions is itself the important limitation: a target needing continuous infusion is a different clinical proposition from one addressable orally, independent of molecular quality.

Evidence that argues against this (1)
    • An 80 pM Ki with 37,000-fold selectivity is the strongest chemistry in this entire interactome and it is the clearest demonstration that potency and selectivity are not the limiting variables. The same compound's trial did not reach its maximum tolerated dose, so its modest activity is consistent with inadequate exposure as well as with a non-limiting pathway. Blood 2018

ChEMBL v34 target CHEMBL1795117 (DOT1L) carries 193 activity rows at pChEMBL >= 6 with a best Ki of 0.46 nM (CHEMBL2171169), dominated by one SAM/SAH-mimetic series, and carries no curated mechanism-of-action record for pinometostat.

    What this evidence cannot show

The absence of a pinometostat mechanism record is a curation gap, not a biological statement; mechanism records in ChEMBL frequently sit on family or protein-complex targets rather than single-protein ones. The 193 rows and the 'best Ki' are from a page-local sample of 25 rows returned in activity-id order, so the best value is the best in a sampled page dominated by one publication, not a potency-ranked view of the whole target.

Evidence that argues against this (1)
    • A ChEMBL activity-row count measures how much medicinal chemistry has been DONE, not how tractable the target is, and the best reported affinity belongs to the most optimised compound in whichever series someone chose to pursue. Zero rows means nobody tried; thousands of rows means a target had a decade of industrial attention. 193 rows dominated by one SAM/SAH-mimetic series is a single chemical idea counted many times, which is the failure mode a row count is most prone to.
