Raising HEXIM1 to brake the fusion's output without touching the fusion - one supporting experiment, one chemical series, and no therapeutic window

also called HEXIM1 induction · HMBA analogues · pharmacological P-TEFb sequestration

Instead of attacking the fusion protein, one could try to put more of the brake protein into the cell so the elongation enzyme is held back. It has been tried once in a KMT2A-rearranged leukaemia cell and it slowed growth - but every dividing cell in the body needs the same enzyme, so there is no obvious safe dose.

The same thing, for a scientist

The proposal is to raise HEXIM1 and shift P-TEFb into the inactive 7SK-bound pool, suppressing the fusion's transcriptional output without engaging the fusion protein or the menin interface. **It has one supporting experiment in a KMT2A-rearranged cell:** tetracycline-inducible ectopic FLAG-HEXIM1 in MOLM-13 (KMT2A::MLLT3) markedly attenuated suspension-culture growth without changing CDK9 or cyclin T1 levels, and potentiated JQ1-induced differentiation and apoptosis; conversely HEXIM1 knockdown raised c-Myc and growth and compromised JQ1's in vivo survival benefit [E1]. **The chemical matter is thin.** HMBA is the archetypal HEXIM1 inducer and failed clinically, attributed to a short half-life; an unsymmetrical HMBA analogue series with improved potency was reported in LNCaP prostate cells with no in vivo or selectivity data [E2]. **The pharmacology points the wrong way first.** HMBA, SAHA and JQ1 all acutely *release* P-TEFb from the reservoir; HEXIM1 induction is the feedback that follows [E3][E4]. **And the mechanism cannot be fusion-selective**, because KMT2A::AFF1 cannot enter the reservoir at all - it lacks AFF1 1-346 - so raising HEXIM1 acts only on the shared free P-TEFb pool [E5]. This entry exists to record the idea honestly, including that it is probably not a therapeutic strategy in its present form.

In vivo validation of raising HEXIM1none - the in vivo arm tested knockdown, not overexpression 1
Chemical matterHMBA (failed clinically) and an analogue series in prostate cells 2
Direction of acute drug effectopposite - known inducers release P-TEFb first 34
BufferingHEXIM2 holds bound P-TEFb constant when HEXIM1 changes
Fusion selectivitynone possible - the fusion is not a reservoir client 5
Normal-tissue costP-TEFb is required by every proliferating cell; human reservoir loss-of-function causes developmental disease 6

The honest summary is that this is a real biological lever with one supporting datum, no selectivity argument, and a chemical series that was optimised against the feedback response rather than against the desired pharmacology.

The version of the idea that might survive

Not as a single agent. The MOLM-13 result is a combination result: HEXIM1's level sets how well a BET inhibitor works, and knocking it down measurably blunted JQ1 in vivo 1. That reframes HEXIM1 from a target into a response biomarker and a modifier - the authors say as much, recommending HEXIM1 induction be tracked as a predictive biomarker of BET-antagonist activity.

The testable version for this project is therefore: does HEXIM1 protein level predict menin-inhibitor or BET-inhibitor response in KMT2A-rearranged blasts? That is a correlative study on existing material, not a drug discovery programme, and nobody has done it.

How this could bite us back
  • P-TEFb is required by every proliferating cell. A global elongation brake has no obvious therapeutic window and this entry does not claim one.
  • The fusion cannot be sequestered by the reservoir, so this intervention can never be fusion-selective - only dose-selective, if that.
  • HEXIM2 buffers changes in HEXIM1 to hold bound P-TEFb constant, and it is not inducible by any known agent, so the system is built to absorb this intervention.
  • Every known HEXIM1 inducer acutely releases P-TEFb before HEXIM1 rises. In a disease of excessive elongation, the first hours of dosing plausibly make things worse.
  • Humans with partially dismantled 7SK snRNPs have primordial dwarfism or developmental delay, so the axis is developmentally load-bearing - a serious concern in an infant population.
  • HMBA already failed in the clinic, and the successor chemistry has no in vivo data, no selectivity panel and no leukaemia experiment.

  1. Does HEXIM1 protein level in diagnostic KMT2A-rearranged blasts predict menin-inhibitor or BET-inhibitor response? This is a correlative study on banked material and has not been done.
  2. Does inducing HEXIM1 attenuate growth in a KMT2A::AFF1 B-lineage line, or is the MOLM-13 result myeloid-specific?
  3. Is there a molecule that raises HEXIM1 without first releasing P-TEFb? Every known inducer does the opposite acutely.

Tetracycline-induced ectopic FLAG-HEXIM1 in MOLM-13 (KMT2A::MLLT3) AML cells markedly attenuated suspension-culture growth without changing CDK9 or cyclin T1 levels and potentiated JQ1-induced differentiation and apoptosis, while shRNA knockdown of HEXIM1 raised c-Myc and growth and significantly compromised JQ1's survival benefit in NOD/SCID xenografts.

    What this evidence cannot show

One KMT2A-rearranged line, in vitro for the overexpression arm; the in vivo arm tested knockdown only. Read from PMC full text because the PubMed record carries no abstract. HEXIM1 knockdown alone did not change in vivo growth.

Evidence that argues against this (1)
    • A tetracycline-inducible transgene inside a leukaemia cell is not a drug. Any pharmacological HEXIM1 inducer raises HEXIM1 in every cell it reaches, and the experiment says nothing about that. Devaraj SG et al. 2016
  2. E2moderate confidence 3 of 5: One strong study, or several smaller consistent ones.paper

HMBA analogues with unsymmetrical structures and increased hydrophobicity were developed as more potent HEXIM1 inducers, motivated by HMBA's short half-life having limited its clinical application.

    What this evidence cannot show

A medicinal-chemistry letter, potency measured by HEXIM1 induction in LNCaP prostate cancer cells. No leukaemia data, no in vivo pharmacokinetics, no selectivity panel. A count of activity rows here would measure effort, not tractability.

Evidence that argues against this (1)
    • Optimising for HEXIM1 induction may be optimising for the feedback response to P-TEFb release rather than for sequestration, since that is what the parent compound does acutely. Bartholomeeusen K et al. 2012
  3. E3established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.paper

HMBA, SAHA and JQ1 all cause a rapid, transient release of free P-TEFb from the 7SK snRNP followed by reassembly, with HEXIM1 induction as a downstream consequence, and their effects on the P-TEFb equilibrium are cooperative.

    What this evidence cannot show

HeLa and Jurkat in an HIV-latency framework.

Evidence that argues against this (1)
    • In AML the same class of agent is described as acting through HEXIM1 induction, so the two literatures emphasise opposite halves of one cycle and have not been reconciled in a single cell type. Devaraj SG et al. 2016
  4. E4established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.paper

HDAC inhibitors that reactivate HIV do so by releasing free P-TEFb from the inhibitory 7SK snRNP, and in resting primary CD4+ T cells where P-TEFb levels are very low SAHA has minimal effect until P-TEFb levels are first raised.

    What this evidence cannot show

An HIV-latency model; the readout is viral transcription, not a leukaemia programme.

Evidence that argues against this (1)
    • The dependence on pool size means the effect of any such agent in a KMT2A-rearranged blast cannot be predicted without measuring the pool, which nobody has done. Bartholomeeusen K et al. 2013
  5. E5strong confidence 4 of 5: Replicated in peer-reviewed work by more than one group.database

KMT2A::AFF1 begins at AFF1 residue 347 and therefore lacks the AFF1 cyclin T1-binding sequence (1-308) that is necessary and sufficient for AFF1's incorporation into the 7SK snRNP, so the fusion protein is not a client of the reservoir and raising HEXIM1 can act only on the shared free P-TEFb pool.

    What this evidence cannot show

A sequence-position argument combined with a HeLa deletion series; no direct co-IP of a fusion protein against reservoir components has been published.

Evidence that argues against this (1)
    • Acting on the shared pool is not the same as having no effect: if the fusion's output is disproportionately sensitive to free P-TEFb concentration, a global reduction could still be differentially effective. That is a quantitative claim nobody has tested. Lu H et al. 2014
  6. E6established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.paper

Partially dismantling the 7SK reservoir in humans causes developmental disease: a de novo MEPCE nonsense variant produces global developmental delay and seizures with 7SK snRNP disintegration and enhanced P-TEFb activation, and biallelic LARP7 loss causes Alazami syndrome with facial dysmorphism, severe intellectual disability and primordial dwarfism.

    What this evidence cannot show

Germline, constitutional, lifelong perturbations in small numbers of patients, which is a different exposure from a timed drug in an adult or child with leukaemia. These are loss-of-brake phenotypes; the proposal here is the opposite direction, so they bound the axis rather than the specific intervention.

Evidence that argues against this (1)
    • A separate primary report establishes the LARP7 arm independently, and in both cases the phenotype is growth restriction rather than proliferation, which is the direction one would predict from too much free P-TEFb only if the relationship were non-monotonic. Alazami AM et al. 2012
