The ENL YEATS acetyl-reader inside KMT2A::MLLT1: what it reads, what it does, and why both control arms carry it

also called MLL-ENL YEATS domain function · ENL YEATS H3K27ac reader in the fusion · YEATS domain MLL-ENL leukemogenesis

The partner half of this fusion protein carries a small module that grips histones tagged with an acetyl mark, and it is the module that lets the fusion sit on the genes it switches on. Deleting it stops the fusion causing leukaemia in mice outright, and breaking only its grip on the acetyl mark cuts the number of leukaemia stem cells about three-fold.

The same thing, for a scientist

The ENL YEATS domain (MLLT1 residues ~1-138) is an acyl-lysine reader that binds histone H3 acetylated at K9, K18 and K27. In the KMT2A::MLLT1 chimera it is retained by 84.1% of t(11;19) patients, and the paper that established that number also dissected what it does inside the fusion. Deleting ENL 5-371 (MLL-ENL-deltaYEATS) abolishes leukaemogenesis in vivo despite preserving serial replating and the AHD; the reader-dead point mutants F59A and Y78A, which abolish H3K27ac binding (BLI K_d 66 uM for wild-type YEATS on H3K27ac peptide), instead extend latency, are transplantable into secondary recipients, and reduce leukaemic stem-cell frequency from 1/108 (95% CI 1/62-1/190) to 1/355 (95% CI 1/198-1/637) by extreme limiting dilution. The two lesions separate by mechanism: deletion loses chromatin occupancy (reduced FLAG-ChIP at Eya1 and Meis1), whereas the point mutants retain occupancy and lose downstream epigenetic output (H3K9ac, H3K79me2 and H3K4me3 fall in proportion to transcript). All YEATS mutants retain PAF1 co-IP, so the H3Kac and PAF1 surfaces of the same domain are separable in that assay. The point that matters for this beat: the reciprocal-negative (n=153) and reciprocal-positive (n=101) arms both break at or upstream of MLLT1 exon 2 and both retain the full YEATS domain, so the reader is constant across the comparison; the YEATS-deficient 15.9% form a third, separate group.

What the domain reads

The ENL YEATS domain binds histone H3 acetylated at K9, K18 and K27, with crystallographic and ChIP evidence that endogenous ENL co-localises with H3K27ac and H3K9ac at promoters of actively transcribed leukaemia-essential genes 3. In the fusion context the affinity was measured directly by biolayer interferometry against an immobilised biotinylated H3K27ac peptide: K_d = 66 uM for the wild-type domain, with F59A and Y78A abolishing the interaction 1. A fluorescein-labelled derivative of the chemical probe SGC-iMLLT binds wild-type ENL YEATS at K_d 0.26 uM and loses affinity against every mutant in the order delta21-26 (4.6 uM) > Y78A (8.82 uM) > F59A (>8 uM) > triple (>8 uM), so the probe and the histone occupy the same groove 1.

Note that this is a micromolar interaction with a single modified peptide. Avidity in a nucleosomal and multivalent context is not measured by it, and the number should not be read as the affinity of the fusion protein for chromatin.

Deletion versus reader-death: two different phenotypes

Mice receiving MLL-ENL-transduced marrow died at a median of 66 days; mice receiving MLL-ENL-deltaYEATS (ENL 5-371 removed, AHD retained) failed to develop leukaemia at all, even though the construct still supported third-round serial replating ex vivo 1. The reader-dead point mutants did not phenocopy that: they extended latency, and the extension was transplantable into secondary recipients 1.

Extreme limiting dilution analysis on primary leukaemic cells put the difference on a stem-cell axis: 1/108 cells (95% CI 1/62-1/190) for MLL-ENL against 1/355 (95% CI 1/198-1/637) for the F59A/Y78A leukaemias, with no difference in bone-marrow homing of CD45.2 donor cells 1.

The mechanistic split is the useful part. FLAG-ChIP showed reduced binding of MLL-ENL-deltaYEATS at the Eya1 and Meis1 loci, and PAF1 occupancy mirrored the fusion, i.e. deletion costs occupancy. The point mutants held occupancy and lost output: H3K9ac, H3K79me2 and H3K4me3 fell in proportion to Eya1/Meis1 transcript 1. A Hoxa9-promoter luciferase reporter was unchanged by any of the mutants, so the defect is not a general loss of transactivation capacity 1.

The PAF1 surface is separable from the acetyl-reading surface - but two laboratories disagree about the reagent

ENL reaches PAF1 through the same YEATS fold. In the fusion, all four YEATS constructs tested (F59A, Y78A, delta21-26, triple) still co-immunoprecipitated endogenous and exogenous PAF1, which the authors read as non-overlapping, non-exclusive binding sites 1.

That sits against the prior study from which the delta21-26 deletion was taken, which reported H3 and PAF1 competing for ENL, showed that PAF1 recruitment determines the transforming potential of MLL-ENL, and proposed that the KMT2A moiety supplies a constitutive PAF1 tether letting the fusion escape the H3 competition 4. The two accounts are not reconcilable as stated, and the 2023 paper says so explicitly: its BLI finding that delta21-26 also disrupts H3K27ac binding contrasts with the earlier report, which used a different biochemical assay 1.

A 2026 chemical-genetic screen adds a third constraint: in-frame deletions of PAF1 confer resistance to ENL/AF9 YEATS inhibitors, and in both sensitive and resistant isogenic lines the inhibitors downregulate ENL target genes equally - so target-gene suppression is not sufficient to explain the anti-leukaemic effect 5. Any model in which YEATS inhibition works purely through the Eya1/Meis1 axis is under-determined.

What this means for the 153-versus-101 control arm

Both arms of the clean control comparison - 153 patients breaking 5' of MLLT1 but 3' of ACER1, and 101 breaking inside MLLT1 intron 1 - deliver a partner contribution beginning at MLLT1 exon 2, and therefore both carry the complete YEATS domain 2. That is the reason the comparison is worth running: the reader module, which this entry shows is the single largest known determinant of MLL-ENL leukaemic stem-cell frequency, is held constant while reciprocal-allele status varies.

The corollary is a warning. The 15.9% of t(11;19) patients whose break falls downstream of MLLT1 exon 2 (partial or absent YEATS) are a third group, not a subset of either arm, and pooling them into either arm would mix the beat's variable of interest with the strongest known modifier of the phenotype 2. KOPN-8, the cell line most of this fusion's pharmacology runs in, sits in that third group.

An internal inconsistency in the source, recorded rather than smoothed over

The paper reports the non-retaining 15.9% two ways that do not agree. The results text says 'partial inclusion (6.0%) or exclusion of the YEATS domain (9.9%)'; the Figure 1C legend says partial n=22/302 (7.3%) and absent n=26/302 (8.6%) 2. Both pairs sum to 15.9% and the headline 84.1% (n=254/302) is unaffected, but the split between partial and absent differs by about four patients each way depending on which sentence is read. Anyone building a three-way stratification on this paper should use the figure-legend counts, which carry explicit denominators.

How this could bite us back
  • A YEATS-domain inhibitor is not a fusion-specific agent. It occupies the same groove in wild-type ENL and in wild-type AF9, and wild-type ENL is required for haematopoietic stem-cell self-renewal, so on-target marrow toxicity is the expected dose-limiting effect rather than an off-target surprise.
  • Roughly one in six t(11;19) patients makes a fusion with a partial or absent YEATS domain and would be structurally unable to respond to a YEATS antagonist aimed at the fusion, while still having wild-type ENL for the drug to hit. A trial that did not genotype the breakpoint would dilute its own effect and could not tell the two failure modes apart.
  • Reader-dead point mutants keep chromatin occupancy and still make leukaemia, only more slowly. A drug that only blocks acetyl-reading therefore predicts delay rather than eradication, which in a relapsed setting is the profile that produces a response without a durable remission.
  • In-frame PAF1 deletions confer resistance to this inhibitor class while leaving target-gene suppression intact, so a pharmacodynamic readout built on ENL target genes would report on-target engagement in a resistant patient.

  1. Does the YEATS domain matter equally in the reciprocal-negative and reciprocal-positive arms? Every YEATS experiment to date uses a retroviral single-allele system, which is reciprocal-negative by construction, so the entire YEATS literature is an experiment on one arm only.
  2. Is the leukaemic stem-cell frequency difference (1/108 versus 1/355) reproduced in a lymphoid model? Every in vivo MLL-ENL YEATS experiment here is myeloid, while the large majority of human KMT2A::MLLT1 is lymphoblastic.
  3. Which account of the PAF1 surface is right, and does it matter clinically? If PAF1 and H3Kac genuinely compete, a YEATS inhibitor would displace histone and could increase PAF1 occupancy rather than decrease it.
  4. Does the 15.9% YEATS-deficient group differ clinically from the 84.1%? Nobody has linked the Frankfurt breakpoint partition to outcome.

In murine MLL-ENL leukaemia, deleting the ENL YEATS domain abolishes leukaemogenesis in vivo while reader-dead point mutants (F59A, Y78A) extend latency and reduce leukaemic stem-cell frequency from 1/108 to 1/355; wild-type ENL YEATS binds H3K27ac peptide with K_d 66 uM, and all YEATS mutants retain PAF1 co-immunoprecipitation.

    What this evidence cannot show

Retroviral overexpression of MLL-ENL in murine marrow progenitors, not an endogenous human t(11;19) system. Limiting-dilution confidence intervals nearly touch at their extremes (1/190 versus 1/198), so the stem-cell difference is real but modest. The H3K27ac K_d of 66 uM is a single-peptide measurement and says nothing about nucleosomal avidity. Full text read from PMC11246743 via efetch db=pmc after fullTextXML returned HTTP 500.

Evidence that argues against this (1)
    • The same domain's PAF1 contact was reported by a different laboratory as directly competitive with histone H3, with PAF1 recruitment determining MLL-ENL transforming potential - which is hard to square with all four YEATS mutants retaining PAF1 binding here. The 2023 authors acknowledge the conflict and attribute it to different biochemical assays. Hetzner K et al. 2018
  2. E2strong confidence 4 of 5: Replicated in peer-reviewed work by more than one group.paper

84.1% (n=254/302) of t(11;19) patients retain the full ENL YEATS domain in the resulting fusion; the non-retaining 15.9% break downstream of MLLT1 exon 2 and are reported as partial n=22/302 (7.3%) and absent n=26/302 (8.6%) in the figure legend but as 6.0% and 9.9% in the results text.

    What this evidence cannot show

The 302-patient breakpoint set is a re-analysis of the Frankfurt diagnostic archive, a referral series and not population-based; breakpoints are inferred from genomic position rather than from sequenced fusion protein. The internal 6.0%/9.9% versus 7.3%/8.6% discrepancy is unexplained in the paper. Whether a break upstream of exon 2 always yields an in-frame YEATS-containing protein is an inference from splicing, not a measurement.

Evidence that argues against this (1)
    • The same archive's periodic recombinome publications report t(11;19) breakpoint distributions at coarser resolution and do not independently confirm the exon-2 partition used here, so the three-way split rests on a single analysis of a single archive. Meyer C et al. 2023
  3. E3established confidence 5 of 5: Multiple independent lines of evidence; treated as settled in the field.paper

ENL, but not its paralogue AF9, is required for AML maintenance; ENL binds acetylated histone H3 and co-localises with H3K27ac and H3K9ac at promoters of actively transcribed leukaemia-essential genes, and structure-based disruption of the YEATS-acetyl interaction reduces RNA polymerase II recruitment to ENL targets.

    What this evidence cannot show

This work is about wild-type ENL in AML cell lines, not about the fusion protein. Its ENL-versus-AF9 asymmetry is a statement about the wild-type paralogues and does not transfer directly to the fusion, where the partner moiety is tethered by KMT2A rather than by its own recruitment.

Evidence that argues against this (1)
    • The companion paper published alongside it reached ENL by an unbiased CRISPR screen in an MLL-AF4 line and framed the dependency as a general leukaemia dependency on transcription control rather than an acetyl-reading one, although it also found the YEATS domain necessary for ENL-dependent growth. Erb MA et al. 2017
  4. E4strong confidence 4 of 5: Replicated in peer-reviewed work by more than one group.paper

ENL overcomes polycomb silencing by recruiting PAF1 through the YEATS domain; H3 and PAF1 compete for that surface, PAF1 determines the transforming potential of MLL-ENL, and the KMT2A moiety supplies a constitutive PAF1 tether that lets the fusion bypass the H3 competition.

    What this evidence cannot show

Murine primary-cell transformation and in vitro binding; the competition model is inferred from binding assays rather than from a structure of the ternary state. Read from abstract only in this session - the full text was not retrieved, so the claims here are limited to what the abstract asserts.

Evidence that argues against this (1)
    • In the 2023 fusion-focused study every YEATS mutant, including the delta21-26 deletion taken from this paper, retained PAF1 co-IP, and the same deletion lost H3K27ac binding by BLI - the opposite of a clean separation of the two surfaces. Hu H et al. 2023
  5. E5moderate confidence 3 of 5: One strong study, or several smaller consistent ones.paper

In-frame deletions of PAF1 confer resistance to ENL/AF9 YEATS-domain inhibitors, and in isogenic sensitive and resistant leukaemia lines the inhibitors downregulate ENL target genes equally, so suppression of ENL target genes is not sufficient to explain the anti-leukaemic effect of YEATS antagonists.

    What this evidence cannot show

Read from abstract only. The models are not specified as t(11;19) in the abstract, so whether this applies to a YEATS-retaining KMT2A::MLLT1 background is unestablished. A CRISPR modifier screen identifies sufficiency for resistance, not the mechanism of the parent drug.

Evidence that argues against this (1)
    • The original chemical-probe paper for this inhibitor class reported anti-leukaemic activity that did track with displacement of ENL from chromatin and downregulation of its targets, which is the model this screen argues is incomplete. Garnar-Wortzel L et al. 2021
