Two clones, two antibodies: a four-month-old with refractory KMT2A::AFF1 mixed-phenotype leukaemia treated with blinatumomab and gemtuzumab together
also called infant MPAL blinatumomab gemtuzumab case · KMT2A-AFF1 MPAL dual immunotherapy case
A four-month-old girl arrived exhausted, pale, bruising and struggling to breathe, with a white cell count of 762,000 and blood that would not clot properly; her leukaemia turned out to be two different diseases in one child, a B-cell one and a myeloid one. Chemotherapy failed three times, so her doctors gave two antibody drugs together, each aimed at one of the two clones, which worked well enough for a transplant; she relapsed eleven months later, was rescued again with CAR-T cells, and was alive and in remission at three years old.
The same thing, for a scientist
A 4-month-old girl presented with asthenia, pallor, cutaneous haemorrhages, hepatosplenomegaly and upper-airway obstructive respiratory signs; maximum WBC was 762 x 10^9/L with spontaneous tumour lysis and grade 3 disseminated intravascular coagulation. Marrow showed 80% lymphoblasts and 9% peroxidase-negative myeloblast-like cells with dysgranulopoiesis. Flow cytometry showed two distinct CD34+CD135+ populations: a pro-B clone (70%, CD19bright, CD22dim partial, cCD79a partial, CD20-, CD24dim, no surface light chain or cytoplasmic mu, with myeloid antigens CD11c/CD64/CD15/CD33 on 30% of CD19+ blasts) and a monocytic clone (30%, CD19-, cCD79a-, MPO-, bright CD33/CD11b/CD65/CD15/CD11c/CD64 with partial CD14), meeting WHO 2016 criteria for B/myeloid MPAL. FISH showed KMT2A rearrangement in 93% of nuclei and RT-PCR confirmed KMT2A::AFF1 (MLL-AF4); a FLT3 missense mutation was present at 43% allelic frequency. CSF status was CNS3. On the Interfant-06 high-risk arm she was a poor prednisone responder (day 8 blasts 127.9 x 10^9/L), failed to reach remission at the end of induction, progressed during consolidation, and failed high-dose cytarabine with amsacrine, with both clones persisting. Sequential blinatumomab then gemtuzumab ozogamicin achieved CR1 at day 38 with MRD undetectable by flow and by genomic PCR on the KMT2A breakpoint. She was transplanted at 9 months of age after busulfan, fludarabine and thiotepa, relapsed 11 months later at 21 months of age with sinus and orbital disease, CNS2 CSF and 54% marrow blasts, the relapse clone retaining the pro-B phenotype with six-fold higher FLT3 (CD135) expression and a newly acquired monoallelic IKZF1 deletion. Salvage chemotherapy with midostaurin achieved CR2, and autologous CD19 CAR T cells followed. At 36 months of age she was in persistent MRD-negative CR2 with no known sequelae. [E1]
How it began
The report gives five presenting features and they are, unusually, all the textbook ones at once: asthenia, pallor, cutaneous haemorrhages, hepatosplenomegaly, and obstructive respiratory signs of the upper airway 1.
- The asthenia and pallor are
concept-pallor-and-fatigue-anaemia- a four-month-old who is tired and pale, which is exactly what a parent notices and exactly what is easiest to attribute to something else. - The cutaneous haemorrhages are
concept-bruising-petechiae-thrombocytopenia, and here they sat on top of grade 3 disseminated intravascular coagulation. - The hepatosplenomegaly is
concept-hepatosplenomegaly-infant-leukemia. - The white cell count reached 762 x 10^9/L with spontaneous tumour lysis syndrome, without renal failure -
concept-hyperleukocytosis-emergencyarriving before any treatment had been given.
A non-SARS coronavirus, HCoV-NL63, was found on the nasopharyngeal swab. The report does not connect it to anything; it is recorded here because it is the kind of incidental positive that can explain away a sick infant for another day.
What was found: one child, two diseases
Marrow was hypercellular with 80% lymphoblasts and 9% cells resembling myeloblasts with fine granulations but peroxidase-negative, plus atypical dysgranulopoiesis 1.
Flow cytometry resolved two distinct populations, both CD34+ and CD135+ (FLT3):
| Pro-B clone (70% of blasts) | Monocytic clone (30%) | |
|---|---|---|
| B markers | CD19 bright, CD22 dim partial, cCD79a partial, CD24 dim; CD20 negative; no surface kappa/lambda, no cytoplasmic mu | CD19 negative, cCD79a negative |
| Myeloid markers | CD11c, CD64, CD15, CD33 on 30% of the CD19+ blasts | CD33, CD11b, CD65, CD15, CD11c, CD64 bright; CD14 partial (15%); MPO negative |
| Lymphoid crossover | - | partial CD22 and CD24 |
The authors note the crossover explicitly: the two populations form a continuum of expression rather than two clean compartments. This is the lineage-ambiguity that concept-flow-cytometry-immunophenotype-kmt2a-r describes as an intrinsic property of KMT2A-rearranged disease, seen at its most extreme.
FISH found KMT2A rearrangement in 93% of nuclei (concept-fish-split-signal-kmt2a) and RT-PCR confirmed KMT2A::AFF1 (MLL-AF4) (concept-molecular-confirmation-fusion-transcript). A FLT3 missense mutation was present at 43% allelic frequency. CSF status was CNS3 - overt CNS disease at diagnosis (concept-cns-involvement-at-diagnosis).
What was done and what happened
Chemotherapy failed, three times. On the Interfant-06 high-risk arm she was a poor prednisone responder with 127.9 x 10^9/L blasts at day 8. The CSF normalised after three intrathecal injections, but remission was not achieved: an M2 marrow (9% blasts) at the end of induction, progression to M3 (32% blasts) fifteen days into the IB consolidation, then failure of high-dose cytarabine with amsacrine, with flow showing both clones still present (80% B-lymphoid, 20% myeloid) 1.
Then two antibodies, in a deliberate order. After parental consent and national regulator authorisation, the team attacked each clone separately. Blinatumomab (CD19/CD3) went first, at 5 then 15 micrograms/m2/day, precisely so that T cells could expand before gemtuzumab ozogamicin - a CD33-directed calicheamicin conjugate that is cytotoxic to those same T cells - was added from day 11, at 3 mg/m2 on days 11, 15 and 18, with triple intrathecal therapy every 15 days.
Toxicity: generalised seizures at day 25 in a febrile context lasting 5 minutes, with all radiological, biological and microbiological investigation negative; blinatumomab was stopped, restarted at day 30 and continued to day 73. The only other grade 3/4 toxicity was an undocumented febrile neutropenia. No sinusoidal obstruction syndrome and no liver toxicity, which is the feared combination when gemtuzumab precedes a busulfan-containing transplant.
It worked. CR1 at day 38 of blinatumomab/gemtuzumab, with MRD undetectable by flow and by genomic PCR built on the KMT2A breakpoint. Transplant at 9 months of age after busulfan, fludarabine and thiotepa with defibrotide prophylaxis; full donor chimerism at months 1 and 2; ciclosporin stopped at month 3; MRD negative again at month 6 (concept-hsct-in-kmt2a-r-leukemia).
Then it relapsed. Eleven months after transplant, at 21 months of age, with invasion of the sinuses and orbits, CNS2 CSF and 54% marrow blasts. The relapse clone was pro-B with the original phenotype (CD19 bright, CD10-, partial CD33, no monocytic antigens) - the myeloid clone did not come back. KMT2A rearrangement and the FLT3 mutation persisted; a monoallelic complete IKZF1 deletion had been acquired; and FLT3 (CD135) was expressed six times more strongly than at diagnosis.
Salvage was intensive chemotherapy with twice-daily midostaurin - chosen on the FLT3 finding - plus triple intrathecal therapy, achieving marrow and extramedullary CR2 with undetectable breakpoint MRD, followed three months later by autologous CD19 CAR T cells with grade 2 cytokine release syndrome and haematological and neurological toxicity.
Outcome as reported: 36 months old, in persistent MRD-negative CR2 twelve months after the CAR-T infusion, with no known sequelae.
Why this case is in the graph
This is the graph's only entry in which a named therapeutic strategy is chosen from first principles at the bedside and then tested on one child, and the reasoning is written down.
- Lineage plasticity is not an abstraction. Two CD34+CD135+ populations, a continuum between them, and a relapse in which only one of the two came back.
risk-lineage-switch-cd19-directed-therapysays that CD19-directed pressure can select a myeloid escape; here the pre-existing myeloid clone was co-targeted on purpose, and the relapse was lymphoid. - The sequencing of two immunotherapies was a mechanistic argument. Blinatumomab needs functional effector T cells; gemtuzumab kills them. Giving blinatumomab an 11-day head start is a hypothesis about drug interaction, executed in a patient, with the reasoning stated.
- A second lesion changed the second-line plan. FLT3 mutant at 43% at diagnosis, CD135 six-fold brighter at relapse, midostaurin at salvage. The fusion is the driver; it was not the only thing that mattered.
- Survival here is a sequence, not an event. Failed induction, failed consolidation, failed high-dose cytarabine, two antibodies, a transplant, a relapse, midostaurin, CAR-T. A child alive at 36 months in CR2 has been through eight lines of treatment before her third birthday.
It is also the closest thing in this arm to a long-term survivor of refractory KMT2A::AFF1 infant leukaemia - with the honest qualification that 36 months of age, 12 months after CAR-T, is not the same as cure.
An error in the published abstract, recorded here
The abstract of this paper describes the patient as "a 4-year-old infant". The full text says "Four-month-old little girl" and reports transplant at 9 months of age and relapse at 21 months of age, so the abstract's "4-year-old" cannot be right; it is a typographical error for four months. Anyone extracting ages from abstracts - including automated pipelines - will get this one wrong. This entry uses the full text 1.

}- Targeting two antigens does not remove the escape route, it narrows it. The relapse here retained the original lymphoid phenotype with brighter FLT3, so the surviving clone was the one both antibodies were aimed at, changed quantitatively rather than by lineage.
- Gemtuzumab ozogamicin before a busulfan-containing transplant carries a real risk of sinusoidal obstruction syndrome; it did not occur in this child, which is a single favourable observation and not reassurance.
- Every line here works by recruiting or replacing the immune system - bispecific antibody, allograft, CAR T cells. In an infant whose immune system is itself four months old, that is a strategy built on a substrate whose capacity nobody has characterised.
- A menin inhibitor was not available to this child. When it is, it will be entering a sequence this long, in patients this heavily pretreated, and its single-agent response rates were measured in exactly that population.
- Blinatumomab was given an 11-day head start so that T cells could expand before a T-cell-toxic conjugate arrived. Is that interval right, and is there any pharmacodynamic readout (T-cell count, CD69 induction) that should set it rather than a round number?
- The relapse clone was pro-B with six-fold higher FLT3 and a new IKZF1 deletion, and the myeloid clone did not return. Was the myeloid clone eradicated by gemtuzumab, or outcompeted? Single-cell sequencing of banked diagnostic and relapse material would separate these.
- Does co-targeting a pre-existing myeloid clone actually reduce the rate of CD19-directed lineage escape in KMT2A-rearranged infants, or does it merely change which clone relapses? This is testable in the Interfant and iBFM cohorts and has not been.
- A FLT3 mutation at 43% allelic frequency in a KMT2A::AFF1 infant - how often is FLT3 co-mutated at diagnosis, and should FLT3 inhibition be part of first-line rather than salvage in that subgroup?
- E1moderate confidence 3 of 5: One strong study, or several smaller consistent ones.paper

Full text read (PubMed Central, PMC7953899), not abstract only; gene symbols are stripped from the PMC plain-text rendering, so the fusion partner (KMT2A::AFF1), the mutated gene (FLT3) and the deleted gene (IKZF1) were recovered by re-reading the same article's rendered PMC page directly, and each is quoted verbatim there. A single case report: it demonstrates feasibility and tolerance in one child and establishes no response rate, and publication bias is explicit - the authors present it as a strategy worth testing prospectively, not as evidence that it works. Follow-up ends at 36 months of age, 12 months after CAR-T, which is remission and not cure. The published abstract states '4-year-old infant', which contradicts the full text's 'four-month-old' and the reported transplant at 9 months of age; this entry follows the full text. The report does not state the KMT2A breakpoint intron, although a genomic breakpoint PCR assay was built, so the fusion is recorded at gene level only. This entry omits the treating institution and the month and year of presentation, as identifying detail that adds nothing clinically.
Evidence that argues against this (2)
- Against reading dual immunotherapy as protective against lineage escape: blinatumomab alone has been reported to drive a KMT2A-rearranged infant ALL into an M5 AML phenotype within 11 days, and that case ended in death from extramedullary disease - so CD19-directed pressure in this genotype has a documented failure mode that this child's regimen was designed around but which no single case can show it prevents. source 2018
- Against generalising the outcome: the same report states that in an international cooperative study of 233 children with ambiguous-lineage acute leukaemia, the KMT2A-rearranged cases aggregated in the hybrid-treatment group with a 5-year event-free survival of only 28%, and that Interfant-06 gave the KMT2A-rearranged group a 6-year EFS of about 36%. source 2021
