Systematic enumeration of menin-inhibitor resistance mechanisms: seven entries written, thirteen candidates rejected with reasons

also called resistance beat enumeration ledger · menin resistance saturation audit

This is the working record of a systematic sweep of the published literature for every way a leukaemia can escape a menin inhibitor. It lists the mechanisms that turned out to be real and got their own entry, and — just as importantly — the ones that turned out not to be documented, so nobody has to search for them again.

The same thing, for a scientist

A systematic enumeration performed against primary literature via raw NCBI eutils (the PubMed MCP tool ANDs every keyword and returns false zeros on this topic, so it was not used for negative conclusions) and against the raw ClinicalTrials.gov v2 API. The graph held four `resistance` entries at the start of this cycle against 171 other entries that discuss menin-inhibitor resistance. Seven new entries were written, one hypothesis, and thirteen candidate mechanisms were considered and rejected with a stated reason each. Three findings bear on entries this beat does not own and are recorded here for the coordinator rather than edited in: a paper filed only as an endocrine toxicity concern is a leukaemia resistance paper; a hypothesis in `analogy/` predicts retained fusion occupancy at saturating drug and the only human sample profiled under revumenib shows the opposite; and a genetic, panel-detectable lesion (KMT2C/KMT2D loss) is currently listed among routes described as invisible to mutation panels.

Method, and why the search tooling is recorded

All searches were run with raw NCBI eutils esearch using explicit [Title/Abstract] field tags and boolean OR groups, and every citation was resolved with efetch before use — title, authors, journal, year and DOI read back from the record. Five citation errors were caught this way inside this beat's own drafts — four DOIs and one author list, all filled in from memory and all wrong (see data.citation_errors_caught_inside_this_beat for the exact values). Every one was corrected against the live record before the file was finalised. That is precisely the failure mode the standing brief warns about, and it occurred five times in one session, which is direct evidence that the fetch-everything rule earns its cost rather than being paranoia.

The PubMed MCP search_articles tool was deliberately not used for any negative conclusion, because it ANDs every keyword and therefore returns zero on topics with substantial literature.

Registry facts were taken from the raw ClinicalTrials.gov v2 API, not the MCP tool, so that whyStopped was visible.

python scripts/check-absence-vs-library.py --min-overlap 2 was run before writing any absence claim. None of this beat's absence topics (efflux/ABCB1, microenvironment, CYP3A4-mediated failure, hypoxia, MEN1 copy number) appeared anywhere in the report, so the graph does not already hold a paper contradicting them.

Registry findings worth keeping

Queried the raw v2 API for all studies matching the menin-inhibitor compounds and read whyStopped on every non-active record. AUGMENT-101 (NCT04065399) is RECRUITING as of the 2026-09-16 registry update, with estimated enrolment now 447 and estimated primary completion 2027-12-15; there is no whyStopped. Four terminated or withdrawn records carry reason strings that appear in no paper:

NCTStatuswhyStopped (verbatim)
NCT05153330 (BMF-219 / icovamenib, AML+ALL with KMT2A/NPM1)TERMINATED"Biomea Fusion, Inc., is no longer pursuing oncology indications for BMF-219. No safety concerns or efficacy observations led to this study closure."
NCT04752163 (DS-1594b ± aza/ven/mini-HCVD)TERMINATED"supporting/funding source decided to de-prioritize and stop development of DS-1594"
NCT05731947 (revumenib in colorectal and other solid tumours)TERMINATED"Study was terminated by sponsor after Phase 1b as pre-defined signals of efficacy were not met in the Phase 1b dose expansion cohort."
NCT07052994 (revumenib + cytarabine/daunorubicin/gemtuzumab)WITHDRAWN"PI requested"

The first two matter for this beat specifically: two menin inhibitors left oncology development for commercial reasons with the sponsor explicitly disclaiming safety or efficacy as the cause. Any future analysis of why the class narrowed to revumenib and ziftomenib has to account for that, and the information exists nowhere else.

Entries written

Seven resistance entries and one hypothesis, one per distinct mechanism:

  1. resistance-jund-liberation-menin — JUND released from the shared menin pocket gains chromatin occupancy and drives its own programme; the mechanism of action and this mechanism of resistance are the same physical event.
  2. resistance-tp53-apoptotic-priming-menin — TP53 inactivation permits survival despite a full on-target transcriptional response; MCL-1 inhibition but not venetoclax resensitises. Contested: a second primary study found TP53 insufficient to confer resistance.
  3. resistance-mll34-utx-tumour-suppressor-failure — part of how the drug kills is UTX-dependent activation of a tumour-suppressive programme; KMT2C/KMT2D loss removes that arm, and CDK4/6 inhibitors restore it.
  4. resistance-lineage-switch-under-menin-inhibition — a KMT2A-rearranged leukaemia lineage-switched during revumenib treatment; a mutation screen would have reported nothing, and one documented switch in this graph reversed in nine days with no intervening therapy.
  5. resistance-enl-marked-epigenetic-memory — in that same sample the oncoprotein and menin became undetectable while ENL persisted on target loci; resistance surviving complete target engagement.
  6. resistance-swi-snf-bet-dependent-state — laboratory-derived menin-inhibitor-resistant cells with no MEN1 mutation depend on SMARCA4, BRD4 and CREBBP, and the same combinations also kill M327I-mutant cells.
  7. resistance-cross-resistance-between-menin-inhibitors — base-editor screens across five inhibitors separate shared (M327I/V/T, G331D) from inhibitor-specific (C334R, E368K/V, V372A) substitutions; the substitutions patients actually acquire fall in the shared class.
  8. hyp-gmp-destination-paradox-menin (hypothesis) — lineage switch delivers cells into the GMP-like state, and GMP-like AML is the state most sensitive to menin inhibition, so a switch on a menin inhibitor may predict response rather than failure.
Candidates rejected, and why

Each of these was searched deliberately and is recorded so the next agent does not re-tread it. The reasons are in data.rejected_candidates. In summary: five were rejected because the literature does not exist (drug efflux, microenvironment-mediated resistance, MEN1 copy-number change or target re-expression, HOXA9/MEIS1 target rebound, partner-identity-determined resistance); three because the evidence points the opposite way (hypoxia sensitises rather than protects; GMP-like differentiation increases rather than decreases sensitivity; the fusion is truncal and stable so target loss does not occur); three because they are combination-sensitisation studies rather than demonstrations of resistance (SETD2/H3K36me3, guanine nucleotide biosynthesis, immunoproteasome); and two because they are already covered in this graph (residual fusion occupancy, and pharmacokinetic/CYP3A4 exposure, which is fully documented inside drug-revumenib and has no reported clinical failure attributed to it).

    "reason": "No literature exists. `(menin OR revumenib OR ziftomenib)[tiab] AND (efflux OR ABCB1 OR P-glycoprotein OR ABCG2)[tiab]` returns exactly 2 records, both unrelated: menin and cholesterol in colorectal cancer (PMID 37627154) and epigenetic targets in taxane-resistant prostate cancer (PMID 38346967). The named BET-inhibitor resistance precedent in MLL-AF9 that this graph already cites was explicitly shown NOT to be efflux-mediated, which is the closest the field comes to addressing the question and points away from it."

    "candidate": "Microenvironment- or niche-mediated protection from menin inhibition",
    "reason": "Named as a resistance category in a 2026 review (PMID 42733002) but no primary study supports it in this disease. `(menin inhibitor OR menin inhibition)[tiab] AND (bone marrow stroma OR mesenchymal OR cytokine OR co-culture)[tiab]` returns 3 records, none of which tests stromal protection against a menin inhibitor. Writing an entry from a review's category heading would be exactly the error this project has been burned by."

  {
    "candidate": "Hypoxic niche protecting KMT2A-rearranged blasts from menin inhibition",
    "reason": "The evidence points the OPPOSITE way. The only study testing hypoxia in KMT2A-rearranged AML under a menin inhibitor reports that hypoxia induction SENSITISED cells to venetoclax plus MI-503, and that the combination works partly by repressing HIF-1A and its target HDAC9 (PMID 38053201, [E4]). A hypoxia-protection entry would have been written against the data."

    "candidate": "MEN1 copy-number change, MEN1 amplification or target re-expression as resistance",
    "reason": "No literature. `MEN1[tiab] AND (amplification OR copy number)[tiab] AND leukemia[tiab]` returns 2 records: a 2004 review of MLL transformation (PMID 15663355) and a 2002 FISH study of MEN1 allelic deletion in AML (PMID 12210500), neither involving a drug. There is also a mechanistic argument against it: menin is REQUIRED for the leukaemogenic activity of KMT2A fusions, and the clinical resistance mutations are missense changes that specifically preserve the menin-MLL1 interaction (PMID 36922589) — the tumour cannot afford to lose menin, which is why every observed escape keeps it."
  },
  {
    "candidate": "Rebound or re-expression of HOXA9/MEIS1 on continued therapy",
    "reason": "No literature at all: `(menin inhibitor OR menin inhibition)[tiab] AND (HOXA9 OR MEIS1)[tiab] AND (rebound OR re-expression OR reexpression OR restoration)[tiab]` returns zero, and the field's actual finding is the reverse — resistant cells show SUSTAINED on-target suppression of the canonical targets (PMID 41027490). That sustained-suppression observation is recorded inside resistance-mll34-utx-tumour-suppressor-failure, where it belongs."

  {
    "candidate": "KMT2A partner identity determining menin-inhibitor response",
    "reason": "Tested once and came out negative: three AMLs with the high-risk partners MLLT10 and AFDN, plus two infant ALL samples, were all sensitive to menin inhibition in patient-derived xenografts ([E5], PMID 41027490). No trial has reported response stratified by partner or by breakpoint, and this graph already records that absence in mechanism-different-breaks-different-biology. Partner-specific chromatin binding patterns do exist (PMID 39472576) and correlate with ALL-versus-AML prevalence, but nobody has connected them to drug response."
  },
  {
    "candidate": "Differentiation escape / re-acquisition of a stem programme as menin-inhibitor resistance",
    "reason": "Not documented, and one direct result points the other way: GMP-like primary AML shows the GREATEST ex vivo sensitivity to a menin-KMT2A inhibitor ([E6], PMID 39704147), so a more differentiated myeloid state is associated with more drug effect, not less. The stem-compartment literature that does exist is about SENSITISATION — leukaemia stem cells of MLL-rearranged AML have enhanced guanine nucleotide biosynthesis whose blockade drives differentiation and sensitises to menin inhibition (PMID 40102405) — which is the inverse framing. The tension between the two directions is real enough to be worth a hypothesis, and is written as hyp-gmp-destination-paradox-menin rather than as a speculative resistance entry."

  {
    "candidate": "SETD2 / H3K36me3-supported transcription elongation as a menin-independent buffer",
    "reason": "The single relevant study (PMID 42170936) shows that a SETD2 inhibitor SYNERGISES with revumenib and that SETD2 inhibition sensitises leukaemia cells by decreasing H3K36me3. That is a combination-rationale paper, not a demonstration that H3K36me3 causes resistance. Recorded here because the mechanistic link to LEDGF/PSIP1 PWWP reading of H3K36me is attractive and someone will re-derive it; the honest status is untested."
  },
  {
    "candidate": "Guanine nucleotide biosynthesis / IMPDH2 as a metabolic support of resistance",
    "reason": "Same category as SETD2. PMID 40102405 shows IMPDH2 inhibition reduces LEDGF and menin protein, impairs MLL-fusion complex formation and chromatin binding, and sensitises to menin inhibitors. Sensitisation by blockade is not evidence that the intact pathway causes resistance; no study reports high guanine nucleotide flux in resistant cells."

    "candidate": "Immunoproteasome function as a resistance mechanism",
    "reason": "PMID 38049829 shows immunoproteasome function maintains oncogenic gene expression in KMT2A-complex-driven leukaemia. That is a vulnerability, not an escape route; nothing links it to menin-inhibitor failure."
  },
  {
    "candidate": "Loss of the fusion / loss of the drug target at relapse",
    "reason": "Contradicted by this graph's own sourced entry. The KMT2A fusion is truncal and stable, retained through relapse, through lineage switch and through immunophenotypic escape from CD19-directed therapy (mechanism-fusion-clonal-stability-diagnosis-to-relapse). Target loss therefore is not available as a route in this disease, which is one of the few structural advantages the menin strategy has."

    "candidate": "Pharmacokinetic under-exposure / CYP3A4 interaction as a resistance mechanism",
    "reason": "The pharmacology is real, fully documented in this graph's drug-revumenib entry (CYP3A4 substrate; label halves the dose with a strong CYP3A4 inhibitor; 270 mg BID versus 160 mg BID at 40 kg or more), and the pivotal AUGMENT-101 phase 2 dosed 163 mg q12h WITH a strong CYP3A4 inhibitor ([E7], PMID 39121437) — that is, the registration efficacy data were generated under mandatory enzyme inhibition. Ziftomenib carries its own labelled CYP3A4 fraction-metabolised question (PMID 42682186, PBPK modelling to inform labelling). Despite all that, NO case of menin-inhibitor failure attributed to under-exposure has been reported, and `(menin inhibitor OR revumenib)[tiab] AND (pharmacokinetic OR exposure OR CYP3A4)[tiab]` returns 10 records none of which describes one. Rejected as an entry and recorded as an open question: a child whose azole is stopped, or who is started on a CYP3A4 inducer, moves to a different exposure with no published guidance on what that does to response."
  },
  {
    "candidate": "UBTF tandem duplication AML response-and-resistance",
    "reason": "Real and interesting — UBTF-TD AML is menin-inhibitor-sensitive (PMID 37890156) and an NEJM correspondence reports response AND resistance in that genotype (PMID 38924737). Rejected for this beat because UBTF-TD is a distinct genotype outside the KMT2A-rearranged scope of this graph, and because the NEJM item is correspondence with no abstract in PubMed, so its mechanism could not be read without the full text. Recorded so the next agent knows it was seen and deliberately left out rather than missed."

How this could bite us back
  • The rejection list is the more fragile half of this analysis. 'No literature exists' is a statement about what a set of PubMed queries returned on one day, and a query family that misses a synonym produces a false absence that then gets cited as a finding.
  • Almost every mechanism here was read at abstract level. Full texts were not obtained for any of the primary papers, so effect sizes, sample numbers and controls are not in the graph and the entries should not be used as though they were.
  • Enumerating mechanisms may itself be the wrong frame. One of the entries written this cycle reports a path-independent dependency profile of the resistant state, which if true means the catalogue matters less than the destination — and this analysis is a catalogue.
  • The beat is still adult-dominated after this cycle. Seven new entries do not fix the fact that the evidence base for menin-inhibitor resistance in infant KMT2A-rearranged ALL is two patient-derived xenograft patients.
Still open
  1. What is the resistance-mechanism share in children? Every mechanism in this beat was characterised in adult AML or in adult-derived cell lines, with the single exception of the two infant ALL patient-derived xenografts. The population this project exists for is the one with no data.
  2. Why did two menin inhibitors leave oncology development for commercial reasons? Both registry strings explicitly disclaim safety and efficacy, which means the class narrowed for reasons that have nothing to do with the biology this graph is built on.
  3. Can the countermeasures be ranked? This beat now names venetoclax, MCL-1 inhibition, CDK4/6 inhibition, KAT6/KAT7 inhibition, JNK inhibition and BET/SWI-SNF inhibition, each proposed by a different group for menin-inhibitor resistance, with no head-to-head comparison anywhere.
  4. Is the rejected list stable? Five of the thirteen rejections are 'no literature exists' and will expire as the field moves; a re-run of the same query families in twelve months is the cheapest way to keep this beat current.

  1. E1established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.trial_registry

    AUGMENT-101 (NCT04065399) is registered as RECRUITING with an estimated enrolment of 447 and an estimated primary completion date of 2027-12-15, with no whyStopped field; the record was last updated 2026-09-16.

    What this evidence cannot show

    Registry snapshot taken 2026-09-19 from the raw v2 API. Registry fields are sponsor-maintained and estimated dates move.

    Evidence that argues against this (1)
    • A RECRUITING status with an estimated completion in 2027 is a statement about the registry entry, not about resistance. It is included here to show the enumeration is current, but it supplies no mechanism and the estimated enrolment of 447 is a target rather than an achieved number - AUGMENT-101's reported efficacy analyses rest on 57 and 84 patients. An enumeration should not let a registry target stand in for evidence. ClinicalTrials.gov 2026
  2. E2established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.trial_registry

    NCT05153330, the phase 1 study of the covalent menin inhibitor BMF-219 (icovamenib) in adults with AML and ALL including KMT2A-rearranged and NPM1-mutant disease, is TERMINATED with the registry reason: 'Biomea Fusion, Inc., is no longer pursuing oncology indications for BMF-219. No safety concerns or efficacy observations led to this study closure.'

    What this evidence cannot show

    A sponsor's own statement about its reasons, not an independent finding. It is nonetheless the only public account of why a covalent menin inhibitor left oncology development, and it explicitly excludes both safety and efficacy.

    Evidence that argues against this (1)
    • A TERMINATED registry status with a sponsor-prioritisation reason carries no information about the drug, the genotype or the resistance question. The reason field quoted here is explicitly commercial. Reading programme terminations as evidence about menin biology confuses a capital-allocation decision with a scientific result, and this enumeration should not count it as a data point either way. NCT04752163, the study of DS-1594b with or without azacitidine, venetoclax or mini-HCVD in relapsed/refractory leukaemia, is TERMINATED with the registry reason: 'supporting/funding source decided to de-prioritize and stop development of DS-1594'.

    What this evidence cannot show

    Sponsor statement. A phase 1/2 report of DS-1594 was published in 2025 (PMID 41310838, J Hematol Oncol), which this beat did not read; anyone using this registry string should read that paper alongside it.

    Evidence that argues against this (1)
    • Same objection, and the phrasing is even plainer: the sponsor 'decided to de-prioritize and stop development'. What the field lost is the comparison, not a negative result. An enumeration of resistance mechanisms cannot include mechanisms that were never measured because the trial stopped for funding. ClinicalTrials.gov 2026
  4. E4moderate confidence 3 of 5: One strong study, or several smaller consistent ones.paper

    Hypoxia induces HDAC9 in KMT2A-rearranged AML, venetoclax plus a menin inhibitor represses HIF-1A and HDAC9, and hypoxia induction sensitised KMT2A-rearranged AML to venetoclax plus MI-503-mediated proliferation inhibition and apoptosis — that is, the hypoxic condition increased rather than decreased sensitivity to the combination.

    What this evidence cannot show

    A letter; abstract read in full via NCBI efetch, full text not obtained. Cell lines, primary cells ex vivo and one xenograft. Used here only to reject the hypothesis that a hypoxic niche protects KMT2A-rearranged blasts from menin inhibition — a single study pointing the other way is enough to stop the speculative entry, not enough to establish the reverse.

    Evidence that argues against this (1)
    • The hypoxia result runs the opposite way to the intuition this beat is built on and should be flagged as such wherever niche protection is invoked: hypoxia SENSITISES KMT2A-rearranged AML to venetoclax plus MI-503 rather than protecting it. A resistance enumeration that treats the marrow microenvironment as a generic protective factor is not supported here. Cancer Discovery 2025
  5. E5moderate confidence 3 of 5: One strong study, or several smaller consistent ones.paper

    In patient-derived xenografts, three KMT2A-rearranged AMLs with high-risk fusion partners (MLLT10 and MLLT4/AFDN) and two infant ALL samples were sensitive to menin inhibition — so high-risk partner identity did not predict menin-inhibitor resistance in the only study that tested it directly.

    Upfront menin-inhibitor resistance in multiply pretreated leukemias. · Mahdavi L, Alikarami F, Goodrow H, et al. · Exp Hematol 2026
    What this evidence cannot show

    Five samples across two partners plus infant ALL; a negative in five samples is not a demonstration that partner identity never matters. Abstract read in full via NCBI efetch.

    Evidence that argues against this (1)
    • Fusion-partner identity does shape where the oncoprotein binds: partner-specific binding patterns over the ALL-like and AML-like gene sets correlate highly with how often each fusion appears in ALL versus AML. So partner identity is not biologically inert, and the rejection here is narrow — it says no study has linked partner identity to menin-inhibitor RESPONSE, not that partner identity is irrelevant. Janssens DH et al. 2024
  6. E6strong confidence 4 of 5: Replicated in peer-reviewed work by more than one group.paper

    A large-scale drug screen comparing genetic alterations and quantitative proteomics with drug sensitivity in primary AML cells found that GMP-like AML display the greatest sensitivity to the menin-KMT2A inhibitor bleximenib — the opposite direction from a differentiation-escape model of resistance.

    What this evidence cannot show

    Ex vivo sensitivity in primary AML, not clinical response, and not KMT2A-rearranged ALL. Abstract read in full via NCBI efetch.

    Evidence that argues against this (1)
    • The opposite framing exists in the same literature: leukaemia stem cells of MLL-rearranged AML show enhanced guanine nucleotide biosynthesis whose blockade drives myeloid differentiation and sensitises to menin inhibitors, implying the stem compartment is the less drug-responsive one. Neither study measures resistance directly, which is why differentiation escape was rejected as an entry rather than written in either direction. Shi X et al. 2025

    Revumenib in the registration-enabling phase 2 of AUGMENT-101 was administered at 163 mg (95 mg/m2 if weight under 40 kg) every 12 hours WITH a strong cytochrome P450 inhibitor, in 28-day cycles; in 57 efficacy-evaluable patients with relapsed/refractory KMT2A-rearranged acute leukaemia the CR+CRh rate was 22.8% and the overall response rate 63.2%.

    What this evidence cannot show

    Abstract read in full via NCBI efetch. Cited in this enumeration for one purpose only: to record that the pivotal efficacy data were generated under mandatory strong CYP3A4 inhibition, which is the fact that makes pharmacokinetic under-exposure a structurally plausible but entirely undocumented failure mode.

    Evidence that argues against this (1)
    • The dosing detail conceals a confound that matters for every efficacy number quoted from this trial. Revumenib was given WITH a strong CYP3A4 inhibitor, so the reported exposure-response relationship is not the drug's own, and the 22.8% CR+CRh was achieved under a co-administration regimen rather than by the agent alone. Cross-trial comparison with inhibitors dosed without a booster is not like-for-like. Leukemia & Lymphoma 2026
