JUND liberation — the drug's mechanism of action and one of its mechanisms of resistance are the same event

also called menin-JUND displacement resistance · AP-1/JUND escape from menin inhibition · JNK-JUND bypass

The leukaemia drug works by prising a protein called menin off the fusion protein, but menin holds a second protein, JunD, in exactly the same groove. Freeing JunD lets it switch on its own set of genes, and those genes help the leukaemia survive the drug.

The same thing, for a scientist

Menin binds the KMT2A MBM1 motif and a JUND motif in the same deep peptide pocket [E4][E5], so every clinical menin inhibitor must release both. Quantitative mass spectrometry in MLL-rearranged leukaemic cells shows that menin-MLL inhibitors do disrupt the menin-JUND interaction; menin-inhibitor treatment then increases JUND chromatin occupancy, upregulates JUND target genes, and this contributes to resistance to the inhibitor. CRISPR inactivation of JUND or pharmacological JNK inhibition synergistically enhanced the anti-leukaemic effect of menin-MLL inhibition, producing reduced proliferation, cell-cycle arrest and apoptosis, and mice engrafted with JUND-deficient leukaemia cells carried a lower tumour burden than mice engrafted with wild-type cells [E1]. The older biology makes the direction unsurprising: menin represses JUND-activated transcription by blocking JNK-mediated JUND phosphorylation [E4][E5], and JunD deprived of menin does not merely lose repression — in fibroblasts it inverts from growth suppressor to growth promoter [E3]. This entry exists because the same observation is currently filed in this graph only as an endocrine toxicity concern. The evidence says it is also a resistance mechanism, and it is the one resistance mechanism that is structurally guaranteed by the drug's own binding mode rather than acquired under selection.

What was actually measured

Ozyerli-Goknar and colleagues asked what menin inhibitors do to JUND, a question the field had left open despite the shared pocket being known since 2012 4. In MLL-rearranged leukaemic cells they showed by quantitative mass spectrometry that menin-MLL inhibitors disrupt menin-JUND interactions as well as menin-MLL; RNA sequencing and chromatin binding assays then showed that inhibitor treatment increased JUND chromatin occupancy and upregulated JUND target genes, and that this contributes to resistance against menin-MLL inhibitors. CRISPR-mediated JUND inactivation or JNK inhibition synergistically enhanced the anti-leukaemic effect of the inhibitors — reduced proliferation, cell-cycle arrest, apoptosis — and JUND-deficient leukaemia cells produced a lower tumour burden in immunocompromised mice 1.

The logical shape of this result is unusual and worth stating plainly: the drug's therapeutic act (vacating the menin pocket) and this escape route (vacating the menin pocket) are the same physical event. There is no selection step, no mutation, no clonal outgrowth. Every treated cell receives the resistance mechanism at the same instant it receives the therapy, in proportion to how well the drug works.

Why the direction of the effect was predictable but not proven

Menin binds JunD directly and represses JunD-activated transcription, and MEN1 missense mutations found in patients disrupt that interaction 5. The structural work then showed one pocket, two peptides, opposite transcriptional consequences: menin-MLL1 promotes transcription, menin-JUND blocks JNK-mediated JUND phosphorylation and suppresses JUND-induced transcription 4. And in fibroblasts, JunD stripped of menin does not simply stop suppressing growth — it promotes it, whether the stripping is done by a JunD mutation that cannot bind menin or by a Men1-null background 3.

So the prediction was available for over a decade. What was missing, and what 1 supplies, is the demonstration in leukaemic cells under a menin-MLL inhibitor, with a chromatin and transcriptional readout and a genetic epistasis test.

This is filed in the wrong place in this graph

PMID 42371796 currently appears in this graph inside the endocrine/ folder, where the framing is menin-inhibitor toxicity in hormone-producing tissue, and inside two analogy/ nodes as an illustration that the drug redistributes chromatin binding rather than only subtracting it. Both uses are correct. Neither records the paper's own headline finding, which is a therapy-resistance finding in leukaemia, in the disease this project is about, with an in vivo arm.

The consequence of the misfiling is practical. A reader of the four pre-existing resistance entries would conclude that escape from menin inhibition requires either a MEN1 mutation, a co-mutation, loss of PRC1.1, or a slow adaptive rewiring. JUND liberation is none of those: it is immediate, universal and dose-proportional.

What follows for combination design

The countermeasure is already named and already exists as a drug class. JNK inhibition phenocopied JUND knockout in synergising with menin-MLL inhibition 1, and the mechanism is specific — menin normally represses JUND by blocking JNK-mediated phosphorylation 4, so removing menin without removing JNK activity is exactly the condition that activates JUND.

This also reframes a combination question the field is currently arguing about. Most proposed menin-inhibitor partners (venetoclax, FLT3 inhibitors, hypomethylating agents) are orthogonal agents chosen to cover routes the menin inhibitor does not reach. A JNK inhibitor is different in kind: it is a partner chosen to neutralise a liability the menin inhibitor creates. If the effect size in patients resembles the preclinical effect size, a menin inhibitor given without such a partner is giving away part of its own efficacy.

Honest limits

One primary paper, published 2026. No patient data: the resistance contribution is established in cell lines and in a xenograft, not in a treated child or adult. The compounds are described as menin-MLL inhibitors rather than specifically revumenib, ziftomenib or bleximenib at clinical exposure, so the dose-response relationship between KMT2A displacement and JUND displacement — the number that would say how much of the drug's window is spent liberating JUND — remains unmeasured. That measurement is already specified as an experiment in this graph (exp-jund-displacement-coip-under-clinical-menin-inhibitor), which was written for endocrine safety reasons and would answer the efficacy question at the same time.

How this could bite us back
  • The mechanism of action IS the mechanism of resistance. Every increment of target engagement delivers an increment of escape, so this liability cannot be dosed away, scheduled away, or avoided by a better pocket binder of the same class.
  • It arrives without selection. There is no resistant clone to detect, no variant allele frequency to track, and no DNA assay that will ever show it — so a relapsed child will be sequenced, told nothing was found, and the mechanism will go unrecorded.
  • It generalises to the whole strategy. This project's thesis is that obligate protein-protein interfaces are the druggable handle; JUND shows that an interface pocket with more than one physiological client cannot be blocked selectively, and pockets with exactly one client are rare.
  • The countermeasure has its own essential-function problem. JNK signalling is required well beyond leukaemia, and adding a JNK inhibitor to a menin inhibitor in an infant stacks two developmental-signalling insults with no paediatric dose-finding for either combination.
  • The endocrine and leukaemia consequences of the same event may need opposite handling. JNK inhibition would suppress liberated JUND in the marrow and in the parathyroid alike; that is plausibly protective in both, but no study has shown one dose does both.
Still open
  1. What fraction of a menin inhibitor's achievable anti-leukaemic effect is cancelled by JUND liberation? Nobody has run the arithmetic, and the experiment is a JUND-knockout versus wild-type dose-response curve in the same line.
  2. Does the JUND displacement IC50 sit above, below or on top of the KMT2A displacement IC50 for revumenib, ziftomenib and bleximenib? If JUND is displaced at lower concentration than KMT2A, there is a dose range in which the drug is purely harmful.
  3. Is a JNK inhibitor tolerable in an infant alongside a menin inhibitor? The combination is mechanistically compelling and pharmacologically untested in children, and JNK is not a leukaemia-restricted kinase.
  4. Does JUND liberation occur in patients on revumenib? A JUND target signature scored on banked on-treatment marrows from AUGMENT-101 would settle it retrospectively without a new trial.
  5. Do other physiological occupants of the same menin pocket exist in haematopoietic cells? If so, they are also being liberated at every dose and nobody has looked.

Quantitative mass spectrometry of MLL-rearranged leukaemic cells demonstrated that menin-MLL inhibitors disrupt menin-JUND interactions; RNA sequencing and chromatin binding assays showed that menin-MLL inhibitor treatment increased JUND chromatin occupancy leading to upregulation of target genes and contributing to resistance against menin-MLL inhibitors; CRISPR-mediated inactivation of JUND or pharmacological JNK inhibition synergistically enhanced the anti-leukaemic effects of menin-MLL inhibitors with reduced proliferation, cell-cycle arrest and apoptosis; and immunocompromised mice engrafted with JUND-deficient leukaemia cells exhibited reduced tumour burden compared with mice engrafted with wild-type leukaemic cells.

    What this evidence cannot show

Abstract read in full via NCBI efetch; full text not obtained. Cell lines plus a xenograft; no patient samples and no on-treatment clinical material. The abstract names menin-MLL inhibitors generically and does not state which compound at which concentration produced the mass-spectrometry result, so the potency of JUND displacement relative to MLL displacement is not established here.

Evidence that argues against this (1)
    • The same paper is read elsewhere in this graph primarily as evidence of chromatin redistribution and as an endocrine liability rather than as a leukaemia resistance route. If the JUND target programme is tissue-specific, the leukaemia resistance finding and the endocrine toxicity concern do not transfer to one another in either direction; the structural basis for the shared pocket says nothing about what the liberated factor does in a given lineage. Somatic MEN1 mutations at the revumenib-menin interface cause clinical resistance by attenuating small-molecule binding while preserving the menin-MLL1 interaction and preventing inhibitor-induced eviction of menin and MLL1 from chromatin — establishing that eviction from the shared pocket is the drug's mechanism of action, and therefore that anything else evicted from that pocket is evicted as an obligatory consequence of the drug working.

MEN1 mutations mediate clinical resistance to menin inhibition. · Perner F, Stein EM, Wenge DV, et al. · Nature 2023
    What this evidence cannot show

Cited here for mechanism of action, not for JUND. This paper does not address JUND. The inference that pocket vacation releases JUND is structural [E4] and was tested directly only in [E1].

Evidence that argues against this (1)
    • This establishes that inhibitor-induced eviction is the mechanism of action, which is what the JUND argument needs - but it also bounds it. If interface mutations preserve menin-MLL1 binding while blocking the drug, then in a resistant patient the inhibitor is NOT evicting menin, so it is not liberating JUND either. JUND liberation can therefore only explain resistance that arises while the drug still works, not the MEN1-mutant resistance that accounts for 39% of post-revumenib relapses. The two mechanisms are alternatives, not additive. Blood 2026

Stable overexpression of wild-type JunD in JunD-null immortalised fibroblasts inhibited proliferation and reverted the transformed-like phenotype, whereas a JunD missense mutant with disabled menin binding had opposite, growth-promoting effects, and wild-type JunD enhanced transformed-like characteristics when overexpressed in Men1-null fibroblasts — so JunD changes from growth suppressor to growth promoter when its binding to menin is prevented.

Transcription factor JunD, deprived of menin, switches from growth suppressor to growth promoter. · Agarwal SK, Novotny EA, Crabtree JS, et al. · Proc Natl Acad Sci U S A 2003
    What this evidence cannot show

Immortalised mouse fibroblasts with overexpressed constructs, not leukaemia cells, and a genetic Men1-null background rather than pharmacological pocket occupancy. No independent replication in a haematopoietic cell was located. Used here only for the direction of the effect, not its magnitude.

Evidence that argues against this (1)
    • This is a fibroblast experiment in a MEN1 tumour-suppressor framework, not a leukaemia one. JunD's growth-suppressive behaviour in JunD-null immortalised fibroblasts says little about what JUND liberation does in a KMT2A-rearranged blast, where menin is acting as an oncogenic cofactor rather than a tumour suppressor - the sign of menin's effect differs between the two settings, so the sign of JUND's may too. Blood 2026
  4. E4established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.paper

Human menin contains a single deep peptide-binding pocket that binds short MLL1 and JUND peptides in the same manner but with opposite effects on transcription: menin-JUND blocks JNK-mediated JUND phosphorylation and suppresses JUND-induced transcription, while menin-MLL1 promotes transcription.

    What this evidence cannot show

Structural and biochemical; predicts but does not demonstrate pharmacological displacement of JUND by a clinical compound. That step is supplied by [E1].

Evidence that argues against this (1)
    • The shared-pocket structure establishes that MLL1 and JUND compete for one site; it does not establish that clinical menin inhibitors liberate enough JUND to matter, or at what exposure. The leukaemia-relevant demonstration is separate and recent, and it is the evidence this entry should lean on rather than the structural inference. Blood 2026
  5. E5established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.paper

A yeast two-hybrid screen identified JunD as a direct menin partner, menin repressed JunD-activated transcription, and several naturally occurring clustered MEN1 missense mutations disrupted the menin-JunD interaction.

    What this evidence cannot show

Founding interaction paper; no leukaemia and no drug.

Evidence that argues against this (1)
    • A yeast two-hybrid screen with transcriptional-repression assays is a discovery-era result about menin's tumour-suppressor role in endocrine tissue. Naturally occurring MEN1 missense mutations disrupting menin-JunD binding explain MEN1 SYNDROME biology; they are not evidence about resistance to a menin inhibitor in leukaemia, and this entry should not treat the two contexts as continuous.
