Does the fusion 'lose restraint'? The pattern of KMT2A-side loss, and why the tidy reading does not survive contact with the ledger
also called loss of restraint hypothesis · KMT2A subtraction pattern · targeting retained, restraint lost
There is an attractive story that the leukemic protein keeps KMT2A's ability to find genes and throws away everything that would make it stop. The tidy version is partly true, but most of the discarded parts have never been tested, and the best-evidenced loss is not a brake at all.
The same thing, for a scientist
The 'loss of restraint' reading says the fusion systematically retains targeting (AT-hooks, CXXC, menin/LEDGF docking) and discards regulation, producing a protein that can find its targets and cannot turn itself off. Judged against the ledger (`ana-kmt2a-lost-module-ledger`), the reading is **half-defensible, and its usual telling is wrong about which regulation is lost**. **Revised cycle 31.** Point (1) of the previous version - that the best-evidenced KMT2A-side loss is the MOF/KAT8 contact, an activating output rather than a brake - is **withdrawn**: MOF is required for MLL-AF9 leukemia but not through the fusion's target programme, and equally required in NUP98-fusion AML, so it is a dependency of the cell and not a demonstrated cost of the fusion's missing contact [E5]. What replaces it strengthens the restraint reading in one respect and redirects it in another: the KMT2A-side losses that DO have measured consequences are three independent **degradation determinants** - the PHD2 E3 ligase, the taspase1 cleavage sites and the FYRN degron - and all three, when disabled in wild-type MLL1, make the protein more abundant and more chromatin-bound [E6]. So the fusion does discard brakes; they are brakes on **abundance**, not on activity. Two objections to the strong form survive: the PHD function the story treats as removed is on current evidence **outsourced**, and the wild-type allele is genetically required for MLL-AF9 leukemogenesis [E7]; and removing restraint is demonstrably **not sufficient**, since a truncated MLL lacking everything C-terminal to the breakpoint does not immortalise unless dimerised or fused to an activator. The defensible residue is therefore: the fusion is **exempt from** regulation that still operates on its wild-type counterpart at the same loci, and the regulation it most clearly escapes is its own disposal.
Stating the hypothesis so it can be attacked
The strong form: KMT2A fusions transform because the break separates a targeting module from a regulatory module, and it is the loss of regulation that drives the disease.
What is genuinely in its favour:
- The break position is remarkably consistent, and it falls between the targeting apparatus and everything downstream. That is not obviously a coincidence.
- At least two independent brakes sit in the lost region and nowhere else: PHD2's CDC34-dependent E3 ligase activity, which limits MLL1 abundance and target occupancy [see
subtraction-kmt2a-phd2-e3-ligase], and the PHD3/CyP33 activator-to-repressor switch. Two independent restraints lost in one cut is a real pattern, not a rhetorical one. - Re-inserting PHD3 into MLL-ENL blocks immortalisation - the only in-cis add-back result that exists, and it points the hypothesis's way.
- The 1994 founding observation was exactly this shape: activation domain lost, repression domain and DNA-binding retained [see
subtraction-kmt2a-tad-cbp-contact].
Three findings that break the strong form1 deeper
1. The best-evidenced loss is an activating output, not a brake. Mll1 deletion selectively depletes H4K16 acetylation at MLL1 target genes with reduced transcription, and that H4K16ac depends on recruited MOF - which binds the MLL1 C-terminus the fusion discards. The single loss with the most functional evidence behind it therefore makes the protein less able to activate, which is the opposite of the restraint story's direction.
2. The restraint may be outsourced rather than removed. MLL1 fusion proteins lack the PHD fingers and require prebinding of a wild-type MLL1 complex for stable HoxA9 association 1. If a PHD-containing wild-type complex is sitting at the locus the fusion occupies, the brake has not left the building. What the fusion has acquired is exemption, not absence.
3. Loss of restraint is not sufficient. Induced dimerisation of a truncated MLL immortalises bone marrow and imposes a reversible myeloid differentiation block with Hoxa7, Hoxa9 and Meis1 upregulation; dimerised MLL binds a CpG island at Hoxa9 with higher affinity than undimerised MLL; and the authors' model is that either dimerisation/exon duplication or fusion to a transcriptional activator produces Hox upregulation and transformation 2. The truncated MLL in that system is a protein that has lost every module in this beat's ledger. It needed something added before it transformed anything.
That third point is the decisive one. If losing restraint were the mechanism, a truncated MLL should be oncogenic on its own. The experiment closest to that test says it is not - something must be added, whether a partner's elongation machinery or an artificial dimerisation.
So what is the defensible version?
Not 'the fusion loses restraint'. Rather:
The break produces a molecule that retains chromatin targeting, gains a constitutive activating output from the partner, and is structurally exempt from the regulation that still governs its wild-type counterpart at the same loci.
This version survives all three objections. It does not require the lost modules to be individually load-bearing (most have never been tested). It accommodates the trans-dependency (the brake operates, just not on the fusion). And it puts the gain where the dimerisation experiment says the gain must be.
It is also more testable. 'Exempt from regulation that still operates' predicts that raising CyP33/PPIE in a fusion-driven cell should repress the locus via the wild-type allele while leaving the fusion untouched - a measurable, allele-resolved prediction. 'The brake is gone' predicts nothing that can be checked.
The explicit lost / load-bearing accounting the King asked for
Losses with a demonstrated functional consequence (2 of 15):
- PHD3 - in-cis add-back blocks replating immortalisation in MLL-ENL. One study, one fusion, in vitro, unreplicated (
ana-phd3-addback-replication-audit). And the consequence points at restoration breaking the fusion, not at loss building it. - MOF contact - indirect but strong: Mll1 deletion depletes H4K16ac at target genes with reduced transcription. Measured on wild-type protein, not on the fusion.
Losses demonstrated NOT to matter (1 of 15):
- SET domain - dispensable for HSC maintenance and for MLL-AF9 leukemogenesis. Contradicted pharmacologically by MM-401, unresolved.
Losses that are merely absent, with no demonstrated consequence (11-12 of 15):
- PHD1, PHD4, the pre-PHD C2HC finger, the bromodomain (as a separable unit), the HCF-binding motif, FYRN, FYRC, both taspase1 cleavage sites, and - functionally - the TAD, whose job the partner does differently.
The ratio is the finding. Eleven or twelve of fifteen lost modules have never been asked whether their loss matters. Writing a mechanistic narrative over that base is the mirror image of the error this project made before, when it read 'retained' as 'required'. Here the temptation is to read 'lost' as 'load-bearing', and for roughly four fifths of the ledger there is no evidence either way.
CORRECTION (cycle 31): point (1) is withdrawn, and the pattern is brakes on abundance
What was withdrawn. This entry's first argument against the restraint reading was that the best-evidenced KMT2A-side loss is an activating output - the MOF/KAT8 contact - rather than a brake. That argument rested on this graph's own ranking in subtraction-kmt2a-mof-contact, which has since been corrected. MOF was finally tested inside a fusion-driven leukemia: it is required, but not through the fusion's target programme (no clear Meis1 or Hoxa downregulation), the mechanism is global H4K16ac loss and DNA-damage-repair failure, and it is equally required in NUP98-fusion AML 5. MOF is a dependency of the cell. It is not the KMT2A half's best-evidenced loss, so it cannot be the counterexample that sinks the restraint reading.
What replaces it. The losses with measured consequences turn out to be brakes after all - but not the brakes the story names. Three independent determinants of KMT2A turnover sit C-terminal to the breakpoint and are gone from every der(11) fusion: the PHD2 E3 ligase, the taspase1 cleavage sites, and the FYRN degron that marks an unpartnered MLL N-terminal fragment for destruction. Disabling each of them in wild-type MLL1 produces the same phenotype: more protein, longer-lived, more chromatin-bound 6.
So the corrected verdict on the 'loss of restraint' reading is not that it is over-stated, but that it is aimed at the wrong faculty. The fusion is not conspicuously freed from brakes on what it does. It is freed from brakes on how much of it there is and how long it stays. That is written up as hyp-kmt2a-half-loses-abundance-control-not-activity-control.
What still stands against the strong form. Points (2) and (3) are unaffected and one of them is stronger: the PHD function is outsourced rather than removed, and the wild-type Mll allele is now known to be genetically required for MLL-AF9 leukemogenesis and maintenance 7, which is a stronger statement than the prebinding model it previously rested on. And loss of restraint remains demonstrably insufficient on its own.
The honest caveat. All three degradation determinants were measured on wild-type MLL1, never on a fusion, and one of them (taspase1) has a well-executed study reading it in the opposite direction. Three wild-type results pointing one way is a pattern worth testing, not a conclusion.

- If the fusion's advantage is exemption rather than absence, then therapies designed to 'restore the brake' to the fusion must re-engineer the oncoprotein itself; nothing that acts on the wild-type allele will help, because the wild-type allele's brake is already working and is already being ignored.
- If loss of restraint is not sufficient and a partner-supplied gain is required, then the partner side remains the load-bearing half and the KMT2A-side subtraction beat is describing context rather than cause - which is a reason to keep investing in menin and elongation interfaces rather than in restoring lost KMT2A modules.
- Does raising CyP33/PPIE in a KMT2A-rearranged cell repress HOXA9 through the wild-type allele while leaving the fusion untouched? That is the allele-resolved prediction the 'exemption' model makes and the 'brake removed' model does not, and it needs only CUT&RUN plus an inducible CyP33.
- Is undimerised truncated MLL genuinely non-transforming? The claim is load-bearing for this entry's verdict and rests on the logic of an inducible-dimerisation design rather than on a sentence in an abstract. Someone should read the Martin 2003 full text and settle it.
- Does any of the 11-12 untested lost modules matter? A single in-cis add-back panel in one KMT2A::AFF1 line - HBM, PHD2, TAD, each short enough to insert - would move roughly a third of the ledger off 'unmeasured' in one experiment.
- If the KMT2A half's losses are brakes on abundance, is the fusion measurably longer-lived than wild-type KMT2A in the same cell? That single measurement decides whether this entry's corrected verdict is right, and it has never been published head-to-head.

An essential MLL1 recruitment domain includes the CXXC domain and the PHD fingers and is controlled by interactions with the PAF elongation complex and H3K4me2/3; MLL1 fusion proteins lack the PHD fingers and require prebinding of a wild-type MLL1 complex, plus CXXC recognition of DNA, for stable HoxA9 association; wild-type MLL1 and MLL1 fusion proteins have overlapping yet distinct recruitment mechanisms.

ABSTRACT ONLY. Recruitment assays at one locus (HoxA9) in cell systems; establishes a trans requirement for stable binding, not for transformation. The 'prebinding' model is the authors' interpretation of recruitment data rather than a direct observation of ordered occupancy.
Evidence that argues against this (1)
- The recruitment domain result is a statement about where MLL1 GOES, and this entry uses it as a statement about restraint. Requiring prebound wild-type complex plus CXXC-DNA recognition for stable HoxA9 association bears on targeting, and a domain can be necessary for localisation while contributing nothing to the restraint of activity that this entry is trying to adjudicate. Induced dimerisation of a truncated MLL immortalises bone marrow and imposes a reversible block on myeloid differentiation associated with upregulation of Hoxa7, Hoxa9 and Meis1; both dimerised MLL and exon-duplicated MLL are potent transcriptional activators; dimerised MLL binds a CpG island within the Hoxa9 locus with higher affinity than undimerised MLL; MLL-AF9 is not dimerised in vivo; and the data support a model in which either MLL dimerisation/exon duplication OR fusion to a transcriptional activator results in Hox upregulation and ultimately transformation.

ABSTRACT ONLY, and this is the key caveat for how the entry uses it: the abstract states that INDUCED DIMERISATION of a truncated MLL immortalises, and contrasts dimerised with undimerised MLL for Hoxa9 binding affinity, but it does not contain an explicit sentence reporting that undimerised truncated MLL fails to immortalise. The inference that truncation alone is insufficient rests on the experimental logic of an inducible-dimerisation design and on the authors' model sentence requiring either dimerisation or fusion to an activator. A reader wanting to rely on this should read the full text. Murine bone marrow, in vitro.
Evidence that argues against this (1)
- A caution in the same abstract: MLL-AF9 is explicitly NOT dimerised in vivo, so the dimerisation model is a parallel route to transformation rather than a description of how natural fusions work. It supports 'something must be added' without specifying that the addition is dimerisation. Ono R et al. 2005

Upon Mll1 deletion, H4K16 acetylation is selectively depleted at MLL1 target genes in conjunction with reduced transcription, and recruited MOF activity rather than the intrinsic histone methyltransferase activity of MLL1 is central to maintaining those target genes; the SET domain itself is dispensable for haematopoiesis and for MLL-AF9-driven leukemogenesis.

- This cuts against reading the SET domain as a lost restraint module in either direction, because the same body of work finds MLL1's methyltransferase activity dispensable for haematopoiesis and leukemogenesis. Conditional Mof deletion in MLL-AF9-transformed cells abolishes colony formation and extends median survival from 22-24 to 31 days in vivo, with MOF HAT activity required for rescue - but MOF loss does NOT clearly downregulate MLL-AF9 target genes such as Meis1 and the Hoxa cluster, the mechanism is global H4K16ac depletion and DNA-damage-repair gene loss, and Mof is equally required for NUP98-fusion-driven AML with the MYST inhibitor MG149 killing three non-KMT2A-rearranged leukemia lines.
