Nine survivors in ten carry a long-term complication, and it is worse the younger they were

also called late effects infant ALL · long-term complications infant leukaemia survivors · toxicity burden infant therapy

Of 67 children who survived infant leukaemia for at least five years, 90% had at least one lasting health problem and two thirds had several - problems with learning, weight, teeth, hearing, lungs and hearts. Babies diagnosed before six months, and babies whose leukaemia carried the KMT2A rearrangement, ended up with more of them.

The same thing, for a scientist

The most complete late-effects accounting in infant ALL: 67 survivors of >=5 years from the 119-patient ANZCHOG cohort, median follow-up 11.8 years (range 5.2-27.7) [E1]. At least one long-term complication in 89.6% (60/67); multiple sequelae in 65.7% (44/67), including one survivor with nine. Late effects occurred in 86.2% of KMT2A-germline survivors and 95.2% of transplant recipients. Mean number of late effects: 2.55 with KMT2A rearrangement vs 2.17 germline; 2.78 diagnosed <6 months vs 2.20 >=6 months. By system: neurodevelopmental 38.8%, with deficits in one or more domains in 89.5% of the 19 formally assessed; psychological 32.8%; dental 38.8%; weight problems 43.3%; urinary 14.9%; gastrointestinal 20.9%; ophthalmological 9.0%; respiratory 9.0%; ENT 7.5%; short stature 7.5%; pubertal delay 11.9%; type 2 diabetes 4.5%; hypothyroidism 4.5%; anthracycline cardiomyopathy 6.0% at cumulative doxorubicin-equivalent 150-225 mg/m2, two of whom received heart transplants; one second malignancy (1.5%) [E1]. Only 7 of 67 received any radiation. The authors state the neurodevelopmental figure is likely an underestimate because only 28.4% had formal assessment, and the cardiac figure likewise because only clinically significant cardiomyopathy was captured.

The two most consequential lines

Cardiomyopathy at 150-225 mg/m2. Four survivors developed clinically significant anthracycline cardiomyopathy at cumulative doses that are modest by paediatric oncology standards, and two of the four needed heart transplants. The authors conclude this warrants discussion of standardising dexrazoxane for infants exposed to anthracyclines 1. Infant hearts are not small adult hearts, and the graph's risk-anthracycline-cardiotoxicity-infant-leukemia entry should be read against this.

KMT2A-germline survivors carry an 86.2% late-effect rate. These are the infants with the best prognosis (5-year EFS 73%) receiving the same intensive backbone. The authors' own conclusion is that they are the ideal candidates for de-escalation in a trial 1. That is a therapeutic recommendation arising directly from a toxicity measurement, and it is the clearest actionable item this beat found.

What blinatumomab changes, and what it does not

Adding one post-induction course of blinatumomab to Interfant-06 chemotherapy in 30 infants gave, at median 4.2 years follow-up, 4-year disease-free survival of 83.3% against 44.0% in a selected Interfant-06 historical cohort, and 4-year overall survival 93.3% against 60.2% 2. No infection-related fatality occurred post-induction against 4% in Interfant-06 2. Intravenous immunoglobulin was given to 63% and G-CSF to 17%, and immunoglobulin G falls after blinatumomab and recovers nearly completely before the end of treatment 23. Pharmacokinetics were comparable across paediatric age groups (mean steady-state concentration 706 +/- 194 pg/mL/d, median clearance 0.89 L/h/m2, range 0.57-2.66) 2.

So immunotherapy is reducing the acute toxicity burden. It does not touch the anthracycline, the alkylator, the steroid or the intrathecal therapy that produce the late effects tabulated above, because it is added to them rather than substituted for them. A late-effects profile measured on 1992-2011 therapy will not improve until something is removed.

How this could bite us back
  • Immunotherapy added on top of the existing backbone improves survival while leaving the entire late-effects burden in place, and better survival means more survivors carrying it.
  • Anthracycline cardiomyopathy occurred at cumulative doses considered modest, so dose-capping strategies calibrated on older children do not protect infants.
  • De-escalating therapy for KMT2A-germline infants trades a measured 86.2% late-effect rate against an unmeasured relapse risk, and at roughly 38 such infants per 20 years per region the trial to settle it would take decades.
  • A menin inhibitor or BH3 mimetic that permits chemotherapy reduction is the only route that touches this burden, which makes late-effect reduction, not response rate, the endpoint that matters for targeted agents in infants.
  • Blinatumomab-induced hypogammaglobulinaemia is managed with IVIg in 63% of infants, so the apparent absence of post-induction infectious death is partly purchased with a blood product and its own supply and reaction risks.
Still open
  1. Should dexrazoxane be standard for every anthracycline-exposed infant? Four clinically significant cardiomyopathies at 150-225 mg/m2 in 67 survivors is a signal, and no infant trial has randomised it.
  2. Can therapy be de-escalated for KMT2A-germline infants, who have 73% 5-year EFS and an 86.2% late-effect rate? The authors propose it; nobody has run it.
  3. What is the true neurodevelopmental burden when every survivor is formally assessed, rather than the 28.4% who happened to be?
  4. Does adding blinatumomab allow any chemotherapy component to be removed, and which one would recover the most late-effect burden per unit of efficacy risk?

    What this evidence cannot show

Retrospective ascertainment of late effects from records, not prospective surveillance, so under-capture is near certain and the authors say so for both neurodevelopment (only 28.4% formally assessed) and cardiac function (subclinical dysfunction not sought). Survivors only - 67 of 119, so the denominator excludes everyone who died, and any toxicity that contributed to death is absent. 20 years of changing protocols. Read in the open-access full text at PMC10522653.

Evidence that argues against this (1)
    • A previous study found no significant long-term complications in KMT2A-germline survivors, which this cohort contradicts; the authors attribute the discrepancy to that study's short follow-up, but the disagreement is unresolved and the direction of ascertainment bias differs between retrospective record review and prospective assessment. With median follow-up 4.2 years, 30 infants with KMT2A-rearranged ALL given Interfant-06 chemotherapy plus one post-induction blinatumomab course had 4-year disease-free survival of 83.3% versus 44.0% and 4-year overall survival of 93.3% versus 60.2% in historical controls, with no post-induction infection-related fatality against 4% in Interfant-06, IVIg in 63% and G-CSF in 17%, and blinatumomab pharmacokinetics comparable across paediatric age groups.

    What this evidence cannot show

30 infants, single-arm, compared with a selected historical Interfant-06 cohort rather than a randomised control; historical comparison inherits era effects in supportive care, which is precisely the variable this beat shows dominates infant outcomes. The absence of post-induction infection-related fatality is a zero-event observation in 30 patients and its upper confidence bound is about 12%. Abstract read; full text paywalled.

Evidence that argues against this (1)
    • The comparison is against a historical control from the same consortium in an earlier era, and Interfant-06 itself showed a 10-12 point outcome gap between regions on the same protocol, so a selected historical control can account for a substantial part of a difference of this kind. Blinatumomab reduces immunoglobulin G in infants with KMT2A-rearranged ALL, with near-complete recovery to normal (>4 g/L) before the end of treatment, across 169 IgG measurements in 30 infants.

    What this evidence cannot show

30 infants and 169 measurements taken as clinically indicated rather than on a protocol schedule, so the recovery trajectory is estimated from a non-random sampling pattern. IVIg supplementation was given concurrently in 63% of the cohort, which directly raises measured IgG and confounds any statement about endogenous recovery. Abstract read; full text paywalled.

Evidence that argues against this (1)
    • Because 63% received IVIg, an apparent return to normal IgG may reflect supplementation rather than recovered B-cell function; the paper's own recommendation that future trials monitor immunoglobulin recovery concedes the question is open. Vieira Martins M et al. 2026
  4. E4strong confidence 4 of 5: Replicated in peer-reviewed work by more than one group.review

Infant ALL survivors' long-term complications are described as occurring across nearly all organ systems, with current knowledge of quality of life in survivors extremely limited, which is the stated argument for reducing chemotherapy intensity in favour of targeted and immune therapies.

A review's framing rather than a measurement. Read in the open-access full text at PMC12399962.

Evidence that argues against this (1)
    • Current immunotherapy is added to the chemotherapy backbone rather than replacing any of it, so the late-effects burden measured on historical therapy is unchanged by it until an agent is removed; no infant trial has yet removed one. Vieira Martins M et al. 2026
