The differentiation block — what it actually is, operationally, in KMT2A::AFF1 leukaemia

also called maturation arrest · differentiation arrest · HOXA lock · blocked B-lymphoid maturation

The blasts in this leukaemia are frozen at an immature stage: they keep dividing but never finish becoming working blood cells. The freeze is maintained actively, by the fusion protein rewriting which genes stay switched on, which is why releasing it is a treatment strategy.

The same thing, for a scientist

The differentiation block in KMT2A-rearranged leukaemia is not a passive failure to mature but an actively maintained transcriptional and chromatin state. Operationally it is defined by four measurable features: (1) an arrest at a defined immature stage detectable by immunophenotype and by projection of transcriptomes onto haematopoietic references; (2) sustained high expression of a stage-inappropriate transcription factor module, HOXA-type or IRX-type depending on fusion partner; (3) aberrant spreading of the fusion, of menin and of H3K79me2/3 into the bodies of target genes with disruption of normal H3K36me3 patterning, at unmethylated CpG regions; and (4) extensive promoter CpG island hypermethylation that silences differentiation genes and is pharmacologically reversible. Its most striking functional demonstration is that KMT2A-rearranged B-ALL blasts are refractory to reprogramming into pluripotency, whereas MLL-AF4-overexpressing normal progenitors are not.

Go deeper
Four operational definitions, not one metaphor1 deeper

'Differentiation block' is used loosely in the field. Here is what can actually be measured.

1. Position on a differentiation axis. Multi-omics profiling of 84 infants with KMT2A-rearranged leukaemia, using single-sample and single-cell inference of haematopoietic differentiation, assigned every case to one of five integrative clusters corresponding to defined stages of B-lymphopoietic and early hemato-endothelial development, with different master transcription factors: IRX-type differentiated, IRX-type undifferentiated, and three HOXA-type clusters for MLLT1, MLLT3 and AFF1. The undifferentiated IRX-type cluster was the most refractory and carried RAS pathway mutations in 100% of cases 1. So the block has a coordinate, and the coordinate is prognostic.

2. A transcription factor module held on. The KMT2A-AFF1 gene regulatory network built from ChIP-seq, patient RNA-seq and CRISPR essentiality screens identified core transcription factors, notably RUNX1, that regulate downstream targets through feed-forward loops and cascades. A core set of nodes is present in both ALL and AML forms, and a KMT2A-AFF1:RUNX1 cascade acts as a repressor arm 2. The block is therefore a network property, not the action of a single gene, which is why removing any one node rarely releases it.

3. Chromatin spreading at the fusion's targets. At a subset of gene targets, MLL-AF4 binding spreads into the gene body, accompanied by spreading of menin binding, increased transcription, increased H3K79me2/3, disruption of normal H3K36me3 patterns, and unmethylated CpG regions in the gene body. Spreading genes are preferentially downregulated by DOT1L inhibition, and this creates synergy with other targeted agents 3. This is the most concrete molecular definition of the block available, and it contains a direct therapeutic prediction that has been tested.

4. DNA methylation that silences the alternative. Distinct promoter CpG island methylation patterns separate genetic subtypes of MLL-rearranged infant ALL: t(4;11) and t(11;19) are extensively hypermethylated, whereas t(9;11) and MLL-wild-type infant ALL epigenetically resemble normal bone marrow. Among t(4;11)/t(11;19) patients, the degree of promoter hypermethylation influenced relapse-free survival, with accentuated methylation marking high relapse risk, and the demethylating agent zebularine reversed the aberrant methylation and induced apoptosis 4.

Point 4 deserves emphasis for this project: t(4;11) is one of the hypermethylated subtypes and t(9;11) is not 4. A therapeutic model built on 'KMT2A-rearranged leukaemia' as one entity discards that.

The experiment that shows the block is deep, not superficial

The most unusual evidence that this state is stably locked comes from reprogramming. Multiple strategies (mono- and polycistronic OKSM(L), retroviral, lentiviral, episomal and Sendai virus vectors, plus transcriptomic-epigenetic 'boosters') failed to generate iPSCs from primary MLL-rearranged B-ALL blasts and B-ALL lines, in vitro and in vivo. When iPSCs did emerge, they lacked the leukaemic fusion gene, proving a non-leukaemic myeloid origin. Critically, MLL-AF4-overexpressing haematopoietic stem cells and B progenitors reprogrammed successfully, so neither B-cell origin nor the fusion gene itself is the barrier. Global transcriptome and DNA methylome profiling pointed to a developmental/differentiation refractoriness specific to MLL-rearranged B-ALL 5.

That is a rare kind of result: a cell state so fixed that the most powerful known reprogramming cocktail cannot move it, while the same fusion in a normal progenitor moves easily. The block is a property of the leukaemic state, not of the fusion.

The block is stage-setting, not merely stage-holding

The classic demonstration that a KMT2A fusion installs a self-renewal program rather than reverting cells to a stem state came from MLL-AF9. Leukaemia stem cells from leukaemias initiated in committed granulocyte-macrophage progenitors transferred disease with as few as four cells, while keeping an immunophenotype and global expression profile very similar to normal GMPs, and re-activating only a limited subset of genes highly expressed in normal HSCs 6.

The consequence for the block is precise. The fusion does not de-differentiate the cell. It freezes the differentiation state it found and grafts limitless self-renewal onto it. That reframes differentiation therapy: the task is not to push a stem cell down a lineage, it is to remove an inappropriate self-renewal module from a cell that is already partly committed.

Why releasing the block is not automatically good

Menin inhibition works clinically by releasing differentiation, and the price is differentiation syndrome, a mechanism-linked and potentially fatal toxicity that scales with efficacy. There is a second, less discussed price. MLL-Af4-transformed human cells can assume a myeloid state under environmental pressure while retaining lymphoid potential, and the same work noted t(4;11) patients whose leukaemia evaded CD19-directed therapy by myeloid lineage switch 7. If the arrest is released in a cell whose lineage commitment was never completed, the destination is not guaranteed to be a mature B cell.

That is the central unresolved question of this beat and is written up as a testable hypothesis in hyp-menin-release-lineage-destination.

How this could bite us back
  • Differentiation syndrome is a mechanism-linked and potentially fatal toxicity whose severity scales with how well the drug releases the block.
  • Release of the arrest in a cell with incomplete lineage commitment can produce a myeloid rather than a B-cell outcome, which is lineage switch rather than cure.
  • The block is a network property with redundant nodes and a core shared between the ALL and AML forms, so removing one node is likely to be compensated.
  • Extensive promoter hypermethylation in t(4;11) provides a second, menin-independent mechanism to keep differentiation genes silent after the fusion is pharmacologically neutralised.
Still open
  1. When menin inhibition releases the block, where do the cells go? Mature B, myeloid, or nowhere? Single-cell transcriptome plus surface proteome on serial marrows would answer this and nothing else will.
  2. t(4;11) is hypermethylated and t(9;11) is not. Does that mean a DNA methyltransferase inhibitor plus a menin inhibitor is a t(4;11)-specific combination that would look inactive if tested across unselected KMT2A-r disease?
  3. Is the reprogramming refractoriness of MLL-r B-ALL blasts reversible by menin or DOT1L inhibition? A reprogramming assay would be an unusual but sensitive readout of whether the lock has genuinely been opened.
  4. Does the block in the IRX-type undifferentiated, RAS-mutated cluster depend on menin at all, or is it held by signalling rather than chromatin?

    What this evidence cannot show

Differentiation stage is inferred by projection onto reference maps rather than measured directly. Predominantly Japanese cohort. A five-cluster solution over 84 samples needs independent replication.

Evidence that argues against this (1)
    • Five integrative clusters were derived by clustering 84 cases on combined RNA, methylation, exome and targeted sequencing, and the number five is a choice made during the analysis rather than a count of things in the world. Differentiation-stage assignment is by similarity to normal reference profiles, so a leukaemia that expresses a fetal programme will be assigned a fetal stage whether or not it is arrested there. A computationally inferred network integrating datasets of differing provenance; edges are statistical predictions, and only a subset were experimentally validated. Cell-line-heavy input data.

Evidence that argues against this (1)
    • The gene regulatory network is a reconstruction, not a measurement. It is assembled from ChIP-seq occupancy, bulk patient RNA and CRISPR essentiality in lines, and edges inferred by combining three assays inherit the weakest one: occupancy does not imply regulation, bulk RNA averages over the clusters this beat says the disease splits into, and essentiality comes from cell lines whose lineage composition this project has shown to be confounded. "A core set of nodes shared between the ALL and AML forms" is also the expected result of reconstructing a network from genes both forms express. Oncofusion knowledge graph (internal analysis) 2026

At a subset of target genes MLL-AF4 binding spreads into the gene body together with spreading of menin binding, increased transcription, increased H3K79me2/3, disruption of normal H3K36me3 patterns and unmethylated CpG regions in the gene body; expression of these spreading genes is preferentially downregulated by DOT1L inhibition, which synergises with additional targeted drugs.

    What this evidence cannot show

Genomic profiling in leukaemia cell lines and a limited number of primary samples; spreading is defined by a computational threshold on ChIP signal, so the gene set depends on that threshold. Sensitivity to DOT1L inhibition in vitro has not translated into clinical benefit for pinometostat as a single agent.

Evidence that argues against this (1)
    • Spreading of fusion and menin binding into gene bodies with raised H3K79me2/3 and disrupted H3K36me3 is a description of a transcribed locus. Broad H3K79 domains and H3K36me3 redistribution both FOLLOW elongation, so the correlation cannot order the events, and unmethylated CpG in a gene body is the expected state of an actively transcribed gene. eLife 2021

Differential methylation hybridisation is a 2009-era platform with lower resolution than current bisulfite sequencing, so the absolute methylation calls should be treated as subtype-level rather than locus-precise. Zebularine activity is in vitro; no clinical translation in infant ALL followed. Survival analysis is retrospective and subgroup-based.

Evidence that argues against this (1)
    • The therapeutic inference that follows from promoter hypermethylation - that stripping the marks should help - has now been tested in exactly this population and failed. COG AALL15P1 gave five days of azacitidine immediately before each of four post-induction chemotherapy courses in infants with newly diagnosed KMT2A-rearranged ALL, confirmed by whole-genome bisulfite sequencing that methylation decreased in 87% of samples tested, and reported event-free survival similar to prior studies with no impact on survival. Primary MLL-rearranged B-ALL blasts and B-ALL lines resist reprogramming to pluripotency across retroviral, lentiviral, episomal and Sendai virus strategies with added transcriptomic-epigenetic boosters, and any iPSCs obtained lacked the leukaemic fusion gene; by contrast MLL-AF4-overexpressing haematopoietic stem cells and B progenitors reprogrammed successfully, indicating a developmental/differentiation refractoriness specific to the leukaemic state rather than to B-cell origin or the fusion itself.

    What this evidence cannot show

A negative reprogramming result is hard to interpret definitively: reprogramming efficiency of any primary leukaemia is extremely low, so failure may reflect proliferative or survival constraints rather than a chromatin lock. The positive control (MLL-AF4-overexpressing normal progenitors) is the strongest part of the argument.

Evidence that argues against this (1)
    • Failure to reprogramme across four delivery systems is a robust negative and an ambiguous one. Primary leukaemic blasts reprogramme poorly for reasons that have nothing to do with the fusion — they are post-mitotically constrained, genomically damaged and hard to keep alive in culture — and the iPSC lines that did emerge lacking the fusion is the signature of outgrowth from residual normal cells, which is evidence about culture rather than about the block. Mouse retroviral MLL-AF9 myeloid model; the lymphoid t(4;11) disease may differ, and 2006-era profiling cannot resolve intra-population heterogeneity.

Evidence that argues against this (1)
    • This result depends on supraphysiological fusion expression and does not hold at endogenous dosage. When Mll-AF9 was placed under endogenous regulatory control, LSK (Lin-Sca1+c-kit+) stem cells were efficiently transformed while committed granulocyte-monocyte progenitors were transformation-RESISTANT and did not cause leukaemia; GMPs became transformable only at the high fusion dosage produced by retroviral transduction, and Mll-AF9 was expressed at higher levels in HSCs than in GMPs in the first place. The 'committed progenitor becomes a leukaemia stem cell' conclusion is therefore an artefact of the expression system in at least one careful test, and the cell-of-origin question is open rather than settled. Evidence that argues against this (1)
    • Assuming a myeloid state under environmental pressure in a mouse model and patients evading CD19 therapy by lineage switch are two observations joined by analogy. The murine system's "environmental pressure" is cytokine conditions in culture; the patients' pressure is sustained immunological attack on a single antigen. Both show plasticity and they do not establish that the same route is taken. Journal of Clinical Oncology 2025
