SAM supply sustains the KMT2A-rearranged hypermethylated state - the metabolic requirement that IS a consequence of the chromatin programme, and can be attacked as one
also called methionine cycle dependency in KMT2A-r leukaemia · SAM addiction
Chemical tags on DNA and histones do not stay put on their own; the cell has to keep re-applying them, and each one costs a molecule called SAM. KMT2A-rearranged leukaemia keeps far more of these tags than a normal cell, so it has a far larger SAM bill - and if you stop paying the bill, the genes the leukaemia depends on switch off.
The same thing, for a scientist
This mechanism states the direction of causation that the guanine-nucleotide mechanism inverts. Here the fusion's chromatin programme comes **first** and creates a downstream metabolic requirement: KMT2A-rearranged leukaemia is particularly vulnerable to methionine-cycle perturbation, and the dependency is *driven by* an increased need for S-adenosylmethionine to maintain the hypermethylated state; under methionine restriction, important pro-survival KMT2A-r target genes are repressed, and combined with other downstream metabolic changes this results in rapid cell death [E1]. The enzyme node is MAT2A, the sole extrahepatic route to SAM; an orally active MAT2A inhibitor impaired leukaemia progression in KMT2A-rearranged ALL patient-derived xenografts [E1], and an independent laboratory five years earlier found MAT2A overexpressed in a human CRISPR/Cas9-MLLr model with an MLLr-enhanced response to MAT2A inhibition and to knockdown, altering histone methylation, with more pronounced effects in combination with DOT1L or PRMT5 inhibitors [E2]. **The practical significance is that this is a way to reduce the fusion's transcriptional output without touching a single protein-protein interface**, and therefore a route that is indifferent to MEN1 resistance mutations.
Why a maintenance cost is a target at all
Histone and DNA methyl marks turn over. Demethylases remove them, replication dilutes them, and the cell re-applies them continuously. A cell holding an abnormally large number of marks is therefore paying an abnormally large ongoing SAM bill, not a one-off cost. That is what makes a supply-side attack plausible: the difference between the leukaemic cell and a normal cell is not that one uses SAM and the other does not, it is the rate.
The claim that this rate difference is large enough to matter is the whole mechanism, and it rests on two comparative observations 12 plus a PDX result 1. It has one large piece of contrary evidence, recorded in gene-mat2a: MAT2A is depleted in 95.0% of DepMap lines, which is floor compression and means the genetic screen cannot see selectivity here even if it exists.
What 'hypermethylated' means here, and why the ambiguity matters
The source abstract says hypermethylated state without specifying DNA or histone methylation 1. This graph must not resolve that by assumption, because the two readings imply different partner drugs:
- DNA hypermethylation would pair SAM restriction with hypomethylating agents - which would be antagonistic in one reading (both reduce methylation, so why combine) or synergistic in another (different mechanisms of the same reduction).
- Histone hypermethylation, principally H3K79me2 from DOT1L and H3K4 from the KMT2 family, would pair it with DOT1L, PRMT5 or menin inhibitors - and the earlier MAT2A study reports exactly those combinations working 2, which is weak indirect support for the histone reading.
Recorded here as unresolved. It is answerable by reading the two full texts, which this beat did not do.
Consequence, not independent dependency - and why that is the useful kind
Of the mechanisms in this beat, this one is the clearest example of a metabolic requirement that is downstream of the fusion's transcriptional programme. The causal statement is in the source: the methionine dependency is driven by the need to maintain the hypermethylated state 1.
That is strategically better than an independent dependency, not worse. An independent metabolic dependency is a second target you must also hit. A consequential dependency is a second way to hit the first target - which is why methionine restriction represses KMT2A-r target genes rather than merely slowing growth 1. The test that would confirm it is simple and has not been published: does MAT2A inhibition reduce H3K79me2 and KMT2A-fusion target gene expression on the same timescale and in the same rank order as a menin inhibitor does?

- SAM is the methyl donor for every methyltransferase in the cell, including those maintaining DNA methylation, those on non-histone substrates, and those in the developing brain. Lowering its supply is the opposite of a targeted intervention, whatever the comparative cell-line data show.
- The deliverable arm in infants is the drug, not the diet: methionine is an essential amino acid and these patients are under one year old and frequently failing to thrive.
- Every named combination partner - DOT1L, PRMT5, HDAC inhibitors - carries its own dose-limiting toxicity, and DOT1L inhibition has already returned a modest single-agent clinical result in this disease.
- Resistance route already visible: the methionine salvage pathway. MTAP status was not reported in either source study and MTAP deletion is common in leukaemia, so a large fraction of patients may have an altered baseline for exactly this axis.
- Does MAT2A inhibition reduce KMT2A-fusion target gene expression in the same rank order as a menin inhibitor? If yes, this is a metabolic route to the same output and should be prioritised; if no, it is general cytotoxicity wearing a mechanism.
- DNA or histone methylation? Both source abstracts say 'hypermethylated' or 'histone methylation' without specifying the mark, and the answer determines the partner drug.
- Can the functional MLLr-enhanced response be reconciled with MAT2A being depleted in 95% of DepMap lines, or does the comparative result depend on which non-MLLr lines were chosen as comparators?
- Does the mechanism hold in menin-inhibitor-resistant, MEN1-mutant cells? That is the scenario in which a non-interface route to the same output would be worth most.

KMT2A-rearranged leukaemia is particularly vulnerable to perturbations of the methionine cycle; this methionine dependency is driven by an increased need for S-adenosylmethionine to maintain the hypermethylated state of KMT2A-rearranged leukaemias; important pro-survival KMT2A-r target genes are repressed under methionine restriction, which combined with other downstream metabolic changes results in rapid cell death; the orally active MAT2A inhibitor FIDAS-5 impaired leukaemia progression in patient-derived xenograft models, and a drug screen revealed strong synergy between MAT2A inhibition and histone deacetylase inhibitors.

Abstract read this session; full text not retrieved. The abstract does not define 'hypermethylated' as DNA or histone methylation, does not name the KMT2A fusion partners in the PDX panel, and does not state how many PDX models were used - so 'KMT2A-rearranged ALL' must not be read as infant t(4;11)/KMT2A::AFF1. The causal statement that the dependency is driven by the SAM requirement is the paper's own interpretation of its data; the abstract does not describe the experiment that establishes direction. FIDAS-5 has no clinical data. Methionine restriction as an intervention has broad antiproliferative effects across tumour types, so the disease-specificity rests on the comparative experiments rather than on the manoeuvre.
Evidence that argues against this (1)
- This project's own dependency analysis places MAT2A at a Chronos mean of -1.349 with 95.0% of DepMap lines depleted and a KMT2A-selectivity difference of -0.158 myeloid and -0.014 lymphoid. That is floor compression: a gene required by almost every line has no dynamic range for selectivity to appear in, so the screen provides no support for a genotype-specific dependency and is consistent with a pan-essential enzyme whose inhibition is simply cytotoxic. In a human CRISPR/Cas9-MLL-rearranged leukaemia model, MAT2A was significantly overexpressed relative to healthy controls; non-MLLr and MLLr cell lines and the model showed an MLLr-specific enhanced response to the MAT2A inhibitor PF-9366 and to siRNA knockdown across proliferation, viability, differentiation, apoptosis, cell cycling and histone methylation; combination with chemotherapy or with inhibitors of the SAM-dependent methyltransferases DOT1L and PRMT5 gave more pronounced effects.

Abstract read this session. The 'histone methylation' readout in the abstract is unspecified as to which mark, which is the same ambiguity as E1 and from an independent group. Engineered CRISPR/Cas9-MLLr model plus cell lines; overexpression relative to healthy CD34+ controls is a weak comparator, since a biosynthetic enzyme scales with proliferation. PF-9366 is a tool compound with limited independent characterisation in haematopoietic cells.
Evidence that argues against this (1)
- Overexpression of a biosynthetic enzyme in a proliferating cell is expected rather than informative: SAM demand scales with methylation, transcription and growth, so MAT2A being higher in leukaemia than in healthy controls does not by itself distinguish a dependency from a consequence of proliferation.
