Auditing the seven strikes before applying them: nine of ten decisions stand, four for different reasons than were given, and one clinical argument fails
also called target list rewrite audit · strike audit cycle 22 · the strikes, re-checked
Another team argued that seven candidate drug targets should be dropped. Before dropping them we went back and read every paper they cited. Almost all the decisions were right, but several were right for the wrong reason, and one of the arguments does not survive reading the trial it rests on.
The same thing, for a scientist
The cycle-21 counter-evidence sweep of `data/graph/nodes/interactome2/` concluded that PRMT1, RBBP5, ASH2L, CTR9, CDC73, CCNT1 and ELL2 should be struck, MEF2C redefined, MEIS1 demoted and KDM6A reframed. This entry re-fetched every cited PMID and the cited trial registration, read each paper's title and abstract, and read two in full text. All fourteen PMIDs resolve to the papers the sweep says they are — no repeat of the five unrelated-PMID incidents. But a citation that resolves is not a citation that supports, and on that test the results diverge: five decisions are confirmed exactly as argued, four are confirmed with the stated reasoning materially corrected, and one — CCNT1 — survives on a different leg entirely, because the clinical evidence offered for it does not say what it was said to say. The decisions are recorded here beside the original list rather than replacing it.
The verdicts
| Decision | Cited PMID | Resolves? | Supports the claim? | Verdict |
|---|---|---|---|---|
| PRMT1 — strike | 31395602, NCT03666988 | yes | partly | Strike, reasoning corrected. The FLT3 paper argues for PRMT1 inhibition, not against it. The real fusion-specificity objection is in a paper the sweep cited for something else. |
| RBBP5 — strike | 24813891 | yes | yes | Strike, confirmed. |
| ASH2L — strike | 24813891, 27563068 | yes | yes | Strike, confirmed. |
| CTR9 — strike | 25099282 | yes | yes, but | Strike, reasoning corrected. The germline-tumour-suppressor argument, applied consistently, would disqualify menin. |
| CDC73 — strike | 12434154 | yes | yes, but | Strike, reasoning corrected. Same objection; pan-essentiality is the load-bearing reason, not HPT-JT. |
| CCNT1 — strike | 30370956 | yes | no | Strike on the co-dependency leg only. The clinical leg fails. |
| ELL2 — strike | 25238757 | yes | the opposite | Strike, reasoning inverted. The cited paper shows ELL2 is not dispensable. |
| MEF2C — redefine | 29431698 | yes | yes | Redefine, confirmed. |

Every PMID above was fetched from NCBI this cycle and its title and abstract read 1. Two — 26766589 and 38989615 — were read in full text. Nothing was cited from memory, and one identifier this agent recalled from memory during the work (a purported MEN1 cloning paper) turned out to be a telomerase paper, which is the sixth instance of the failure mode this project keeps hitting and the reason the rule exists.
PRMT1: the right answer from the wrong paper1 deeper
The sweep's case was that PMID 31395602 shows PRMT1's oncogenic function in MLL-r ALL runs through FLT3 R972/R973 methylation, "relocating the target to a kinase already drugged."
The paper says the first half and rejects the second. Its closing sentence is: "These results indicate that abolishing FLT3 arginine methylation through PRMT1 inhibition represents a promising strategy to target MLL-r ALL cells" 2. It is an argument for a biomarker-selected PRMT1-inhibitor combination in exactly this disease — MS023 plus PKC412 outperformed PKC412 alone in patient-derived xenografts. Cited as a reason to strike PRMT1, it is cited against its own conclusion.
The decisive fusion-specificity objection is elsewhere, and the sweep had the paper in hand: PMID 26766589, which it cited for the KDM4C circuit. Read in full, that paper reports a screen of Prmt1 shRNA across more than ten MLL and non-MLL fusions and states that "while transformation mediated by most of the other MLL fusions were not affected by Prmt1 knockdown, MLL-GAS7 and MOZ-TIF2 exhibited high degrees of Prmt1 dependence", and separately that "MLL-AF9 recruits only Kdm4c but not Prmt1" 3. Most MLL fusions do not need PRMT1; the two that do are not KMT2A::AFF1; and KMT2A::AFF1 was not among the dependent fusions.
That, with pan-essentiality and a terminated first-in-human trial 4, strikes PRMT1 as a KMT2A::AFF1 complex member and as a standalone target. It does not strike the MLL-r ALL combination hypothesis, which remains a live, published, biomarker-defined proposal that this project is simply not pursuing. The register should say so rather than imply the literature is against it.
What the terminated trial does and does not establish
NCT03666988 is confirmed TERMINATED: a GSK phase 1 first-in-human of GSK3368715 in solid tumours and DLBCL, 31 participants, adults 18 and over, primary completion 2021-03-04, terminated during Part 1 so the Part 2 expansion analyses were never performed 4. The registry record carries no leukemia cohort and no paediatric cohort.
So the trial establishes that one PRMT1 inhibitor failed in adults with solid tumours and lymphoma. It is weak evidence about a different compound, at a different exposure, in a biomarker-selected paediatric leukemia — which is precisely what the counter-evidence already attached to the interactome entry says, and it is right.
CTR9 and CDC73: a safety argument the project has already overruled for its own lead target
Both citations check out. CTR9: inactivating germline mutations in 3 of 35 Wilms tumour families and 0 of 1,000 controls, P < 0.0001, segregating with disease with a second mutational event in available tumours 5. CDC73/HRPT2: thirteen heterozygous germline inactivating mutations in fourteen HPT-JT families plus three somatic inactivating mutations in 48 parathyroid adenomas, none in controls 6. Both are bona fide germline tumour suppressors. Neither result is in doubt.
The inference is. The argument as written is this gene is a germline tumour suppressor in a childhood or familial cancer, therefore reducing its function pharmacologically in a child is unacceptable. Run that argument on MEN1:
- MEN1 is a tumour suppressor gene; germline inactivating mutations cause multiple endocrine neoplasia type 1, whose defining lesion is neuroendocrine parathyroid disease, and sporadic tumours show biallelic MEN1 inactivation by mutation plus loss of the other allele 78.
- The syndrome caused by germline CDC73 loss is hyperparathyroidism-jaw tumour syndrome. The syndrome caused by germline MEN1 loss includes primary hyperparathyroidism in essentially all patients by age 50.
- Menin inhibitors are FDA-approved, and revumenib's own label reports increased intact parathyroid hormone in 34% of 241 patients, so the predicted endocrine effect is not hypothetical — it is measured, accepted, and being monitored rather than treated as disqualifying.
The project cannot hold that germline-parathyroid-tumour-suppressor status forbids targeting and simultaneously pursue menin. Either the argument is wrong or the lead programme is. It is the argument that is wrong: germline haploinsufficiency across a lifetime and partial reversible occupancy over months are different exposures, which ana-target-safety-triage-v2 already says in the constraint context and which applies verbatim here.
The strikes still stand, on the reasons that were already sitting underneath them. CTR9: no primary study tests CTR9 individually in KMT2A-fusion leukemia, the MEN1 co-dependency is r = 0.006 with a confidence interval straddling zero, and it is DepMap common-essential. CDC73: DepMap common-essential at -1.122 with 98.7% of lines depleted. Those are ordinary, sufficient reasons. The predisposition genetics belongs in bite_back as an added caution, not in the kill line as the argument.
CCNT1: the trial says something close to the opposite
This is the one decision whose stated evidence does not survive reading. The claim was: P-TEFb inhibition was run in AML with 7 of 9 treatment-related deaths in the alvocidib arm.
The arithmetic is right and everything around it is wrong 9. In that randomised phase II of three regimens in 90 relapsed/refractory AML patients:
- the alvocidib arm (FLAM) had the best response of the three: 28% against 14% and 16%;
- there were 9 treatment-related deaths and 7 were in the FLAM arm, 4 of those in elderly patients;
- the FLAM arm was older: median 62 against 55 for both comparators;
- alvocidib is a pan-CDK inhibitor given here with cytarabine and mitoxantrone, not a selective CDK9 agent given alone;
- and the authors conclude that FLAM "had an encouraging response rate and should be considered for further clinical development but should be used with caution in elderly patients."
A confounded age imbalance, a multi-CDK drug, a three-drug regimen, and an authors' recommendation to continue development. That is not "P-TEFb inhibition in AML produced toxicity, not selectivity." It is a caution about elderly patients in a combination regimen, which is close to irrelevant to infants.
The graph already holds the other half of this picture and the two halves disagree: target-cdk9-inhibition-kmt2a-r cites a larger alvocidib trial — 165 patients, newly diagnosed AML — reporting a significantly higher complete-remission rate than 7+3 without increased toxicity, and no overall or event-free survival difference. Same drug, two randomised trials, opposite toxicity readings, and the real problem in the larger one is not toxicity at all but that extra remissions did not become extra survivors.
CCNT1 is still struck, because the reasons that do hold are untouched: its MEN1 co-dependency (r = 0.349) fails correction in the 16-gene panel and falls below every survivor in the 2,743-gene deep-pool null, and CDK9 rather than cyclin T1 is where the chemistry, the structures and the clinical-stage compounds are. But the mortality argument must not be propagated onward as a reason to stop CDK9 work, and this entry exists partly to stop that happening.
ELL2: right decision, inverted reason
The stated reason was that ELL2 is "the dispensable paralogue beside pan-essential ELL." The cited paper shows the opposite of dispensable. B-cell-conditional Ell2 knockout mice have curtailed humoral responses to two immunogens and diminished recall responses, fewer plasma cells in spleen, fewer IgG1 antibody-producing cells in marrow, LPS-stimulated plasmablasts four-fold less abundant with a paucity of secreted immunoglobulin heavy chain and distended abnormal endoplasmic reticulum, and severely reduced Ig kappa, ATF6, BiP, Cyclin B2, OcaB and XBP1 10.
ELL2 is not dispensable. It is not required for proliferation in culture, which is all a Chronos score of -0.064 at 1.0% of lines depleted can measure, while its paralogue ELL sits at -1.302 with 94.0%.
So the correct statement is a conjunction, and it is stronger than the original: ELL2 is not the required SEC paralogue in leukemia, and blocking it would cost antibody secretion in a child already immunosuppressed by therapy. One of those is a reason it would not work; the other is a reason it would hurt. Calling it "dispensable" asserts the second is false.
The five that need no correction
RBBP5 and ASH2L. PMID 24813891 says exactly what the sweep says: the SET domain, and thus the histone methyltransferase activity of MLL1, is dispensable for maintaining HSCs and for supporting MLL-AF9-driven leukemogenesis; what is lost is H4K16 acetylation, and SIRT1 inhibition restores it 11. With KMT2A fusions not retaining the SET domain in any case, the rationale for both subunits — enabling MLL1 SET catalysis — has no load left to carry. PMID 27563068 independently shows WRAD is not a fixed four-subunit unit: without WDR5, MLL3 still binds RAD and shows enhanced activity 12.
One caveat the sweep did not state and this audit will: no paper tests RBBP5 or ASH2L in KMT2A::AFF1 either way. The strike removes an inclusion that rested on an inference, and replaces it with a different inference. That is an improvement, not a demonstration, and if anyone later shows a non-catalytic requirement for either subunit the strike should reverse without embarrassment.
MEF2C — redefine. Confirmed 13. Knock-in mice blocking MEF2C phosphorylation have normal haematopoiesis yet resist MLL-fusion leukemogenesis; MRT199665 caused apoptosis and chemosensitivity only in MEF2C-activated lines and primary specimens. The handle is the upstream MARK/SIK kinase and the activating phosphorylation, not MEF2C dosage — which also dissolves the 5q14.3 haploinsufficiency objection, since blocking a phosphorylation does not lower protein level.
MEIS1 — demote. Confirmed 14. Conditional Meis1 knockout in adult mouse haematopoiesis reduces HSPC numbers cell-autonomously through impaired self-renewal and reduced quiescence, causing stem-cell exhaustion and defective long-term haematopoiesis, and downregulates Pbx1-dependent HSC signature genes. The LSC function is the HSC function. MEIS1 also fails Holm correction in the deep-pool null at 0.067.
KDM6A — reframe. Confirmed on all three papers 151617. In CML, of five upregulated KDMs only KDM6A depletion sensitised cells to imatinib-induced apoptosis, and both wild-type and demethylase-dead KDM6A restored resistance — the authors' own words are that this is "contrary to its reported role as a tumor suppressor and independent of its demethylase function." In AML, higher KDM6A expression correlates with venetoclax tolerance and KDM6A loss sensitises to venetoclax, while KDM6A-mutant xenografts are sensitive to olaparib. And in SMARCA4-deficient tumours KDM6A/KDM6B blockade is actively therapeutic. KDM6A is a context-dependent, therapy-selection variable, and the catalytic activity a KDM6 inhibitor would target is not the activity doing the work in CML.
What this audit implies about how strikes should be recorded
Four of ten decisions were right for a reason that does not survive reading the paper. In every one of those four the decision was still correct, because a second, quieter reason was already sitting underneath it. That is a comforting result and also a warning: the strike list was robust to its own reasoning being wrong, which means the reasoning was not what was doing the work, which means nobody would have noticed if a decision had been wrong.
Two practices follow, and both are cheap:
- A strike records its load-bearing reason separately from its supporting reasons, so that if the load-bearing one fails the decision is automatically re-opened rather than silently propped up.
- A safety argument is tested against menin before it is used. MEN1 is common-essential-adjacent, strongly constrained, a germline tumour suppressor, and causes an endocrine syndrome the approved drug measurably recapitulates. Any safety rule that would have killed menin is a rule that is too strict for this project, and three of this cycle's ten decisions invoked one.

}- A strike is an intervention on the project, and the cost of a wrong strike is a target that is never looked at again and leaves no trace of having been considered. That cost is invisible by construction, which is why the struck entries and the reasons are kept in full rather than deleted.
- This audit is itself a claim and could be wrong in the same way. Twelve of the fourteen papers were read at abstract level, and an abstract can conceal the limitation that would change the verdict — the CCNT1 correction was only possible because the abstract happened to report both the age imbalance and the authors' conclusion.
- Correcting the reasoning while keeping the decision is the most seductive failure mode available here: it looks like rigour and produces no change. In four cases the decision genuinely survived on an independent reason. If a fifth had been propped up by nothing, the temptation to find a reason for it would have been real.
- The menin parallel used against the CTR9 and CDC73 arguments cuts both ways. If germline tumour-suppressor status really is a weak objection, then it is weak for menin too, and the endocrine late-effects risk this graph already tracks for revumenib is being underweighted rather than the CTR9 risk overweighted.
- Four of ten decisions were right for a reason that fails on reading. How many earlier decisions in this graph rest on a load-bearing reason nobody has re-read, and is there a cheap way to find out other than re-reading all of them?
- Is there any safety rule currently applied in this project that menin would pass? If not, the project is using rules it does not believe, and the honest move is to replace them with one rule stated once.
- The PRMT1 combination hypothesis (MS023 plus a FLT3 inhibitor, selected on FLT3 R972/973 methylation) is published, unrefuted, and in the right disease. Is declining it a resourcing decision or a scientific one, and does the register say which?
- Nobody has tested RBBP5 or ASH2L in KMT2A::AFF1 in either direction. Given how cheap a single knockdown with a HOXA9 readout in SEM would be, is the strike a conclusion or a decision not to look?

This establishes identity only, which is precisely the limit `scripts/verify-citations.py` documents. Twelve of the fourteen were read at abstract level and two (26766589, 38989615) in full text; the support judgements in the table above are therefore abstract-level for twelve papers, and an abstract can omit a limitation that the methods section would reveal. No paper was assessed for methodological quality.
Evidence that argues against this (1)
- Fetching a record is not the same as reading the right field of it, and this audit's own cycle supplies the demonstration. The registry entry for NCT03666988 was fetched and read here, and E4 was written saying the record 'does not state the sponsor's reason for termination'. Queried through the ClinicalTrials.gov v2 API this session, the same record's statusModule carries whyStopped: 'Overall benefit-risk profile did not support continuation of the study'. The field was always there; the MCP tool used to read the record does not return it. So an audit that verifies identity - the PMID resolves, the NCT resolves - certifies the weakest property a citation has, and the failure mode that actually bit here was depth of read, not identity. The paper cited to strike PRMT1 concludes in favour of PRMT1 inhibition in MLL-rearranged ALL: it reports that MS023 killing parallels baseline FLT3 R972/973 methylation and that MS023 plus PKC412 eliminated MLL-r ALL cells better than PKC412 alone in patient-derived xenografts, ending 'abolishing FLT3 arginine methylation through PRMT1 inhibition represents a promising strategy to target MLL-r ALL cells'.
Targeting PRMT1-mediated FLT3 methylation disrupts maintenance of MLL-rearranged acute lymphoblastic leukemia · See PubMed record PMID 31395602 for the full author list (not transcribed here) · Blood 2019What this evidence cannot show
Abstract read, not full text. 'MLL-rearranged ALL' is not resolved to partner in the abstract, so whether KMT2A::AFF1 specifically was among the models cannot be determined without the methods. The combination result is a patient-derived xenograft experiment, not a trial. The point made here is about what the paper concludes, which the abstract states unambiguously, rather than about how well it supports that conclusion.
Evidence that argues against this (1)
- The compound in that paper is not a PRMT1 inhibitor, so neither its killing nor its conclusion can be assigned to PRMT1 specifically. Eram et al., who developed MS023, characterised it as an inhibitor of human TYPE I PRMTs as a class: high potency against PRMT1, PRMT3, PRMT4 (CARM1), PRMT6 and PRMT8, inactive against type II and type III PRMTs, protein lysine methyltransferases and DNA methyltransferases, binding the substrate site as shown by a PRMT6 co-crystal, and reducing global asymmetric dimethylarginine in cells. 'MS023 killing parallels baseline FLT3 R972/973 methylation' is therefore evidence about type I PRMT activity in aggregate; the authors' closing sentence about PRMT1 inhibition is an interpretation their tool cannot separate from CARM1 or PRMT6. A screen of Prmt1 shRNA across more than ten MLL and non-MLL oncogenic transcription factors found that transformation mediated by most MLL fusions was unaffected by Prmt1 knockdown, with only MLL-GAS7 and MOZ-TIF2 showing high Prmt1 dependence, and that MLL-AF9 recruits KDM4C but not PRMT1.

Full text read (PMC4712026). The screen is a retroviral transduction/transformation assay in mouse c-kit+ HSPCs, which is a colony-formation readout in a mouse primary-cell model, not a human KMT2A::AFF1 system; and the fusions tested are not individually enumerated in the text read, so 'KMT2A::AFF1 was not among the dependent fusions' rests on the two named dependent fusions being MLL-GAS7 and MOZ-TIF2. Absence of KMT2A::AFF1 from a mouse transformation screen is weaker than a negative result in the human disease, and none exists.
Evidence that argues against this (1)
- The genetic screen and the pharmacology are not measuring the same thing, which is a reason they can both be right and a reason neither settles the strike. Eram et al. showed MS023 inhibits PRMT1, PRMT3, PRMT4, PRMT6 and PRMT8 with no activity against type II or III enzymes - so a fusion whose transformation survives Prmt1 knockdown may still be sensitive to blockade of the whole type I family, because the remaining type I enzymes that cover for Prmt1 loss are also inhibited. A shRNA screen that finds 'most MLL fusions unaffected' is evidence against a single-gene PRMT1 dependency and is not evidence against type I PRMT inhibition as a strategy; the strike as written conflates the two. Registry record fetched and read; the whyStopped field was re-read directly from the ClinicalTrials.gov v2 API on 2026-09-19. CORRECTED 2026-09-19: this caveat previously read "the record text read does not state the sponsor's reason for termination, so the widely repeated attribution to 'lack of clinical efficacy and dose-limiting toxicities' is not verified from this source and is not asserted here". That was wrong - the field exists and the MCP registry tool simply does not return it. The sponsor's stated reason is 'Overall benefit-risk profile did not support continuation of the study'. It is a composite judgement and does not itself say whether efficacy, toxicity or both drove the decision, so the commonly repeated attribution remains unverified, but the claim that the registry is silent is withdrawn. The trial had no leukemia cohort and no paediatric cohort, so its relevance to KMT2A::AFF1 infant leukemia is indirect in population, disease and compound.
Evidence that argues against this (1)
- CORRECTION (2026-09-19). This item previously stated that the registry record 'does not state the sponsor's reason for termination' and declined to assert one. That is withdrawn: the reason is in the record and was missed because the MCP registry tool does not return the whyStopped field. Queried directly through the ClinicalTrials.gov v2 API this session, NCT03666988 carries whyStopped: 'Overall benefit-risk profile did not support continuation of the study', with statusVerifiedDate 2022-02 and results first posted 2022-05-20. The sponsor's stated reason is a benefit-risk judgement, which is neither the 'lack of efficacy and dose-limiting toxicities' commonly repeated nor the absence of a reason this entry asserted; it is a single composite sentence and it does not say which side of the ratio failed. Inactivating germline CTR9 mutations were identified in 3 of 35 Wilms tumour families by exome and Sanger sequencing and in none of 1,000 population controls (P<0.0001), each segregating with disease with a second mutational event in available tumours, establishing CTR9 as a Wilms tumour predisposition gene acting as a tumour suppressor.
Germline mutations in the PAF1 complex gene CTR9 predispose to Wilms tumour · Hanks S, Perdeaux ER, Seal S, et al. · Nature Communications 2014What this evidence cannot show
Abstract read, not full text. Three families is a small numerator and the P value comes from the contrast with 1,000 controls rather than from the family count. The finding concerns lifelong germline heterozygous loss in kidney development and is not a statement about transient pharmacological inhibition in haematopoiesis; it is cited here as a caution, not as the reason for the strike.
Evidence that argues against this (1)
- CTR9's germline tumour-suppressor role in the kidney runs opposite to what the PAF1 complex does in this disease, where it is a positive, selectively targetable requirement. Muntean et al. used a conditional allele of the PAFc subunit Cdc73 to show the complex is indiscriminately necessary for AML cell proliferation through epigenetic regulation of proleukemogenic targets including MEIS1 and Bcl2, then engineered a dominant-negative MLL fragment that binds PAFc: disrupting the MLL-PAFc interaction selectively inhibited MLL leukaemic cells while sparing E2A-HLF-transformed cells, and in vivo reconstitution assays showed normal haematopoietic stem cell function was unaffected. A strike on a PAFc subunit justified by a Wilms tumour predisposition is therefore struck against the direction of the only leukaemia-specific separation-of-function experiment in the literature. Thirteen distinct heterozygous germline inactivating mutations in HRPT2/CDC73, encoding parafibromin, were identified in fourteen families with hyperparathyroidism-jaw tumour syndrome, with three somatic inactivating mutations among 48 parathyroid adenomas and none in normal controls, supporting HRPT2 as a tumour-suppressor gene.
HRPT2, encoding parafibromin, is mutated in hyperparathyroidism-jaw tumor syndrome · Carpten JD, Robbins CM, Villablanca A, et al. · Nature Genetics 2002What this evidence cannot show
Abstract read, not full text. A 2002 positional-cloning study; the tumour-suppressor conclusion is stated by the authors as 'suggested' by the data. Same exposure-mismatch caveat as CTR9: germline heterozygous loss over a lifetime is not partial reversible inhibition over months.
Evidence that argues against this (1)
- For CDC73 specifically the counter-evidence is not an analogy but the same gene. Muntean et al. deleted Cdc73 with a conditional allele and found the PAFc is necessary for AML cell proliferation via epigenetic regulation of MEIS1 and Bcl2, and that targeted disruption of the MLL-PAFc interface selectively inhibits MLL leukaemic cells without altering normal haematopoietic stem cell function in reconstitution assays. Parafibromin's tumour-suppressor status in hyperparathyroidism-jaw tumour syndrome describes lifelong germline heterozygous loss in parathyroid tissue; the leukaemia experiment describes acute, complete loss in AML cells and a normal-marrow control. Both can be true, and only the second is about the intervention being struck. Muntean AG et al. 2013

MEN1 is itself a tumour-suppressor gene whose germline inactivation causes multiple endocrine neoplasia type 1, a syndrome defined by neuroendocrine parathyroid, pancreas, duodenum and pituitary lesions, with inactivating germline frameshift, nonsense, missense and in-frame deletion mutations scattered through its ten coding exons in gene carriers.
Multiple endocrine neoplasia type 1 and 2: from morphology to molecular pathology 1997 · Komminoth P · Verhandlungen der Deutschen Gesellschaft fur Pathologie 1997What this evidence cannot show
Abstract read. A 1997 review, included here as a map of an uncontroversial clinical genetics fact rather than as primary evidence, per the standing preference for primary sources. It is nearly thirty years old; the MEN1 tumour-suppressor status it describes has not been revised since, but any quantitative claim about penetrance should be taken from a modern source.
Evidence that argues against this (1)
- The tumour-suppressor-therefore-do-not-inhibit inference has now been tested in humans against this exact protein, and it did not play out as the inference predicts. In AUGMENT-101, 94 patients aged 1.3 to 75 years received the menin-KMT2A interaction inhibitor revumenib; grade 3 or worse events were febrile neutropenia (37.2%), differentiation syndrome (16.0%) and QTc prolongation (13.8%), with a CR+CRh rate of 22.8% and overall response 63.2% in the efficacy-evaluable KMT2A-rearranged population. Whatever the risks of blocking a tumour suppressor's interface, they did not manifest as prohibitive short-term toxicity, and a registration-enabling cohort now exists. Follow-up is far too short to exclude a neoplasia risk, which is the version of the concern that the MEN1 genetics actually supports. Issa GC et al. 2025

Biallelic MEN1 inactivation by mutation plus loss of the remaining allele was found in 4 of 11 sporadic lung carcinoids, with the tumour from a MEN1 patient showing 11q13 loss of heterozygosity and a germline MEN1 mutation — the classic two-hit pattern of a tumour-suppressor gene.
Identification of MEN1 gene mutations in sporadic carcinoid tumors of the lung · Debelenko LV, Brambilla E, Agarwal SK, et al. · Human Molecular Genetics 1997What this evidence cannot show
Abstract read, not full text. n=11 sporadic tumours plus one MEN1-patient tumour, in lung carcinoid rather than the parathyroid lesions that matter most for the parallel drawn here. Cited to establish the two-hit tumour-suppressor mechanism for MEN1, which is the point the parallel needs; the parathyroid phenotype comes from E7.
Evidence that argues against this (1)
- The two-hit mechanism is about biallelic loss sustained over a lifetime in a tissue; the intervention it is cited against is months of interface blockade in another tissue, and the empirical answer for that intervention is now on record. AUGMENT-101 treated 94 patients with revumenib, including children from 1.3 years old, and reported a predictable safety profile dominated by febrile neutropenia, differentiation syndrome and QTc prolongation rather than by anything resembling a loss-of-tumour-suppressor phenotype. The honest residual concern from lung carcinoid and parathyroid genetics is long-latency second neoplasia in survivors, which no trial of this length can address and which nobody is currently measuring in this population. In a randomised phase II of three regimens in 90 relapsed/refractory AML patients, the alvocidib-containing FLAM arm had the highest overall response (28% vs 14% and 16%) and 7 of the trial's 9 treatment-related deaths, 4 of those in elderly patients, with a median age of 62 in the FLAM arm against 55 in the comparator arms, and the authors concluded FLAM 'had an encouraging response rate and should be considered for further clinical development but should be used with caution in elderly patients'.
A randomized trial of three novel regimens for recurrent acute myeloid leukemia demonstrates the continuing challenge of treating this difficult disease · See PubMed record PMID 30370956 for the full author list (not transcribed here) · American Journal of Hematology 2018What this evidence cannot show
Abstract read, not full text. The arms are small and unequal (35, 36 and 19 patients) and a Simon two-stage design per arm is not a powered head-to-head comparison, so neither the response advantage nor the death imbalance should be read as an estimated treatment effect. The age imbalance is stated in the abstract but no adjusted analysis is reported there, so the confounding is visible rather than quantified. Alvocidib is a multi-CDK inhibitor administered with cytarabine and mitoxantrone, so no observed effect can be attributed to CDK9 or P-TEFb specifically.
Evidence that argues against this (1)
- A second randomised trial of the same drug reports the opposite toxicity finding, which is why neither should be used as a general verdict on P-TEFb inhibition. In 165 newly diagnosed intermediate/adverse-risk AML patients, FLAM gave a significantly higher complete remission rate than 7+3 (70% vs 46%, p=0.003) WITHOUT increased toxicity, and with no significant difference in overall or event-free survival. The real objection to this class is that extra remissions did not become extra survivors, which is a very different problem from treatment-related mortality. Zeidner JF et al. 2015

B-cell-conditional Ell2 knockout mice show curtailed humoral and recall responses, reduced splenic plasma cells and marrow IgG1-producing cells, four-fold fewer LPS-stimulated plasmablasts with a paucity of secreted immunoglobulin heavy chain and distended abnormal endoplasmic reticulum, and severely reduced Ig kappa, ATF6, BiP, Cyclin B2, OcaB and XBP1 — so ELL2 is required for antibody secretion and the unfolded protein response, not dispensable.
Transcription elongation factor ELL2 drives Ig secretory-specific mRNA production and the unfolded protein response · Park KS, Bayles I, Szlachta-McGinn A, et al. · Journal of Immunology 2014What this evidence cannot show
Abstract read, not full text. Mouse, and a CD19-cre B-cell-restricted deletion, so it models complete loss in one lineage rather than systemic partial pharmacological inhibition. It says nothing about leukemia and nothing about KMT2A fusions; it is cited solely to show that the word 'dispensable' is the wrong description of ELL2 and that the strike needs a two-part reason.
Evidence that argues against this (1)
- A lineage-restricted conditional-null phenotype in mice is a weak predictor of what a partial inhibitor does in patients, and this disease supplies the worked example. Menin is a bona fide tumour suppressor whose germline inactivation causes a multi-organ neoplasia syndrome, and blocking its KMT2A interface in 94 patients from 1.3 to 75 years old produced febrile neutropenia, differentiation syndrome and QTc prolongation - an ordinary oncology safety profile - alongside a 22.8% CR+CRh rate. The ELL2 objection is right that 'dispensable' is the wrong word for the published mouse phenotype; it does not follow that the phenotype forecasts the clinical liability, in either direction, and the register for both should be 'unmeasured'. The SET domain, and thus the histone methyltransferase activity, of MLL1 is dispensable for maintaining haematopoietic stem cells and for supporting leukemogenesis driven by the MLL-AF9 fusion oncoprotein; on Mll1 deletion H4K16 acetylation is selectively depleted at MLL1 target genes alongside reduced transcription, and SIRT1 inhibition is sufficient to prevent both.
The histone methyltransferase activity of MLL1 is dispensable for hematopoiesis and leukemogenesis · Mishra BP, Zaffuto KM, Artinger EL, et al. · Cell Reports 2014What this evidence cannot show
Abstract read, not full text. Mouse, MLL-AF9 not KMT2A::AFF1, and concerns wild-type MLL1's SET domain. Critically for the two strikes it supports, the study does not test RBBP5 or ASH2L at all: the strike is an inference that a subunit whose function is to enable a dispensable catalysis is itself dispensable, which is reasonable but is not a result. A demonstrated non-catalytic requirement for either subunit would reverse it.
Evidence that argues against this (1)
- This item is the single mouse result on which two of the strikes rest, and a later paper in this same graph reports the opposite about the wild-type allele. Chen et al., using independent conditional animal models, found endogenous MLL1 DISPENSABLE for MLL-rearranged leukaemia; deleting the closest paralogue Mll2 alone significantly impaired survival of MLL-AF9-transformed cells, additional Mll1 loss further reduced viability and proliferation, and the two collaborate by regulating distinct pathways rather than redundantly, leading the authors to nominate MLL2 as the drug target in MLL-rearranged leukaemia. Patricia Ernst is senior author on this and on much of the preceding wild-type-MLL1 literature, so this is the field correcting itself. If the H3K4 methyltransferase that matters in this disease is MLL2 rather than MLL1, an argument that strikes RBBP5 and ASH2L because MLL1's catalysis is dispensable is reasoning about the wrong enzyme. WRAD is not a fixed obligatory four-subunit module: mutating the Win motif-WDR5 interface severely disrupts MLL1 and SETD1A complexes but only modestly disrupts MLL2/4 and SETD1B without significantly altering their in vitro activity, and in the absence of WDR5 MLL3 still interacts with RAD and shows enhanced activity.

Abstract read, not full text. Entirely in vitro with reconstituted complexes, and a separate entry in this graph notes that reconstituted MLL1 core-complex assembly is strongly temperature- and concentration-dependent with the disassembled state favoured at physiological temperature. In vitro subunit-requirement conclusions may not transfer to the cell.
Evidence that argues against this (1)
- Taken together with the dependency data, this in-vitro result inverts the strike it supports. The finding is that Win-motif/WDR5 disruption severely disrupts MLL1 and SETD1A complexes but only MODESTLY disrupts MLL2/4 and SETD1B without significantly altering their activity. Chen et al. then showed that in MLL-rearranged leukaemia the paralogue that matters is MLL2: endogenous MLL1 was dispensable in independent conditional models, Mll2 deletion alone significantly impaired survival of MLL-AF9-transformed cells, and the two regulate distinct pathways rather than substituting for one another. So the complex least disturbed by blocking the WDR5 interface is the complex the disease depends on - which is an argument about which interface to drug, not evidence that the WRAD subunits do not matter here. AML, not infant KMT2A::AFF1 B-ALL. MRT199665 is a tool compound with no clinical development.
Evidence that argues against this (1)
- The human half of this evidence is a biomarker-stratified cell-line panel, and cell-line panels are less stable instruments than they appear. Ben-David et al. genomically characterised 27 strains of a single widely used line plus multiple strains of 13 further lines, found extensive clonal diversification driven by positive selection under culture conditions, and - testing the 27 strains against 321 anti-cancer compounds - found that at least 75% of compounds that strongly inhibited some strains were completely inactive in others. A result of the form 'MRT199665 kills MEF2C-phosphorylation-positive lines and spares negative ones' therefore needs its strain provenance stated before the stratification is read as biology, and the redefinition of MEF2C rests on it alongside the knock-in mouse. Conditional Meis1 knockout in adult mouse haematopoietic cells significantly reduces haematopoietic stem/progenitor cells through impaired self-renewal and reduced quiescence in a cell-autonomous manner, producing stem-cell exhaustion and defective long-term haematopoiesis, and downregulates a subset of Pbx1-dependent HSC signature genes.
Homeodomain transcription factor Meis1 is a critical regulator of adult bone marrow hematopoiesis · See PubMed record PMID 24498346 for the full author list (not transcribed here) · PLoS One 2014What this evidence cannot show
Abstract read, not full text. Mouse; complete conditional deletion, not partial inhibition. It establishes that the normal adult stem cell needs Meis1, which is the therapeutic-window objection, but it does not measure whether the leukemia needs more of it than the stem cell does — and that quantitative comparison is the one that would settle the demotion.
Evidence that argues against this (1)
- That normal adult stem cells need Meis1 does not by itself close the window, because the window can be opened upstream, and there is a published demonstration of exactly that against a MEIS1-dependent axis. Muntean et al. showed the PAF complex is required for AML proliferation through epigenetic regulation of MEIS1 and Bcl2, and that a dominant-negative MLL fragment disrupting the MLL-PAFc interaction selectively inhibits MLL leukaemic cells, spares E2A-HLF-transformed cells, and leaves normal haematopoietic stem cell function intact in in vivo reconstitution assays. Selectivity came from the interaction, not from the target gene. A MEIS1 demotion argued purely from the normal-HSC requirement is therefore an argument against direct MEIS1 inhibition, not against attacking the MEIS1 axis. Muntean AG et al. 2013

In chronic myeloid leukemia, of five KDMs upregulated relative to normal marrow only KDM6A depletion sensitised cells to imatinib-induced apoptosis, and re-introducing either wild-type or demethylase-dead KDM6A restored resistance, with KDM6A recruited by YY1 to the NTRK1 promoter to upregulate TRKA; the authors state the finding is contrary to KDM6A's reported tumour-suppressor role and independent of its demethylase function.

Evidence that argues against this (1)
- KDM6A's direction of effect is context-dependent, and the context closest to this project's disease runs the other way. Van der Meulen et al. found somatic loss-of-function UTX/KDM6A mutations in human T-ALL, exclusively in male patients, showed UTX escapes X-inactivation in female T-ALL lymphoblasts and normal T cells, demonstrated in vitro and in vivo that UTX is a bona fide tumour suppressor in T-ALL, and reported that UTX-inactivated T-ALL shows collateral sensitivity to pharmacological H3K27me3 inhibition. Abstract read, not full text. Solid tumours, not leukemia, and the dependency is conditional on SMARCA4 inactivation which is not a common lesion in KMT2A-rearranged infant leukemia. Cited only to support the general point that a near-zero mean DepMap gene effect across unselected lines does not exclude a genotype-restricted dependency.
Evidence that argues against this (1)
- The genotype-restricted-dependency argument is symmetric, and its other instance is a warning rather than a licence. This item uses SMARCA4-deficient tumours to argue that a near-zero mean DepMap gene effect does not exclude a restricted KDM6 dependency, which is correct. But the best-characterised genotype restriction for this gene in a leukaemia is UTX INACTIVATION itself: Van der Meulen et al. found recurrent somatic UTX loss-of-function in male T-ALL, demonstrated tumour-suppressor function in vitro and in vivo, and showed those tumours are sensitive to H3K27me3 inhibition. So the same reasoning that says 'a restricted KDM6 dependency may exist' also says a restricted KDM6-loss-driven malignancy exists, and no one has determined which side of that line KMT2A-rearranged infant leukaemia falls on. Van der Meulen J et al. 2015
