Phase-2 dossier: the CXXC-DNA interface - two structures, a genetically validated requirement, and a selectivity problem that decides everything

also called KMT2A CXXC dossier · CpG-binding domain phase-2 plan

The fusion protein carries a small module that grips unmethylated DNA and tells the complex where to sit. Deleting it stops the cancer in mouse experiments, and there are good pictures of it. The problem is that about thirty other human proteins carry the same module, so a drug that blocks one would very likely block them all.

The same thing, for a scientist

The KMT2A CXXC domain binds non-methylated CpG DNA and is retained in every KMT2A fusion. Two structures were verified this cycle: 2J2S, the solution structure of the CXXC domain, and 4NW3, the domain in complex with CpG DNA at 2.82 A. The requirement is genetically validated in the classical mouse literature. The phase-2 problem is not potency and not structure - it is selectivity against the human CXXC-domain family (CXXC1/CFP1, MBD1, KDM2A, KDM2B, TET1, TET3, DNMT1, CXXC4/IDAX, CXXC5, KMT2B and FBXL19 — the family is CLOSED AT TWELVE including KMT2A itself, not open-ended; verified 2026-09-21 against three orthogonal UniProt queries, Pfam PF02008, InterPro IPR002857 and ft_zn_fing:CXXC-type, which return the identical twelve reviewed human proteins. Note TET2 is NOT among them and never was: it carries no zinc-finger feature at all, its ancestral CXXC having been split off by a chromosomal inversion into the separate gene CXXC4/IDAX, so a CXXC ligand cannot reach TET2 and TET2 does not belong on this selectivity gate), and the deeper problem that the interaction being blocked is protein-DNA rather than protein-protein. Nucleic-acid-binding surfaces are electropositive, shallow and solvent-exposed - the least tractable class in small-molecule discovery. ChEMBL contains 577 potent rows against KMT2A (CHEMBL1293299), essentially none of which are CXXC-directed; they belong to the WDR5-MLL and menin-MLL programmes. This dossier therefore sets a selectivity gate ahead of a potency gate, which is unusual and deliberate.

Go deeper
What structure exists

Verified against RCSB this cycle 1: 2J2S, solution structure of the non-methyl-CpG-binding CXXC domain of the leukaemia-associated MLL histone methyltransferase (NMR); 4NW3, crystal structure of the MLL CXXC domain in complex with a CpG DNA at 2.82 A. Both are isolated-domain structures with their ligand, which is exactly what a structure-based campaign needs, and a rare completeness for this beat.

What is missing is any structure of the CXXC domain in the context of the fusion protein or on a nucleosome, so the geometry of the real target is unknown.

What would be engaged, and why that is the hard part

A protein-DNA interface. The CXXC zinc-finger presents a positively charged, shallow surface that reads the unmethylated CpG dinucleotide. Three consequences follow directly:

  1. Small molecules are poorly suited. There is no buried hydrophobic pocket; affinity is distributed over electrostatics and a few base contacts. The field's successes against nucleic-acid-binding proteins are overwhelmingly allosteric or degradative, not orthosteric.
  2. Selectivity is a family problem, not a pair problem. The zinc-coordinating CXXC fold is conserved across roughly a dozen human proteins with essential and opposing functions - CXXC1/CFP1 in H3K4 methylation, TET1/TET3 in demethylation, DNMT1 in maintenance methylation. Hitting the family would scramble DNA methylation genome-wide.
  3. A DNA-competitive ligand is a DNA-mimetic. Molecules that mimic the phosphodiester backbone tend to be polyanionic, cell-impermeant, and promiscuous across nucleic-acid-binding proteins.
Chemical matter: the 577 rows that are not about this

ChEMBL holds 577 activity rows at pChEMBL 6 or above against KMT2A (CHEMBL1293299) and 1,014 against the menin/MLL protein-protein-interaction target entity (CHEMBL2093861) 2. Read carelessly, the first number says KMT2A is a well-drugged protein. It is not. Those rows are dominated by the WDR5-MLL and menin-MLL interaction programmes, which engage the partner proteins. To this review's knowledge no published series engages the CXXC domain itself. The number is a warning about how easily a druggability score computed from ChEMBL target-level counts can mislead - and that is a caution that applies to any ML model trained on such counts.

CORRECTED 2026-09-21. This section said 'To this review's knowledge no published series engages the CXXC domain itself.' No series is right; no molecule is not. Liang et al. report dimethyl fumarate, an approved oral therapy for multiple sclerosis and psoriasis, inhibiting the MLL1 CXXC-DNA complex with a relative IC50 of 0.91 +/- 0.09 uM in a single-molecule hairpin-unfolding assay, with the active metabolite monomethyl fumarate inactive at 10 uM 4. It strengthens the STOP verdict rather than weakening it. DMF covalently modifies more than 2,400 cysteines and the CXXC fold coordinates zinc through eight of them, so this is the family-wide reactive-cysteine liability, demonstrated. It was not counter-screened against CXXC1, KDM2B or TET1 and would not be expected to pass gate G1. See ana-the-cxxc-interface-has-exactly-one-published-inhibitor-and-it-is-an-approved-drug.

What would have to be true for a phase-2 programme

The selectivity gate must be passed before any potency work. Concretely: a candidate must show at least 30-fold discrimination between the KMT2A CXXC domain and CXXC1/CFP1, KDM2B and TET1 in a biophysical panel. If it cannot, the programme stops, because a pan-CXXC agent is a genome-wide DNA-methylation disruptor with no plausible paediatric safety case.

The realistic alternative modality, and the one this dossier recommends evaluating first because it is cheap: do not drug the domain, degrade the protein that carries it. A KMT2A-directed degrader engaging any accessible surface would remove the CXXC function as a by-product, and the selectivity problem becomes a ligandability problem on a different, possibly better, surface. That routes this candidate into the degrader dossier.

A potency gate cannot be set from the literature, and 2026-09-21 established why. The natural ligand's affinity is not one number: ten published measurements of the CXXC-DNA contact span 44 uM to 33 nM, driven by construct span (1147-1203 against the 1147-1244 MT domain), tag-borne valency, DNA valency (one CpG against six) and ionic strength (150 against 300 mM) 5. The nearest-to-physiological tagless value is 5.7 uM at 150 mM NaCl; the most multivalent geometry gives 33 nM. No measurement uses full-length KMT2A, a fusion protein or a nucleosome, so the species a competitive ligand must displace has never been characterised at all.

Two consequences for this dossier. First, any target-product profile stating a required potency would be guessing across three orders of magnitude, so the honest G0 is measure the affinity of the relevant species before setting a potency gate. Second, and more decisive: in the same MLL-AF9 swap series that supplies this dossier's functional-substitution warning, affinity rank is inverted against functional rank - the tighter-binding CGBP/CXXC1 domain fails to support leukaemogenesis and the 34-fold weaker DNMT1 domain succeeds - and every swapped domain including two that bind no DNA still targeted the fusion to Hoxa9 by ChIP 6. So a compound could achieve full biochemical engagement and move neither localisation nor function, which means gate G2 as written (chromatin displacement by CUT&RUN) may be unreachable in principle rather than merely demanding. G2 should be re-specified around CpG methylation at the target locus, which is the property that actually tracked function.

recommended alternativeRoute this candidate into the degrader dossier. Degrading the protein that carries the domain removes the CXXC function without solving the protein-DNA ligandability problem, and converts a hard selectivity question into a different and possibly easier ligandability question.
phase2 verdictDO NOT START as an orthosteric small-molecule programme. Carry forward only as a degrader substrate, and only after a one-day ChEMBL row audit confirms no CXXC-directed series already exists.
compute backendLaptop for the ChEMBL audit; Boltz co-folding of the CXXC-DNA complex is possible but its value is limited because the affinity module does not explicitly handle nucleic-acid partners well.
cxxc family verificationThree orthogonal UniProt queries - Pfam PF02008, InterPro IPR002857 and ft_zn_fing:CXXC-type - each return the identical twelve reviewed human proteins (verified independently at the coordinator, 2026-09-21). The selectivity gate is therefore enumerable and finite, which is better news for a ligand campaign than 'and others'.

How this could bite us back
  • Wild-type KMT2A haploinsufficiency causes a developmental syndrome and KMT2A is the most constrained gene in this beat (LOEUF 0.070). Any agent that cannot distinguish fusion from wild type is a developmental toxin in a child.
  • Pan-CXXC engagement would perturb TET-mediated demethylation and DNMT1-mediated maintenance simultaneously, with genome-wide and unpredictable consequences.
  • A ChEMBL-derived druggability score would rate KMT2A as well-drugged on 577 potent rows, when none of them engage this domain. Any ML model built on target-level chemical-matter counts will make this error systematically.
  • A compound could engage the CXXC domain completely and change nothing measurable: in the MLL-AF9 swap series, fusions carrying CpG-binding-dead domains still reached Hoxa9 by ChIP. The go/no-go gate as originally written would read that as a failed hypothesis when it is a mis-specified endpoint.
Still open
  1. Do any of the 577 ChEMBL rows against KMT2A actually assay the CXXC domain, and has anyone ever run a CXXC-directed screen?
  2. Is there an allosteric surface adjacent to the CXXC zinc site that is not conserved across the family, which would convert an intractable orthosteric problem into a tractable allosteric one?
  3. Would selective degradation of the fusion protein over wild-type KMT2A be possible at all, given that the two share the entire retained N-terminus including this domain?
  4. What affinity would a competitive ligand have to beat? Published values for the natural ligand span 44 uM to 33 nM and none of them is of full-length KMT2A, a fusion protein or a nucleosome, so the potency requirement is unknown across three orders of magnitude.

Two structures of the KMT2A/MLL CXXC domain were verified by direct query of the RCSB data API this cycle: 2J2S (solution structure of the non-methyl-CpG-binding CXXC domain, NMR) and 4NW3 (MLL CXXC domain in complex with CpG DNA, X-ray, 2.82 A).

    What this evidence cannot show

Isolated-domain structures. Neither shows the CXXC domain in the context of a fusion protein or a nucleosome, so the accessible surface in the real target is unverified.

Evidence that argues against this (1)
    • Both structures are of an isolated CXXC domain on designed oligonucleotides. A DNA-binding interface is among the least tractable target classes for a small molecule because the natural ligand is a polyanion with a large contact surface, and structure availability does not change that. RCSB PDB 2026

ChEMBL holds 577 activity rows at pChEMBL 6 or above against KMT2A (CHEMBL1293299) and 1,014 against the menin/MLL interaction target (CHEMBL2093861), but the KMT2A rows reflect partner-directed programmes (WDR5-MLL, menin-MLL) rather than any CXXC-directed series.

    What this evidence cannot show

The attribution of those rows to WDR5- and menin-directed programmes is an inference from the ChEMBL target naming and the known literature, not a row-by-row audit; a full audit is a G1 task.

Evidence that argues against this (1)
    • If a row-by-row audit found a genuine CXXC-directed series hidden in those 577 rows, the feasibility verdict here would improve materially. The audit costs a day and has not been done. source 2026

KMT2A is the most loss-of-function-constrained gene in this beat (pLI 1.0, LOEUF 0.070), consistent with its role as a developmental regulator whose haploinsufficiency causes Wiedemann-Steiner syndrome - which is the organismal warning against any whole-protein KMT2A-directed strategy.

    What this evidence cannot show

Constraint applies to the wild-type gene. A degrader or ligand selective for the fusion protein over wild-type KMT2A would not be subject to this argument, but no such selectivity has been demonstrated.

Evidence that argues against this (1)
    • A gnomAD constraint metric is not a safety signal for a drug, and this beat uses it as one across every dossier. pLI and LOEUF measure intolerance to HETEROZYGOUS loss-of-function across a whole lifetime from conception, in a population sample. A drug gives partial, reversible occupancy in an adult for weeks. High constraint says the gene matters in development; it does not predict on-target toxicity of transient pharmacological inhibition, and this project has already had to make the same correction for germline MEN1 penetrance in the endocrine beat. KMT2A at pLI 1.0 and LOEUF 0.070 is the most constrained gene in the beat, and haploinsufficiency causes Wiedemann-Steiner syndrome — a developmental phenotype, not a prediction about inhibiting one domain of the protein in an adult. gnomAD, Broad Institute 2026

Dimethyl fumarate, an approved oral therapy for relapsing multiple sclerosis and psoriasis, inhibits the MLL1 CXXC-DNA complex with a relative IC50 of 0.91 +/- 0.09 uM in a single-molecule assay, with the metabolite monomethyl fumarate inactive at 10 uM; DMF covalently modifies more than 2,400 cysteine residues and CXXC-family proteins have not been identified as DMF targets by chemical proteomics.

    What this evidence cannot show

A relative IC50 from a four-parameter logistic fit to single-molecule unfolding times, isolated synthesised domain on naked DNA, no cell, no fusion protein, no counter-screen against any other CXXC domain, and the covalent mechanism inferred rather than demonstrated on this domain. In the MLL-AF9 CXXC swap series, DNA-binding affinity rank is inverted against functional rank - the tighter-binding CGBP/CXXC1 domain does not support leukaemogenesis while the roughly 34-fold weaker DNMT1 domain does - and every swapped domain including two with no detectable DNA binding still targeted the fusion to the Hoxa9 locus by ChIP; the discriminating property was protection of specific Hoxa9 CpGs from methylation.

    • Cierpicki's C1188D point mutant, which abolishes DNA binding within the native MLL fold rather than swapping the whole domain, does abolish immortalisation and leukaemia - so DNA binding by this domain is required even though affinity does not rank the paralogues. A gate built on 'affinity does not matter' would be as wrong as one built on 'affinity is everything'. Cierpicki T et al. 2010
