TOP2A - the proliferation isoform, the one the drugs are actually measured against, and the only one with a germline association in this disease
also called TOP2A · TOP2 · DNA topoisomerase 2-alpha · topoisomerase II alpha · topoIIalpha
TOP2A is the dividing-cell version of the enzyme that cuts and reseals both strands of DNA. It is not the isoform usually blamed for the DNA break that makes KMT2A leukaemia, but it is the one that does most of the enzyme's work in a proliferating cell, and it is the one whose inherited variants have been looked at in children who develop this disease.
The same thing, for a scientist
TOP2A (P11388, 1531 aa) is the replication- and mitosis-associated type II topoisomerase, distinguished from TOP2B by a 95-residue length difference and by an expression pattern confined in fetal tissue to zones of proliferation [E4, and `gene-top2b` E6/E7]. Three things place it in this beat despite TOP2B carrying the headline attribution. (1) **The fusion needs it.** Gothe et al. report that the chromosomal fusions arising at recurrent TOP2 hotspots depend on the activity of *both* isoforms [E2] - TOP2B-dependence of the break does not make TOP2A a bystander in the product. (2) **The patient junctions were cut by it in vitro.** In a therapy-related t(4;11) where both genomic junctions were sequenced, DNA substrates containing the normal homologues of the patient's MLL and AF-4 breakpoints were cleaved by human topoisomerase II **alpha**, with cleavage enhanced by etoposide and its catechol and quinone metabolites [E3]. That is the only isoform-resolved cleavage assay run on actual patient breakpoint sequence in this disease, and it names alpha. (3) **It is the only TOP2 gene with a germline-variant study in KMT2A-associated childhood leukaemia**, and that study's result for TOP2A was negative [E5]. Taken together the isoform question in this literature is **not resolved in one direction**: genetic depletion says beta for the break, biochemistry on patient sequence says alpha can cut it, and fusion formation needs both.
Does the literature actually resolve the isoform? No - and it resolves differently depending on the assay

Five lines of evidence, four different answers. The generalisation this graph already holds for CK2, BET and HDAC - that a shared catalytic pocket makes the chemistry family-selective and every member-level claim from a drug experiment naming rather than measurement - applies here with a twist: here the genetics also disagree with the biochemistry, so it is not only the pharmacology that fails to resolve the isoform.
What this beat therefore recommends: write 'topoisomerase II' unless the sentence is about a depletion experiment, in which case name the isoform that was depleted and the system it was depleted in.
The germline-variant study, and why its TOP2A result is a bounded negative rather than nothing
Piwkham et al. genotyped 52 tagged SNPs across TOP2A, MLL, CREBBP and EP300 in 241 self-identified Hispanic samples (66 childhood ALL, 175 matched controls), explicitly motivated by the higher observed incidence of therapy-related AML with 11q23 translocations after topoisomerase II inhibitor treatment in Hispanic children 5. Two MLL SNPs and three EP300 SNPs were associated with ALL; no TOP2A SNP was. A replication set of 137 affected White children and 239 controls retained only one MLL SNP.
That is a real negative for common germline TOP2A variation as a susceptibility factor in this population, and it is worth recording because the field's intuition runs the other way. It is also small, single-population, tag-SNP based and about ALL susceptibility rather than about therapy-related AML, which is the outcome the hypothesis was built from. An absence of testing is not a negative result: nobody has genotyped TOP2A or TOP2B against therapy-related KMT2A-r AML as the endpoint.
What TOP2A is and is not in this graph
TOP2A is not a therapeutic target proposal here. It is a common-essential mitotic enzyme; the standard anti-cancer use of TOP2 poisons is to kill proliferating tumour cells through it, and this beat's interest is in the collateral break at KMT2A rather than in blocking the enzyme. The entry exists because the isoform question is repeatedly asserted without evidence in this literature and the graph now has the table above to point at.

| shown for fusion | NO. No imaging, fractionation or subnuclear measurement of TOP2A has been performed in a KMT2A-rearranged cell, and the closest TOP2-and-MLL literature is about TOP2B rather than TOP2A. This absence has a therapeutic edge: under ATP-depleted conditions, when TOP2 is nucleolar, the TOP2 poison ETOPOSIDE IS LESS EFFECTIVE - and etoposide is both a standard-of-care drug in this graph's regimens and the agent implicated in creating KMT2A translocations in the first place. Where TOP2A sits therefore predicts how well an etoposide dose works, and nobody has measured it in this disease. Zero in titles and abstracts; supporting figures not searched. |
| identifier trap | TOP2A (P11388, 1531 aa) and TOP2B (Q02880, 1626 aa) are paralogues 95 aa apart, so verify-accessions.py CAN catch a swap - but the drug biology does not follow the sequence similarity. TOP2B, not TOP2A, carries most etoposide-induced breaks at the KMT2A locus; both are nucleolar under ATP depletion; and the two are routinely written as 'topoisomerase II' with no isozyme named. Verified live against UniProt REST 2026-09-21. |
| epigenetic readout | NO MARK IS WRITTEN, ERASED OR READ - AND YET THE ANSWER IS NOT 'NOT APPLICABLE', WHICH IS WHY THE CHECK MATTERED. THE DOMAIN ARGUMENT, FETCHED LIVE 2026-09-21: UniProt P11388, entry name TOP2A_HUMAN, 1,531 aa, feature table carrying a Toprim domain at 455-572 and a Topo IIA-type catalytic domain at 715-1171, with a Pfam complement of PF00204 (DNA_gyraseB), PF00521 (DNA_topoisoIV), PF08070 (DTHCT), PF02518 (HATPase_c), PF01751 (Toprim) and PF16898 (TOPRIM_C) - SIX FAMILIES, ALL TOPOISOMERASE OR ATPASE, and NO SET, JmjC, HAT, HDAC, bromodomain, chromodomain, Tudor, PHD, CXXC, YTH or DNMT module. There is no mechanism by which this protein could modify or read a histone or DNA modification. THE CHROMATIN-MARK READOUT THAT DOES EXIST SITS ONE PROTEIN AWAY, AND THE PROTEIN IS THIS GRAPH'S CENTRAL ONE. Taspase1 interacts specifically with topoisomerase II alpha and enhances the formation of the transient double-strand breaks that hormone-induced transcription requires; in the SAME work, Taspase1 ALTERS THE H3K4 EPIGENETIC SIGNATURE by cleaving MLL, and both the transcription and the MLL-derived epigenetic labelling fall on Taspase1 knockdown [E_q97_epi1]. That places TOP2A in the same hormone-induced transcription mechanism as the taspase1-MLL cleavage this whole project argues about - but the mark is deposited by MLL, not by TOP2A, and this entry says so rather than borrowing the credit. THE IDENTIFIER FINDING IS THE REPORTABLE PART. TOP2A[tiab] AND H3K4[tiab] returns 0. TOP2B[tiab] AND H3K4[tiab] returns 0. topoisomerase[tiab] AND H3K4me3[tiab] returns 7, and the Taspase1 paper is in it. A SYMBOL-DRIVEN SEARCH FOR THIS BIOLOGY RETURNS A CLEAN ZERO ON BOTH ISOFORMS while the ENZYME-CLASS NOUN finds the paper, because the title and abstract write 'Topoisomerase II alpha' and 'Topoisomerase IIbeta' in words and never the HGNC symbols. This is rule 71's shape - a pathway named by something other than its gene symbol - and it would have produced a bounded absence for both TOP2 entries had the class noun not been tried. WHAT IS STILL ABSENT, BOUNDED BELOW: no histone or DNA modification has been measured at the KMT2A locus, or anywhere, under TOP2A depletion or under etoposide, in anything read for this entry. The breakage mechanism this entry exists for is a DNA-topology mechanism, and whether the chromatin state of the KMT2A breakpoint cluster region influences TOP2A cleavage there is one of the more obvious unasked questions in this graph. |

}- TOP2A is a common-essential mitotic enzyme and inhibiting it is the intended mechanism of a large fraction of curative paediatric chemotherapy. Nothing in this entry is a proposal to target it, and reading the break biology as a reason to avoid TOP2-directed therapy would be a serious misreading.
- If the fusion requires both isoforms (E2), then a hypothetical TOP2B-sparing agent would not necessarily abolish therapy-related KMT2A rearrangement, because TOP2A would still be poisoned by it. The isoform-sparing idea has to specify which isoform's activity the anti-tumour effect needs, and no one has measured that.
- drug efficacy is compartment-dependent - Under ATP depletion TOP2 relocates to the nucleolus and etoposide becomes LESS effective. A metabolically stressed or hypoxic marrow blast is the untested in vivo analogue, which would make a compartment shift a mechanism of etoposide resistance that no sequencing-based resistance assay could detect.
- TOP2A protein is undetectable in G0 (PMID 1651102). A TOP2A-directed poison therefore has no target in a quiescent leukaemic cell, which is the mechanistic form of the persister problem this graph records as cell-state-quiescence and cell-state-relapse-initiating-persister: the drug selects for the cells it cannot see. Anthracyclines and etoposide are given to every infant on these protocols, so the escape route is not hypothetical.
- The DepMap numbers on this entry cannot be used to argue TOP2A selectivity in either direction. Both arms sit past -2 (KMT2A-rearranged -2.1802, other leukaemia -2.0730, pan-essentiality -2.536) and the entry's own how_to_read block says a rank computed in that regime is uninterpretable. Quoting rank 983 of 17,728 as though it meant TOP2A is KMT2A-selective would be exactly the floor-compression error the brief names.
- Which isoform cleaves the KMT2A BCR in a cell that is not cycling? The only isoform-resolved cleavage assay on patient breakpoint sequence used TOP2A on naked DNA (E3) and the only depletion experiment in chromatin implicated TOP2B. Nobody has run a TOP2A degron and a TOP2B degron side by side at this locus.
- Has anyone genotyped TOP2A or TOP2B against THERAPY-RELATED KMT2A-rearranged AML as the endpoint, rather than against de novo ALL susceptibility? The only germline study in this graph used the wrong outcome for its own stated motivation.
- Gothe 2019 says both isoforms are needed for the fusion. Is that because one cuts each partner locus? A two-colour isoform-degron experiment with a bait-and-prey break map would answer it and has not been attempted.
- Does the nucleolar sequestration of TOP2A and TOP2B occur in leukaemic blasts under physiological metabolic stress rather than experimental ATP depletion, and does it confer etoposide resistance there? That would be a resistance mechanism invisible to every sequencing-based assay.
- Does the chromatin state of the KMT2A breakpoint cluster region influence TOP2A cleavage there? This entry's whole rationale is TOP2-mediated breakage at that locus and no histone mark, accessibility measure or DNA-methylation call has been correlated with cleavage position in anything read for it.
- Which TOP2 isoform does Taspase1 actually engage? PMID 36766705 names TOP2B in its title and general claim and TOP2A in its specific co-immunoprecipitation sentence, and this graph holds a recorded identifier trap for this exact paralogue pair - so the isoform attribution in the one paper linking taspase1 to topoisomerases is unresolved from the abstract.

Evidence that argues against this (1)
- The synonym TOP2 on this record is itself the trap: a paper that writes 'TOP2' cannot be assigned to P11388 by symbol matching, and pre-1990 literature predates the cloning of the beta isoform entirely. In a therapy-related t(4;11) ALL with both genomic junctions sequenced, DNA substrates containing the normal homologues of the patient's MLL and AF-4 breakpoints were cleaved by human topoisomerase II alpha, with cleavage enhanced by etoposide and by etoposide catechol and etoposide quinone but not by dactinomycin.

A single patient, and an in-vitro cleavage assay on naked DNA substrates with purified enzyme. The choice of the alpha isoform in that assay reflects reagent availability in 2001 as much as a biological claim; the experiment shows alpha CAN cleave these sequences, not that alpha DID cleave them in the patient. That distinction is exactly the one this entry is built around.
Evidence that argues against this (1)
- Direct mapping of etoposide-induced DSBs across the MLL breakpoint cluster region placed them at the Pol II binding site and the transcription initiation region of a gene-internal MLL promoter and explicitly NOT at a nearby topoisomerase II consensus sequence, attributing MLL instability to transcriptional processes. A functional cleavage site on naked DNA and the position of breaks in chromatin are therefore not the same measurement. Scharf S et al. 2007

In-situ hybridisation for mRNA, semi-quantitative, and haematopoiesis is not resolved in any surveyed organ including fetal liver.
Evidence that argues against this (1)
- A proliferation-restricted expression pattern does not exclude a role in a quiescent cell, because TOP2A expression is inducible and its promoter is actively regulated - a CCAAT-binding regulator of TOP2A expression was characterised precisely because the gene's level is not a fixed property of a cell type. In a case-control study of 66 childhood ALL cases and 175 matched controls of self-identified Hispanic origin genotyped at 52 tagged SNPs across TOP2A, MLL, CREBBP and EP300, two MLL SNPs and three EP300 SNPs were significantly associated with ALL and no TOP2A SNP was.

Small (66 cases), one self-identified ethnic group, tag-SNP coverage rather than sequencing, and the endpoint is ALL susceptibility rather than the therapy-related 11q23 AML that motivated the design. The authors themselves call for replication. Reading a null for TOP2A off a study powered on 66 cases is a weak negative, and it is recorded here as a bounded one.
Evidence that argues against this (1)
- The same paper's positive findings were population-specific: replication in 137 affected White children and 239 controls retained only rs6589664 in MLL. A null for TOP2A in one population therefore carries little information about another, and the authors state the association between ALL and these variants is population specific. TOP2A in human cells exists in a dynamic equilibrium between sequestration in the nucleolus, substrate searching in transcription hubs, and active engagement on chromatin; the equilibrium is highly responsive to changes in DNA topology, and MYC accelerates TOP2A diffusion by limiting its sequestration.
MYC modulates TOP2A diffusion to promote substrate detection and activity. · Cameron DP, et al. · Nat Commun 2026;17 2026What this evidence cannot show
Single-molecule tracking in human non-leukaemic cells. 'Transcription hubs' is the authors' term and this graph's condensate standard applies: a hub inferred from diffusion behaviour is not a visualised compartment. MYC is a KMT2A-fusion target gene, so the link to this disease is by inference and not by experiment. Abstract only.
Evidence that argues against this (1)
- A second study reaches the nucleolar state by a completely different route - ATP depletion, not MYC - and shows the nucleolar pool is rDNA-transcription-dependent and reversible within minutes by BMH-21, which means 'nucleolar TOP2A' is a metabolic state and not a stable address, and a MYC-centric account of it is one mechanism among several. ATP depletion rapidly translocates both TOP2A and TOP2B from the nucleoplasm into the nucleolus; FRAP shows both proteins are highly mobile in nucleoplasm and nucleolus; nucleolar retention of both is sensitive to the RNA polymerase I inhibitor BMH-21, which immediately disperses them back to the nucleoplasm; and under ATP-depleted conditions the TOP2 poison etoposide is LESS EFFECTIVE.

Human non-leukaemic cells, and ATP depletion is a non-physiological perturbation - although a hypoxic or metabolically stressed marrow blast is a plausible in vivo analogue that nobody has tested. The etoposide result is a cell-level cytotoxicity observation and this entry does not extend it to a clinical dose. Abstract only.
Evidence that argues against this (1)
- The etoposide-resistance-under-ATP-depletion result is about total cytotoxicity and does not establish which isozyme is protected; TOP2B is the isozyme that carries most etoposide-induced breaks at the KMT2A locus, so a TOP2A-only reading of the pharmacology would be wrong. Cowell IG et al. 2012

In murine NIH-3T3 cells the 170 kDa form of topoisomerase II (TOP2A) is undetectable until late S phase, peaks in G2-M and decreases through mitosis, and becomes undetectable once all cells reach G0 after serum starvation, while the 180 kDa form (TOP2B) remains present in G0; the 170 kDa enzyme is extracted from isolated nuclei at a lower salt concentration than the 180 kDa enzyme.

Abstract read in full; full text not read. Murine NIH-3T3 fibroblasts, not haematopoietic and not human. The paper names the two proteins by molecular weight only; assignment of 170 kDa to TOP2A is the standard identification, stated here as an identification and not as the authors' wording. Population-average immunoblot on synchronised cells. No DOI exists for this record and none is stored.
Evidence that argues against this (1)
- The paralogues also differ in chromatin association - the 180 kDa form needs more salt to extract - so a purely cell-cycle account of the TOP2A/TOP2B division of labour is incomplete, and the authors explicitly conclude only that the two 'perform different functions'. Using cell-cycle restriction as the whole explanation for isoform-specific drug biology goes beyond what this measurement supports. Yang X et al. 2000

Taspase1 interacts specifically with topoisomerase II alpha, enhancing formation of the DNA double-strand breaks that are a prerequisite for oestrogen-stimulated transcription, and functionally links Taspase1 to hormone-induced, topoisomerase II beta-mediated transient double-strand breaks; Taspase1 ALTERS THE H3K4 EPIGENETIC SIGNATURE upon oestrogen stimulation BY CLEAVING THE CHROMATIN-MODIFYING ENZYME MLL, and both oestrogen-driven transcription and MLL-derived epigenetic labelling are reduced on Taspase1 siRNA knockdown.

Abstract fetched and read in full 2026-09-21; full text not read. OESTROGEN-RESPONSIVE cells, not haematopoietic and not leukaemia (rule 73) - the relevance is that Taspase1 and MLL cleavage are this graph's own central biology (gene-tasp1), not that the system resembles infant ALL. The abstract is INTERNALLY MIXED ON ISOFORM: the title and the general claim are TOPOISOMERASE II BETA, while the specific co-immunoprecipitation sentence names TOPOISOMERASE II ALPHA - so both isoforms appear and this entry does not resolve which carries which role. Methods quoted are co-immunoprecipitation, LC-MS/MS and topoisomerase II DNA-cleavage assays. The H3K4 claim is a SIGNATURE-LEVEL statement ('alters the H3K4 epigenetic signature'); no methylation state and no locus is named in the abstract, so neither is claimed here.
Evidence that argues against this (1)
- The H3K4 change in this paper is attributed to Taspase1 CLEAVING MLL, not to topoisomerase activity, so it is not evidence that a topoisomerase writes, erases or reads a mark - which it does not. What the paper supports is that the TOP2-dependent break step and the MLL-dependent H3K4 step are coupled in one hormone-induced transcription programme. Reading it as 'TOP2A/TOP2B has a chromatin-mark readout' would be an attribution error of exactly the kind this graph's rule 89 is about: a link near the thing is not the thing. The honest statement is that the mark sits one protein away, and that protein is KMT2A. Cells 2023

paralogues 95 aa apart that share the nucleolar behaviour and do NOT share the drug biology - TOP2B carries the MLL-locus etoposide breaks
the two paralogues are separated by cell state, not only by sequence: TOP2A is confined to late S/G2-M and vanishes in G0 while TOP2B persists there (PMID 1651102). Distinct drug biology follows.
NEGATIVE relation: TOP2A protein is undetectable in G0, so a quiescent cell has no target for a TOP2A poison (murine NIH-3T3; PMID 1651102)
the absence of TOP2A from G0 is the mechanistic form of the persister problem for anthracycline and etoposide exposure in this disease
