The state under menin inhibition — differentiation in AML, rapid death in ALL, and a GMP-like exit in the cells that switch lineage instead
also called menin inhibitor response state · revumenib-induced differentiation · differentiation therapy state · post-menin-release state
Menin inhibitors are usually described as drugs that let frozen leukaemia cells finish growing up. That is what happens in the myeloid form of the disease. In the lymphoid form — which is most infant cases — the same drug kills the cells quickly instead, and describing it as a differentiation therapy there is simply wrong.
The same thing, for a scientist
This graph, and the field, routinely describe menin inhibition as differentiation therapy. That description is **lineage-specific and it is the wrong one for the disease this project is mainly about**. Testing revumenib in vitro on KMT2A-rearranged ALL and AML side by side, Adriaanse et al. found that **ALL samples show rapid, dose-dependent induction of leukaemic cell death, whereas AML responses are much slower and promote myeloid differentiation** [E1]. The founding preclinical paper for this drug class is consistent and is usually read too loosely: VTP50469 displaced menin from protein complexes, inhibited MLL chromatin occupancy at select genes, and loss of MLL binding led to **"changes in gene expression, differentiation, and apoptosis"** — three outcomes, not one, with PDX models of both MLL-rearranged AML and MLL-rearranged ALL responding and **multiple mice engrafted with MLL-rearranged ALL remaining disease-free for more than a year** [E2]. A third state exists for the cells that do neither: in a sample that lineage-switched during revumenib treatment, oncoprotein and menin fell to undetectable levels while **ENL persisted on oncoprotein-target loci including the GMP-like lineage-switching programme** [E3]. So there are at least three exits from the fusion programme under this drug — differentiate, die, or switch — and which one a cell takes is not predicted by anything currently measured at diagnosis.
Why the AML/ALL difference is not a detail
If menin inhibition kills KMT2A-rearranged ALL rapidly rather than differentiating it, several inherited assumptions change at once:
- Differentiation syndrome is a toxicity of differentiation therapy. Its risk profile should not be assumed identical in the lymphoid setting, where the mechanism is different.
- Response kinetics and assessment timing differ: a slow differentiating response and a fast cytotoxic one need different assessment schedules, and a marrow at day 14 means different things.
- Combination logic inverts. A cytotoxic mechanism combines naturally with a pro-apoptotic agent; a differentiating mechanism does not necessarily. This is directly relevant to
cell-state-apoptotic-priming-bcl2— a drug that kills by apoptosis in ALL should be measured for BH3-profile effects, which nobody has done. - Lineage-destination worries shrink for ALL. This graph's
hyp-menin-release-lineage-destinationasks where a released blast goes; if the lymphoid blast dies, the question applies mainly to the myeloid and mixed-phenotype forms. That narrowing is already recorded as counter-evidence in that hypothesis and this entry is the state behind it.
Adriaanse also reports that acquired revumenib resistance in KMT2A-rearranged ALL arises either through MEN1 mutations or entirely independently of MEN1 mutation, and that revumenib and the DOT1L inhibitor pinometostat are significantly synergistic in KMT2A-rearranged ALL 1.
Reading the founding paper accurately
The Krivtsov result is the origin of the clinical programme and its abstract is precise in a way the field's shorthand is not. Loss of MLL binding led to changes in gene expression, differentiation, and apoptosis 2. Three outcomes are named and the paper does not claim that differentiation is the mechanism of eradication.
What the paper does establish strongly is depth of response in the lymphoid arm: multiple mice engrafted with MLL-rearranged ALL remained disease free for more than one year after treatment 2. A year of disease-free survival after a finite course is not a differentiation phenotype; it is eradication, and it is the single most encouraging preclinical fact in this graph about this drug class in lymphoid disease.
It also means the two papers agree: the ALL arm dies. E2 shows the depth, E1 shows the kinetics and the mechanism.
The third exit: reduced oncoprotein, persistent ENL, GMP destination
Profiling oncoprotein target sites across 36 MLL-rearranged leukaemia samples, including three that lineage-switched under therapy, Janssens et al. found that genomic enrichment of the oncoprotein is highly variable between samples and dynamically regulated; that at high expression the oncoprotein preferentially activates either an ALL programme enriched for pro-B genes or an AML programme enriched for HSC genes, with fusion-partner-specific binding patterns correlating with each mutation's prevalence in ALL versus AML; and that in lineage-switching samples the oncoprotein levels are reduced and the oncoprotein preferentially activates GMP genes 3.
The revumenib-specific observation is the sharpest: in a sample that switched during revumenib treatment, oncoprotein and menin were reduced to undetectable levels, but ENL persisted on numerous oncoprotein-target loci, including genes of the GMP-like lineage-switching programme 3.
That is a complete, on-target drug effect accompanied by disease persistence through a cofactor the drug does not touch — the strongest published version of the concern this graph tracks as the ENL tether. It also means 'menin inhibitor resistance' includes a class in which the drug worked perfectly.

What the title asserts is that menin inhibitors differ from one another in in vitro sensitivity in this specific disease. Per this project's rule, that is recorded and nothing further is inferred; in particular this entry does not state which inhibitors differ or in which direction. It matters because risk-menin-inhibitors-treated-as-a-class-in-infant-all is an existing entry in this graph and this is a title that bears on it directly.

]- Calling menin inhibition differentiation therapy in KMT2A-rearranged ALL is a mechanistic error with clinical consequences: it sets the wrong expectation for response kinetics, for marrow assessment timing, and for which agents combine sensibly.
- In the one profiled sample that lineage-switched under revumenib, the drug hit its target completely — oncoprotein and menin undetectable — and the disease continued through ENL. A resistance mechanism that operates despite perfect target engagement cannot be overcome by a better or more potent menin inhibitor.
- Acquired resistance in KMT2A-rearranged ALL arises both through MEN1 mutation and independently of it, so MEN1 sequencing at progression will miss a share of resistant patients and a next-generation inhibitor designed around the mutated pocket will not help them.
- The strongest in vivo evidence for the menin-plus-venetoclax combination is in an NPM1-mutated xenograft, not a KMT2A-rearranged one, and the differentiation rationale behind it is myeloid; importing it into infant ALL imports a mechanism that does not operate there.
- A paper reporting that menin inhibitors differ from one another in this exact population exists and cannot be read, so the class-interchangeability risk this graph records remains open with a known, inaccessible piece of evidence sitting on it.
- Is the ALL-dies/AML-differentiates split a lineage effect or a fusion-partner effect? Partner distribution differs systematically between the two diseases, so the published comparison confounds them; the same experiment run within one lineage across partners would separate them.
- Does menin inhibition change apoptotic priming in KMT2A-rearranged ALL? If the drug kills by apoptosis there, dynamic BH3 profiling before and after would predict which BH3 mimetic partners it and whether the combination is additive or redundant. It has not been done in lymphoid cells.
- How often does the third exit happen? Lineage switch under menin inhibition is documented in one profiled sample and in case reports; its incidence in the treated population is unknown and is the number that determines whether it is a curiosity or a class liability.
- Does differentiation syndrome occur in the lymphoid setting, where the mechanism appears not to be differentiation? The toxicity is managed on the assumption that it does, and the mechanistic basis for that assumption is absent.

Testing in vitro response to revumenib in KMT2A-rearranged ALL and AML, ALL samples showed rapid, dose-dependent induction of leukaemic cell death while AML responses were much slower and promoted myeloid differentiation; acquired resistance to revumenib in KMT2A-rearranged ALL cells arose either through acquisition of MEN1 mutations or independently of MEN1 mutation; and revumenib showed significant synergy with the DOT1L inhibitor pinometostat in KMT2A-rearranged ALL.

In vitro only; no in vivo arm and no patient outcomes, so the kinetic difference is a culture observation that has not been shown to correspond to clinical response patterns. Sample numbers per arm are not given in the abstract and primary KMT2A-rearranged samples are scarce. Fusion partner is not resolved, which matters because partner distribution differs systematically between the ALL and AML arms, so a lineage difference and a partner difference are confounded by construction. Ex vivo culture of primary lymphoblasts selects strongly and short-term viability assays favour detecting death over detecting differentiation. This citation was re-fetched from PubMed for this entry rather than copied from the many existing graph entries that cite it. Read: abstract only.
Evidence that argues against this (1)
- An ALL-kills/AML-differentiates dichotomy is complicated by a third outcome observed in patients rather than in culture. Profiling oncoprotein target sites across 36 MLL-rearranged samples including three that lineage-switched under therapy showed that in lineage-switching samples the oncoprotein level is REDUCED and it preferentially activates GMP genes, and in a sample that switched during revumenib treatment the oncoprotein and menin fell to undetectable levels while the cofactor ENL persisted on oncoprotein-target loci. A lymphoid blast that neither differentiates nor dies but becomes myeloid is a third exit, it occurred under this exact drug, and no in vitro viability assay of the kind used in E1 could have detected it. The potent, selective, orally bioavailable menin-MLL inhibitor VTP50469 selectively affected MLL-rearranged cell lines, displaced menin from protein complexes and inhibited chromatin occupancy of MLL at select genes, and loss of MLL binding led to changes in gene expression, differentiation and apoptosis; patient-derived xenografts from patients with either MLL-rearranged AML or MLL-rearranged ALL showed dramatic reductions in leukaemia burden on treatment, and multiple mice engrafted with MLL-rearranged ALL remained disease free for more than one year after treatment.

VTP50469 is the tool predecessor of revumenib, not revumenib itself. The abstract names differentiation and apoptosis together without apportioning the eradication between them or separating the AML and ALL arms on that point, so this paper does not by itself establish which mechanism operates in which lineage - that is E1's contribution. Mouse xenografts in immunodeficient hosts, so no immune contribution and no differentiation syndrome. 'Select genes' is not quantified in the abstract. Read: abstract only.
Evidence that argues against this (1)
- Depth of response in xenografts has not translated into comparable depth in lymphoid patients, and the class's clinical record is strongest in myeloid disease. This graph separately records that the only two complete remissions in the 51-patient adult pinometostat phase 1 were both t(11;19), on a drug that does not touch menin at all, and holds absence entries for partner-stratified and reciprocal-status-stratified response in menin inhibitor trials - meaning the clinical evidence that would test whether the lymphoid arm responds as deeply as these mice did has not been published in stratified form. A year of disease-free survival in NSG mice is a statement about a xenograft's dependence, not about a child's disease. Profiling oncoprotein target sites in 36 broadly representative MLL-rearranged leukaemia samples including three that underwent lymphoid-to-myeloid lineage switching in response to therapy showed that genomic enrichment of the oncoprotein is highly variable and dynamically regulated; at high levels the oncoprotein preferentially activates either an ALL programme enriched for pro-B-cell genes or an AML programme enriched for haematopoietic-stem-cell genes, with fusion-partner-specific binding patterns highly correlated with each mutation's prevalence in ALL versus AML; and in lineage-switching samples oncoprotein levels are reduced and the oncoprotein preferentially activates GMP genes, with one sample that switched during revumenib treatment showing oncoprotein and menin reduced to undetectable levels while ENL persisted on numerous oncoprotein-target loci including genes of the GMP-like lineage-switching programme.
MLL oncoprotein levels influence leukemia lineage identities · Janssens DH, Duran M, Otto DJ, Wu W, Xu Y, Kirkey D, et al., Henikoff S · Nat Commun 2024What this evidence cannot show
The revumenib lineage-switch observation is a single sample, and a single patient sample profiled by CUT&Tag cannot establish that ENL persistence caused the switch rather than accompanying it. Oncoprotein enrichment is measured with antibodies against the N-terminal MLL portion in the standard configuration for this method, which detects wild-type KMT2A as well as the fusion - the attribution problem this project has audited in detail - so 'oncoprotein levels' should be read as an occupancy measurement with a known specificity limit. Three lineage-switching samples out of 36 is a small denominator for a claim about what switching samples do in general. Read: abstract only.
Evidence that argues against this (2)
- The same result can be read as evidence against menin inhibitors specifically causing lineage switch. Lineage switch in KMT2A-rearranged leukaemia is documented after CD19-directed therapy and after conventional chemotherapy - a fact this note asserted without a source until 2026-09-20, when the Q40 counter-evidence quality sweep fetched one and added it as the note below - and the paper's general finding is that reduced oncoprotein level, from whatever cause, shifts preference toward GMP genes. On that reading the revumenib sample is one instance of a general dose-of-oncoprotein effect rather than a drug-class property, and menin inhibition would be no more lineage-switch-prone than any other intervention that lowers effective oncoprotein activity. Two of the three switching samples in this study did not switch on a menin inhibitor. Read: abstract only (same paper). Janssens DH et al. 2024
- INDEPENDENT SOURCE for the 'lineage switch after CD19-directed therapy' claim in the preceding note, added 2026-09-20 by the Q40 counter-evidence quality sweep. An infant with t(4;11)(q21;q23) ALL refractory to cytotoxic chemotherapy was given blinatumomab; after rapid clearance of peripheral lymphoblasts the marrow showed a lineage switch to CD19-NEGATIVE MONOBLASTIC AML, and remission followed myeloid-directed chemotherapy. This matters for the entry's argument because it is the index fusion, the index age group, and a drug that does not touch menin: it supports reading the revumenib lineage-switch observation as an instance of a general dose-of-oncoprotein or selection effect rather than a menin-inhibitor class property. Treatment of AML cell lines with MLL rearrangements or NPM1 mutations, other leukaemias and primary AML patient samples with the menin inhibitor DS-1594b plus venetoclax showed significant synergy with enhanced differentiation and inhibited proliferation measured by proliferation, viability, apoptosis and differentiation assays, western blotting and BH3 profiling; in an NPM1-mutated AML patient-derived xenograft, DS-1594b alone extended survival and the combination more profoundly reduced leukaemic burden and prolonged survival; and menin inhibition was the primary driver of transcriptional change in that model, affecting expression of anti-apoptotic regulators, which the authors give as the mechanistic explanation for the synergy.

Myeloid only, and the in vivo demonstration is in an NPM1-mutated rather than a KMT2A-rearranged xenograft, so the strongest in vivo evidence in the paper is for the genotype that is not this graph's subject. DS-1594b is not an approved agent. BH3 profiling is used, which makes this the closest published approach to measuring apoptotic priming under menin inhibition, but the abstract does not report a before-and-after priming result for KMT2A-rearranged cells specifically. Read: abstract only.
Evidence that argues against this (1)
- A differentiation-plus-venetoclax rationale developed in AML does not transfer to the lymphoid disease, where the same drug class induces rapid cell death rather than differentiation. If the menin inhibitor is already cytotoxic in KMT2A-rearranged ALL, the mechanistic basis claimed for synergy here - menin inhibition altering anti-apoptotic regulator expression to lower the threshold for a BH3 mimetic - may be redundant or may saturate, and the combination's benefit in the lymphoid setting cannot be assumed from the myeloid result. - This graph holds risk-menin-inhibitors-treated-as-a-class-in-infant-all, which argues that treating menin inhibitors interchangeably is unsafe.
