TOP2B - the isoform most etoposide-induced KMT2A breaks depend on, resolved genetically and not resolvable pharmacologically
also called TOP2B · DNA topoisomerase 2-beta · topoisomerase II beta · TOP2beta · topoIIbeta
TOP2B is one of two nearly identical enzymes that cut both strands of DNA to untangle it and then sew them back up. It is the one whose loss stops most of the DNA breakage that etoposide causes at the KMT2A gene, and unlike its twin it is switched on in cells that are not dividing.
The same thing, for a scientist
TOP2B (Q02880, 1626 aa) is the non-proliferation-associated type II topoisomerase paralogue of TOP2A. Genetic depletion of TOP2B removes most etoposide-induced chromosome breaks at the KMT2A locus and most of the overall genotoxicity of etoposide in human cells [E2]; CTCF/cohesin-bound loop anchors are described specifically as vulnerable to TOP2B-mediated DSBs [E3]. Two things bound that attribution and this entry exists to state them. First, **formation of the productive fusion requires both isoforms**, not TOP2B alone [E4] - so the isoform that makes the break is not necessarily the isoform that makes the leukaemia. Second, and more importantly for anyone reading an isoform claim as a therapeutic one, **the isoform resolution is entirely genetic**. Every clinical and dietary TOP2 poison is pan-isoform: ChEMBL files etoposide's mechanism against a PROTEIN FAMILY target comprising P11388 and Q02880 with no member-level entry at all [E5]. This is the fourth time this graph has met the pattern it already generalises for CK2, BET and HDAC - where a family shares a conserved catalytic pocket, the chemistry is family-selective and every member-level claim made from a drug experiment is naming rather than measurement. TOP2B is an exception only because the KMT2A result was obtained by knockdown, not by a compound.
Which isoform, and how firmly1 deeper
The claim as usually stated: TOP2B is the isoform implicated in etoposide-induced KMT2A translocation.
What actually supports it: Cowell et al. depleted TOP2B genetically and found that most etoposide-induced chromosome breaks in the MLL locus, and the overall genotoxicity of etoposide, were TOP2B-dependent 2. Canela et al. attribute loop-anchor fragility specifically to TOP2B 3. Both are genetic attributions in human cell lines.
What limits it: the same Cowell paper reports that TOP2A and TOP2B occupancy and measured etoposide-induced cleavage do not explain where within the breakpoint cluster region the breaks land 2 - so the isoform answers which enzyme and not which position. And Gothe et al. find that fusion formation at the recurrent hotspots requires both isoforms plus transcriptional elongation 4. The honest summary is: TOP2B dominates the cutting, both isoforms are needed for the product, and the field's habit of writing 'topoisomerase II' unresolved is defensible for any statement made from drug data.
The pharmacology cannot resolve the isoform at all
ChEMBL's curated mechanism record for etoposide (CHEMBL44657) points at target CHEMBL2094255, whose target_type is PROTEIN FAMILY and whose components are exactly P11388 (TOP2A) and Q02880 (TOP2B) 5. There is no separate member-level mechanism row. The same is true of every epipodophyllotoxin, anthracycline and dietary bioflavonoid in this literature. So a sentence of the form 'TOP2B-selective inhibition would prevent therapy-related KMT2A-r leukaemia' describes a molecule that does not exist, and no published experiment in this disease separates the isoforms with a compound.
This graph's standing generalisation applies verbatim: when a family shares a conserved catalytic pocket, the inhibitor class is family-selective, not member-selective, and every member-level claim is naming rather than measurement. The TOP2B attribution survives it only because it was made by depletion.
Why the isoform difference is the interesting part for a fetus
TOP2A is a proliferation enzyme and TOP2B is not. In human foetal tissues, in-situ hybridisation shows TOP2A mRNA strongest in zones of proliferation while TOP2B is expressed across a broader range of cell types, and the two patterns are distinct enough that the authors conclude differential regulation and different developmental roles 6. In the early human foetal telencephalon the separation is explicit: TOP2A tracks KI67 and the G2/M fraction, while TOP2B is expressed in post-mitotic compartments with co-expression with KI67 rare 7.
That matters here because the de novo infant disease arises in a fetus with no epipodophyllotoxin exposure. An enzyme that is active in non-cycling cells and coupled to transcription rather than replication is exactly the enzyme that could cut a transcribed locus in a quiescent fetal progenitor. Nobody has measured TOP2A or TOP2B protein, occupancy or cleavage at the KMT2A BCR in a human fetal liver HSC. Both cited expression studies cover brain, kidney, liver, gut, lung and placenta; neither resolves haematopoiesis.
Loss of function
TOP2B is required for development of post-mitotic tissue: Top2b loss impairs proper retinal development and the survival of post-mitotic cells 8. That is the essential-function floor under any proposal to inhibit this isoform, and it sits underneath the delivery problem rather than beside it - the window in which a preventive agent would have to act is fetal (see concept-prevention-window-kmt2a-in-utero).

| shown for fusion | PARTIAL, and it is the strongest fusion-relevant spatial result in this tranche. What is shown is not the localisation of TOP2B in a KMT2A-rearranged cell but the localisation of the KMT2A LOCUS relative to its partner loci, in cells where TOP2B makes the breaks: 2-3% of transcribing MLL alleles sit close to AF9 or AF4, consistent with shared transcription factories, and most etoposide-induced MLL breaks are TOP2B-dependent. That is a pre-leukaemic, translocation-genesis measurement in normal cells, NOT a measurement in an established KMT2A-rearranged blast. No imaging or fractionation of TOP2B protein has been performed in a KMT2A-rearranged leukaemia cell. (TOP2B OR "topoisomerase II beta" OR "topoisomerase IIbeta")[tiab] AND (MLL OR KMT2A)[tiab] returns 6 records. Zero in titles and abstracts; supporting figures not searched. |
| identifier trap | TOP2A (P11388, 1531 aa) versus TOP2B (Q02880, 1626 aa): a 95 aa gap, so accession-plus-length catches a swap - but the biology does not. TOP2B carries most etoposide-induced MLL-locus breaks while TOP2A is the proliferation-associated isozyme and the usual immunohistochemical marker, and a large literature writes only 'topoisomerase II'. The spellings 'topoisomerase II beta', 'topoisomerase IIbeta' and 'TOP2B' are three different PubMed strings and all three were run. Verified live against UniProt REST 2026-09-21. |
| epigenetic readout | NO MARK IS WRITTEN, ERASED OR READ, AND THE ARCHITECTURAL READOUT IS THE ONE THAT MATTERS HERE. THE DOMAIN ARGUMENT, FETCHED LIVE 2026-09-21: UniProt Q02880, entry name TOP2B_HUMAN, 1,626 aa, Toprim domain at 476-593 and Topo IIA-type catalytic domain at 736-1189, with a Pfam complement IDENTICAL to TOP2A's - PF00204, PF00521, PF08070, PF02518, PF01751, PF16898 - and no SET, JmjC, HAT, HDAC, bromodomain, chromodomain, Tudor, PHD, CXXC, YTH or DNMT module. (The identical Pfam sets are worth noting for this entry's own identifier_trap: domain composition cannot separate the paralogues either, only length, 1,626 against 1,531.) It cannot modify or read chromatin, and no histone or DNA modification has been measured under TOP2B depletion in anything read for this entry. WHAT THIS ENTRY ALREADY HAS, AND IT IS A CHROMATIN CLAIM WITHOUT BEING A MARK CLAIM: CTCF and COHESIN-BOUND LOOP ANCHORS are described specifically as vulnerable to TOP2B-mediated double-strand breaks [E3 here]. That is an epigenomic-architecture readout - where in the three-dimensional genome the enzyme cuts - and it is the reason the honest answer for TOP2B is 'no mark, but not no chromatin'. It also makes an unexpected junction with gene-mettl3 in the same tranche, where METTL3 depletion DISRUPTS CTCF BINDING and reduces accessibility at MYB and RUNX1 in MLL-rearranged AML: one entry says an RNA modification holds those anchors together and the other says a topoisomerase breaks DNA at them. Nobody has put the two in one experiment. AND THE ONE PAPER THAT PAIRS A TOPOISOMERASE WITH A HISTONE MARK NAMES THIS ISOFORM IN ITS TITLE: Taspase1 facilitates topoisomerase-II-beta-mediated transient double-strand breaks driving oestrogen-induced transcription, and in the same work Taspase1 alters the H3K4 epigenetic signature by cleaving MLL [E_q97_epi1]. The mark is MLL's, not TOP2B's - but the break step and the H3K4 step are coupled in one programme, which is the closest thing to a chromatin-mark readout either topoisomerase has. Note the isoform hazard: that abstract names TOP2B in the title and TOP2A in its specific co-IP sentence, and this entry's own identifier_trap field already records why the two are hard to separate. SEARCH FINDING, AND IT IS WHY THE DEFAULT ANSWER WOULD HAVE BEEN WRONG: TOP2B[tiab] AND H3K4[tiab] returns 0, as does TOP2A[tiab] AND H3K4[tiab], while topoisomerase[tiab] AND H3K4me3[tiab] returns 7 and contains the paper. Both TOP2 entries would have been written as bounded absences on the symbol form alone. Bounded absence and full title screen recorded in gene-top2a.data.epigenetic_bounded_absence; not duplicated here. |

}- TOP2B is transcription-coupled and required for post-mitotic cell survival. A TOP2B-directed agent would therefore hit exactly the non-dividing tissue that conventional cytotoxics spare, which is the opposite of the usual therapeutic index for this target class.
- The only window in which TOP2B inhibition could prevent a de novo infant KMT2A rearrangement is fetal, and a fetus is the worst possible recipient of an agent that blocks transcription-coupled decatenation in developing post-mitotic tissue.
- If a TOP2B-sparing etoposide analogue were built to reduce secondary leukaemia, it would first have to be shown not to lose anti-tumour efficacy - and the anti-tumour activity of etoposide has never been isoform-attributed either. Sparing TOP2B might spare the cure.
- the therapy creates the lesion - Etoposide's genotoxicity and most etoposide-induced MLL-locus breaks are TOP2B-dependent, and transcribing MLL alleles sit near AF9 or AF4 in 2-3% of cells. Etoposide remains in this graph's regimens. A TOP2B-sparing or TOP2A-selective poison would decouple efficacy from translocation risk, and no such agent is in clinical use.
- a coupling nobody has tested - If the nucleolar TOP2B pool is etoposide-protected, the same relocation would protect both the cell from cytotoxicity and the MLL locus from breakage - so chemoprotection and leukaemogenesis-protection may be the same event. This is cheap to test and has not been.
- Is TOP2B protein present, and is it engaged at the KMT2A breakpoint cluster region, in a human fetal liver HSC or LMPP? Every isoform statement in the de novo disease currently rests on extrapolation from brain and from transformed lines.
- Does any compound separate TOP2A from TOP2B at all in cells? If the answer is genuinely no, then the 'TOP2B-sparing regimen' idea in this graph's open questions is a medicinal-chemistry programme and not a schedule change, and should be labelled as such.
- If both isoforms are needed to make the fusion (E4) but only TOP2B is needed to make the break (E2), what does TOP2A contribute - the second break, the partner-side break, or the joining geometry? Nobody has asked which end of the translocation each isoform supplies.
- Is the 2-3% MLL-AF4 spatial proximity figure the same in a human fetal liver HSC or pro-B progenitor - the cell of origin for infant t(4;11) - as in the transformed cells where it was measured? The whole in-utero translocation model turns on that number and it has never been measured in the right cell.
- Do CTCF/cohesin loop anchors that lose METTL3-dependent m6A become more or less vulnerable to TOP2B breakage? gene-mettl3 and this entry reach the same anchors from opposite directions and the combination has never been tested - it is a single-experiment question with a therapy-related-leukaemia hazard attached.
- Is TOP2B cleavage at the KMT2A breakpoint cluster region chromatin-state dependent? No histone mark, accessibility measure or methylation call has been correlated with cleavage position there in anything read for this entry.

A database curation fact, and this graph's standing caution applies: a ChEMBL record measures what medicinal chemistry has been done and how a curator filed it, not what selectivity is achievable. Absence of a member-level row is not proof that no isoform-selective compound could exist; it is evidence that none is curated, which is the weaker and still decision-relevant statement.

Radiolabelled in-situ hybridisation, mRNA not protein, semi-quantitative intensity comparison. Organs surveyed were kidney, brain, small intestine, liver, lung and placenta. Fetal liver HAEMATOPOIESIS is not resolved - the liver signal is attributed to hepatocytes - so this study does not answer the question this beat most wants answered.
Evidence that argues against this (1)
- mRNA in-situ intensity is not protein and not catalytic engagement, and the ranking of the two isoforms can invert with cellular state: HSF1 has been reported to stimulate the catalytic engagement of TOP2B over TOP2A, so which isoform is cutting is a function of signalling and not only of transcript abundance. In the early human foetal telencephalon at 9-12 post-conceptional weeks, TOP2A immunoreactivity is restricted to proliferative layers and closely matches KI67, whereas TOP2B is expressed in both proliferative and post-mitotic compartments with co-expression with KI67 rare.

Human fetal BRAIN. Transferring the isoform-versus-cell-cycle relationship to a fetal liver haematopoietic progenitor is an assumption, not a measurement, and it is the assumption this entry flags as untested.
Evidence that argues against this (1)
- The same authors attribute TOP2B's post-mitotic role to transcription of LONG genes in neural development. KMT2A is a long gene, which makes the extrapolation attractive - and attractive extrapolations in this literature are exactly what produced confident wrong attributions before, since TOP2 occupancy does not predict breakpoint position even where TOP2 makes the break. Cited from the PubMed record title and its indexing only; the full text was not read in this session, so no quantitative phenotype is asserted. It is used solely to mark that an essential post-mitotic function exists, which is a floor under any inhibition proposal and does not require the numbers.
Evidence that argues against this (1)
- A developmental requirement in a knockout animal is a statement about complete lifelong loss from conception, not about partial reversible pharmacological occupancy in an existing organism - this graph's standing caution on constraint metrics. It bounds a prophylactic fetal strategy specifically, which is the only strategy anyone has proposed here, so the caveat does not rescue the proposal. UniProt Consortium 2026

2-3% of MLL alleles undergoing transcription do so in close nuclear proximity to one of the recurrent translocation partner genes, AF9 or AF4, consistent with their sharing transcription factories; most etoposide-induced chromosome breaks in the MLL locus and the overall genotoxicity of etoposide are dependent on topoisomerase II beta; and topoisomerase II alpha and beta occupancy plus etoposide-induced cleavage data indicate that factors other than local topoisomerase II concentration determine the specific breakpoint clustering.
Model for MLL translocations in therapy-related leukemia involving topoisomerase IIβ-mediated DNA strand breaks and gene proximity. · Cowell IG, et al. · Proc Natl Acad Sci U S A 2012;109:8989-94 2012What this evidence cannot show
This measures where the LOCI are, not where TOP2B protein is, and it is about therapy-related AML translocation genesis in normal cells - not about an established KMT2A-rearranged blast (brief rule 73 cuts the other way here: the system is the right locus in the wrong disease stage). The 2-3% figure is a population frequency from 3D FISH and carries the usual fixation caveats. The authors explicitly state local TOP2 concentration does NOT explain breakpoint clustering, so this entry does not claim it does. Abstract only.
Evidence that argues against this (1)
- A transcription-factory interpretation of two loci being near each other is exactly the model this graph's condensate standard warns about: proximity measured by fixed-cell FISH cannot distinguish a shared factory from independent co-localisation at the same nuclear radius, and the authors' own finding that TOP2 occupancy does not predict breakpoint position undercuts a simple 'factory plus topoisomerase' account. Human non-leukaemic cells. A TAD-border binding pattern is a genomic address; it becomes a subnuclear statement only under an assumption about where TAD borders sit, which this entry does not make. Abstract only.
Evidence that argues against this (1)
- The same protein has a large, inducible nucleolar pool that is not at TAD borders at all and that is abolished within minutes by BMH-21, so a TAD-border description of TOP2B accounts for only one of its states. Morotomi-Yano K et al. 2021

ATP depletion rapidly translocates TOP2B from the nucleoplasm to the nucleolus, where FRAP shows it remains highly mobile, and nucleolar retention is abolished by the RNA polymerase I inhibitor BMH-21; under ATP depletion the TOP2 poison etoposide is less effective.

Human non-leukaemic cells and a non-physiological perturbation. Shared with the TOP2A entry: the paper reports both isozymes and does not separate their etoposide sensitivities. Abstract only.
Evidence that argues against this (1)
- If the nucleolar pool is etoposide-protected, then the same relocation that spares a cell from etoposide cytotoxicity would also spare the MLL locus from TOP2B-dependent breakage - so the nucleolar state is predicted to be simultaneously chemoprotective and leukaemogenesis-protective, a coupling no study has tested and which would be cheap to test. In murine NIH-3T3 cells, the 170 kDa form of topoisomerase II (TOP2A) is undetectable until late S phase, peaks in G2-M and falls through mitosis, and becomes undetectable once all cells reach G0 after serum starvation, whereas the 180 kDa form (TOP2B) is constant once cells enter the cycle and is still present after all cells have entered G0; the 180 kDa enzyme also requires a higher NaCl concentration than the 170 kDa enzyme to extract from isolated nuclei.

Abstract read in full; full text not read. Murine NIH-3T3 fibroblasts, not haematopoietic and not human. The paper names the proteins by molecular weight only (170/180 kDa); the assignment to TOP2A and TOP2B is the standard identification and is stated here as an identification, not as the authors' wording. Immunoblot on synchronised populations - a population-average measurement, not single-cell. No DOI exists for this 1991 record and none is stored.
Evidence that argues against this (1)
- The same data show the 180 kDa enzyme is CONSTANT across the cycle rather than enriched in quiescence, so the paralogue difference is that TOP2A disappears from G0, not that TOP2B is a G0-specific protein. Top2b-mutant mice moreover have normal neurogenesis, i.e. normal proliferation, so TOP2B is dispensable in the dividing compartment. Yang X et al. 2000

In MICE, DNA topoisomerase IIbeta has a critical role in neural development: neurogenesis is normal in IIbeta-mutant mice, but motor axons fail to contact skeletal muscles and sensory axons fail to enter the spinal cord, and the resulting defects in motor axon growth cause breathing impairment and death of the pups shortly after birth.

Abstract read in full; full text not read. Murine germline mutant, neural and neuromuscular phenotype, nothing haematopoietic and nothing about leukaemia. A germline developmental requirement says nothing about whether pharmacological TOP2B poisoning in an adult would produce the same phenotype.
Evidence that argues against this (1)
- Normal neurogenesis in the same mutant bounds the claim tightly: TOP2B is not required to MAKE neurons, only for the postmitotic axon-growth step. Any inference from this paper to a general TOP2B requirement in a proliferating tissue runs against the authors' own control. Woessner RD et al. 1991

Taspase1 interacts specifically with topoisomerase II alpha, enhancing formation of the DNA double-strand breaks that are a prerequisite for oestrogen-stimulated transcription, and functionally links Taspase1 to hormone-induced, topoisomerase II beta-mediated transient double-strand breaks; Taspase1 ALTERS THE H3K4 EPIGENETIC SIGNATURE upon oestrogen stimulation BY CLEAVING THE CHROMATIN-MODIFYING ENZYME MLL, and both oestrogen-driven transcription and MLL-derived epigenetic labelling are reduced on Taspase1 siRNA knockdown.

Abstract fetched and read in full 2026-09-21; full text not read. OESTROGEN-RESPONSIVE cells, not haematopoietic and not leukaemia (rule 73) - the relevance is that Taspase1 and MLL cleavage are this graph's own central biology (gene-tasp1), not that the system resembles infant ALL. The abstract is INTERNALLY MIXED ON ISOFORM: the title and the general claim are TOPOISOMERASE II BETA, while the specific co-immunoprecipitation sentence names TOPOISOMERASE II ALPHA - so both isoforms appear and this entry does not resolve which carries which role. Methods quoted are co-immunoprecipitation, LC-MS/MS and topoisomerase II DNA-cleavage assays. The H3K4 claim is a SIGNATURE-LEVEL statement ('alters the H3K4 epigenetic signature'); no methylation state and no locus is named in the abstract, so neither is claimed here.
Evidence that argues against this (1)
- The H3K4 change in this paper is attributed to Taspase1 CLEAVING MLL, not to topoisomerase activity, so it is not evidence that a topoisomerase writes, erases or reads a mark - which it does not. What the paper supports is that the TOP2-dependent break step and the MLL-dependent H3K4 step are coupled in one hormone-induced transcription programme. Reading it as 'TOP2A/TOP2B has a chromatin-mark readout' would be an attribution error of exactly the kind this graph's rule 89 is about: a link near the thing is not the thing. The honest statement is that the mark sits one protein away, and that protein is KMT2A. Cells 2023

the 180 kDa topoisomerase II (TOP2B) is still present after every cell has entered G0, where the 170 kDa form is undetectable (murine NIH-3T3; PMID 1651102)
the two paralogues are separated by cell state, not only by sequence: TOP2A is confined to late S/G2-M and vanishes in G0 while TOP2B persists there (PMID 1651102). Distinct drug biology follows.
Taspase1 is functionally linked to hormone-induced, topoisomerase-II-beta-mediated transient double-strand breaks; the coupled H3K4 change is deposited by MLL after Taspase1 cleavage
both entries converge on CTCF/cohesin loop anchors - TOP2B breaks DNA at them, METTL3 loss disrupts CTCF binding at them - and the interaction is untested
