ESR1 / estrogen receptor alpha - menin's best-documented non-leukaemic partner, and the only one a menin inhibitor is being given to patients to disrupt on purpose
also called ESR1 · estrogen receptor alpha · ERalpha · ER-alpha · NR3A1 · P03372
Menin, the protein this project wants to drug, has a second job that has nothing to do with leukaemia: it helps the oestrogen receptor switch genes on. That matters twice over. It is a thing the leukaemia drug will also switch off in everyone who takes it - and it is the reason the same class of drug is now being trialled deliberately in breast cancer, where switching it off is the point.
The same thing, for a scientist
ESR1 came out of `analyses/core-complex-interactome-gap` as a partner appearing against **four of the six core baits** (KMT2A, MEN1, PSIP1, AFF4) with no entry of its own. It is written here because it is **not** a leukaemia interactor claim and is important anyway. **The physiology.** Menin is a transcriptional coactivator of ERalpha: it is recruited to the endogenous oestrogen-responsive *TFF1*/pS2 promoter on activation, H3K4 trimethylation there rises, menin knockdown reduces both, and menin binds ERalpha **hormone-dependently**. *The majority of disease-related MEN1 mutations prevent the menin-ERalpha interaction, and those mutants are also defective as coactivators* [E1]. So the coactivator function is not incidental to MEN1 disease. **The pharmacology, and it runs both ways.** A 2025 CRISPR screen found menin as a regulator of KAT6A/B-inhibitor response, and combined KAT6A/B + menin inhibition is synergistic and antiproliferative in **ER+ but not ER- breast lines**, displaces KAT6A and menin-KMT2A from ER-driven promoters with selective Pol II loss there, and works in ER+ PDX and endocrine-resistant models [E2]. **The menin inhibitor is doing to ERalpha signalling exactly what this graph asks it to do to the fusion**, in a different tissue, on purpose.
Why an entry, when this is not a fusion interactor
The instrument that surfaced ESR1 cannot tell a fusion partner from a menin partner from an affinity-purification neighbour, and ESR1's four-bait spread is almost certainly two large-scale datasets rather than four independent biologies. That is not the argument for writing it.
The argument is that this graph carries an endocrine-liability beat whose whole subject is what a menin inhibitor does to normal menin-dependent tissue, and the best-characterised normal menin-dependent transcriptional function in the literature was absent from it. Before this entry, ESR1 and estrogen appeared in nine entries across 1,262, none of them about this.
It also changes the shape of the safety argument. The graph's endocrine work is built around parathyroid, pancreatic islet and pituitary tissue - the MEN1 syndrome organs. ERalpha coactivation is a different axis, present in breast, endometrium, bone and vasculature, and it is not covered by watching for the MEN1 tumour spectrum.
The same pocket, two intentions
In leukaemia this is a liability. In breast cancer it is the mechanism of action. The clinical menin inhibitors bind the MBM1 pocket on menin; Olsen and colleagues report that menin inhibition displaces menin-KMT2A from the promoters of ER-driven genes 2. Whatever fraction of menin's ERalpha coactivator function runs through KMT2A recruitment is therefore on-target for the leukaemia drug too.
Two honest limits on that reading.
First, Dreijerink's ERalpha contact is not shown to be the MBM pocket. The 2006 paper maps the interaction to menin and to MEN1 disease mutations, not to the peptide groove the drug occupies. The bridging claim - the drug's pocket is the ERalpha pocket - is not established by either paper and is not made here.
Second, the 2025 result is a combination result. Menin inhibition alone was not sufficient in those models; the synergy is with KAT6A/B inhibition. A monotherapy menin inhibitor at leukaemia doses may perturb ER programmes much less than the combination does.
What would settle it is the measurement neither paper makes: ER target-gene output, or menin occupancy at TFF1, in patients on revumenib or ziftomenib at leukaemia exposures. That is a tissue nobody biopsies in a leukaemia trial, which is exactly why this liability would be invisible if it existed.
Cell type, cell state, localisation, epigenetics
- Cell type. Breast epithelium, endometrium, ovary, bone, vascular endothelium, hypothalamus. Not a haematopoietic gene in any material sense, which is why it has no business in the leukaemia beats and every business in the liability beat.
- Cell state. Hormone-dependent by construction: the menin-ERalpha interaction in 1 is oestrogen-dependent, so the liability is state-conditional in a way most of this graph's targets are not. A post-menopausal patient and a pubertal patient are not the same exposure.
- Subcellular. Nucleus; cytoplasmic and membrane pools are reported for ERalpha and are not the relevant compartment here 3.
- Subnuclear. At ER target promoters, co-localised with menin-KMT2A and KAT6A 2.
- Epigenetics. The output measured in 1 is H3K4 trimethylation at TFF1, the same mark and the same complex this graph tracks at HOXA9 - which is the cleanest statement of why the leukaemia and the liability are one mechanism in two tissues.

}- Menin's ERalpha coactivator function is on-target biology, not an off-target effect. Any menin inhibitor given for leukaemia is also a partial inhibitor of oestrogen-driven transcription, and the leukaemia trials do not measure it.
- The liability is hormone-state-conditional. The menin-ERalpha interaction is oestrogen-dependent, so exposure is not the same in an infant, a pubertal child and an adult - and this project's population is infants, who will be on the drug before puberty and may reach it afterwards. Nothing in the trial record addresses that interval.
- The endocrine surveillance this graph has built watches the MEN1 tumour spectrum - parathyroid, islet, pituitary. ERalpha coactivation is a different axis in different tissue (breast, endometrium, bone, vasculature) and would not be caught by watching for MEN1 tumours.
- Bone is where this axis and the graph's existing evidence meet, and they agree. `risk-menin-loss-in-osteoblast-lineage-lowers-bone-mass` records that osteoblast-specific menin loss lowers bone mineral density in vivo while menin overexpression raises it - bidirectional, which is rare and strong. ERalpha signalling is independently required for skeletal maturation, so a menin inhibitor plausibly hits the same organ down two routes. The patients are infants who may be dosed for years, and `concept-endocrine-endpoint-absence-menin-trial-registries` establishes that **zero endocrine outcome measures** - including bone age and height velocity - appear across AUGMENT-101, KOMET-001 and cAMeLot-1.
- The opposite reading is also live and should not be suppressed: KAT6A/B and menin inhibitors are in clinical trials with manageable toxicity profiles reported, so the ER axis may simply tolerate partial menin blockade. The honest position is that nobody has looked in the leukaemia population.
- Does ERalpha bind menin at or near the MBM1 peptide groove that the clinical inhibitors occupy? Neither cited paper answers it, and it is the difference between an on-target liability and an unrelated function of the same protein.
- What happens to ER target-gene output, or to menin occupancy at TFF1, at leukaemia monotherapy exposures of revumenib or ziftomenib? Every quoted effect is a combination effect with a KAT6A/B inhibitor.
- ANSWERED for the three lead trials and left here as the scoped remainder: `concept-endocrine-endpoint-absence-menin-trial-registries` found zero endocrine outcome measures across AUGMENT-101, KOMET-001 and cAMeLot-1 on a complete registry read (2026-09-18), so no growth, bone-density or pubertal endpoint exists to collect ER-relevant signal. What is genuinely open is narrower: whether any menin-inhibitor protocol ANYWHERE - including investigator-initiated and the breast-cancer combination trials, where the population is oestrogen-driven by definition - collects an ER-axis endpoint that could be read across.
- Read both papers in full - neither was fetched beyond its abstract for this entry, and PMC12281420 is available.
- If ERalpha supports normal HSC self-renewal (PMID 24451543) and a menin inhibitor disrupts menin-ERalpha, does menin inhibition carry a sex-dependent cost to the normal stem compartment? No trial of revumenib, ziftomenib or bleximenib reports a sex-stratified count-recovery analysis.

Menin is a transcriptional coactivator of the estrogen and vitamin D nuclear receptors. Activation of the endogenous estrogen-responsive TFF1/pS2 gene recruits menin to the promoter and raises H3K4 trimethylation there; menin knockdown reduces both activated TFF1 transcription and H3K4 trimethylation. Menin interacts directly with ERalpha in a hormone-dependent manner, the majority of disease-related MEN1 mutations prevent that interaction, and those ERalpha-interacting mutants are also defective in coactivator function.
Menin links estrogen receptor activation to histone H3K4 trimethylation · Dreijerink KM, Mulder KW, Winkler GS, Höppener JW, Lips CJ, Timmers HT · Cancer Research 2006What this evidence cannot show
Read from the abstract on 2026-09-20; **full text not fetched** - no PMCID in Europe PMC, and route 5 was not attempted for this entry, so the promoter occupancy and knockdown magnitudes are not quoted here. Established as a result within the reported experiments under the grading rule. The work is in breast cancer and cell-line systems from 2006 and is **not** a statement about menin in haematopoiesis.
Evidence that argues against this (1)
- The interaction is mapped to menin and to MEN1 disease mutations, **not** to the MBM peptide groove that revumenib, ziftomenib and bleximenib occupy. So this paper on its own does not establish that a clinical menin inhibitor disrupts ERalpha coactivation - that inference needs the 2025 displacement result, and even that is a combination experiment. CRISPR screens identified menin as a regulator of KAT6A/B-inhibitor response. Co-treatment with KAT6A/B and menin inhibitors is synergistically antiproliferative in ER-positive but not ER-negative breast cancer lines; KAT6A and menin-KMT2A cooperatively regulate ER-driven gene expression through direct effects on ESR1 expression and co-localisation at ER target genes; combined inhibition displaces both from ER-driven promoters with selective RNA polymerase II chromatin loss there; and the combination is effective in ER-positive patient-derived xenografts and in multiple models of endocrine resistance.
Combined inhibition of KAT6A/B and Menin reverses estrogen receptor-driven gene expression programs in breast cancer · Olsen SN, et al., Armstrong SA · Cell Reports Medicine 2025What this evidence cannot show
Read from the abstract on 2026-09-20 (PMC12281420 exists and the full text was not fetched for this entry). Established as a result within the reported models. **Every effect quoted is a COMBINATION effect**; menin inhibition alone was not reported as sufficient, so nothing here licenses a claim about what a menin inhibitor does to ER programmes at leukaemia monotherapy exposures. Cell lines and PDX, not patients.
Evidence that argues against this (1)
- The authors' own framing points the other way from a liability reading: they note KAT6A/B and menin inhibitors are in clinical trials **with manageable toxicity profiles**, which is an argument that disrupting this axis is tolerated. A tolerated toxicity profile in adults with breast cancer is not evidence about infants on a menin inhibitor, in either direction. Dreijerink KM et al. 2006

Database comment on one day. The multi-compartment picture is isoform-dependent, and a claim about ERalpha localisation that does not say which isoform is under-specified (the PKM2 failure shape). Every coded citation is non-haematopoietic.
Evidence that argues against this (1)
- Quoting UniProt alone would overstate the consensus, because the two databases do not agree on the DOMINANT compartment. HPA's independent antibody immunofluorescence returns Vesicles as ESR1's main location with nucleoplasm only additional. This entry argues that HPA is probably wrong, but the honest position is that one database's coded nuclear call and another's vesicular call cannot both be quoted as settled; the multi-compartment picture rests on isoform-resolved experiments in tissues where ERalpha is actually expressed, and neither database is isoform-resolved. Human endothelial cells - not haematopoietic and not leukaemic. Abstract-and-title level. This supports the existence of the membrane pool, not its size in any other cell type.
Evidence that argues against this (1)
- The existence of a functional membrane pool does not license reading membrane ERalpha into this graph's argument. Every menin-relevant ESR1 result is NUCLEAR and transcriptional - menin links oestrogen-receptor activation to H3K4me3, and combined KAT6A/B and menin inhibition reverses ER-driven gene-expression programmes in breast cancer. The membrane pool has no menin connection at all, so it belongs in the localisation record as a compartment and must not be carried into any statement about what a menin inhibitor will do to ER signalling.
