Endogenous aldehydes and silenced ALDH2 — the rival explanation for an FA dependency in acute leukaemia, and the only one with an experiment behind it
also called ALDH2 silencing · aldehyde genotoxicity · UBE2T FANCL synthetic lethality
Cells constantly make small amounts of formaldehyde and acetaldehyde as by-products, and these chemicals glue the two strands of DNA together where they should slide apart. Two systems defend against this: an enzyme that destroys the aldehyde, and the Fanconi crew that repairs the damage it causes. Some leukaemia cells switch the first defence off, which leaves them completely dependent on the second — and that, not anything to do with the KMT2A fusion, may be why this project's screen found the Fanconi genes.
The same thing, for a scientist
A domain-focused CRISPR screen of the ubiquitination machinery across cancer cell lines found UBE2T and FANCL to be selective dependencies in AML, and traced them to a synthetic-lethal interaction with ALDH2: the two act in parallel pathways against endogenous aldehyde genotoxicity, and recurrent DNA hypermethylation silences ALDH2 in human AML, which is by itself sufficient to confer FA-pathway dependency [E1]. This is the rival that matters, because (a) it is the *same two genes* the DepMap signal here nominates most strongly among the enzymes, (b) it is an experiment rather than a correlation, and (c) it makes a prediction that would reassign this project's entire finding: the dependency should track ALDH2 promoter methylation, not KMT2A rearrangement. Against it, in this project's own data: the non-KMT2A AML arm shows the FA set depleted in the **opposite** direction (headline-eight mean d +0.095, permutation p = 0.999), and the KMT2A signal survives with lineage held constant in **both** AML (-0.334) and B-ALL (-0.308) [E2]. Those are not contradictory — the published screen's comparator is other cancer lineages, not other leukaemias, so an AML-wide dependency would cancel in an AML-versus-other-leukaemia contrast — but they do mean the ALDH2 model, as stated, does not explain the KMT2A increment.
The model, and why it is the serious rival
Two parallel defences stand between a cell and its own aldehyde output. ALDH2 destroys acetaldehyde before it reaches DNA. The FA pathway repairs the interstrand cross-links that get through. Knock out one and the cell is fine; knock out both and it is not. That epistasis is established in mouse haematopoiesis and is the basis of the leukaemia result here 1.
What makes it the serious rival rather than a footnote:
- It nominates UBE2T and FANCL specifically — the E2 and the E3 — which is exactly the sub-module that
mech-fa-core-complex-heterogeneity-kmt2aidentifies as carrying this project's signal. Two independent routes converging on the same two enzymes is not nothing. - It is a synthetic lethality with a defined, measurable marker: ALDH2 promoter methylation. That is a testable reassignment, not a vague alternative.
- It explains the dependency without invoking transcription at all, so it needs none of the unmeasured links in
mech-transcription-replication-conflict-kmt2a-r. - It is in acute myeloid leukaemia, the right disease, with human cells.
What this project's data says about it, in both directions
Against the ALDH2 model explaining the KMT2A finding. If the FA dependency in acute leukaemia were simply AML-with-silenced-ALDH2, then an AML-versus-other-leukaemia contrast should show it. It shows the reverse: the eight headline genes average +0.095 in non-KMT2A AML lines against the rest of the leukaemia panel, and the FA set's permutation p is 0.999 — enriched away from dependency 2. And the KMT2A increment survives when lineage is held exactly constant, in both directions: -0.334 for KMT2A-rearranged AML against non-KMT2A AML, -0.308 for KMT2A-rearranged B-ALL against non-KMT2A B-ALL 2.
Why that is not a refutation. The published screen compared AML lines against other cancer lineages, not against other leukaemias. If every leukaemia line in the comparator arm also silences ALDH2, an AML-versus-other-leukaemia contrast cancels a real shared dependency exactly to zero, and a small excess in one subtype is all that remains visible. The two observations are compatible; they are measured against different baselines.
The experiment that separates them is cheap and has not been done here. ALDH2 expression and promoter methylation are available for every DepMap line. If ALDH2 expression predicts FA dependency across the 54-line leukaemia panel better than KMT2A rearrangement status does, this entry replaces the transcription-replication-conflict entry as the explanation. If KMT2A status still adds after conditioning on ALDH2, it does not. That single regression would resolve the most important open question in this beat.
Where the aldehyde story and the KMT2A story could genuinely meet
Speculative and flagged as such, but worth writing down because it is testable.
KMT2A-rearranged infant leukaemia arises in utero, and the fetal haematopoietic compartment is where the aldehyde-FA axis has its sharpest known phenotype — Fanconi anaemia's haematopoietic failure is a fetal and early-postnatal stem-cell attrition, not an adult one. A fusion that expands a fetal-liver-like progenitor pool could be placing cells in a compartment where aldehyde defence is already marginal, which would make an FA requirement a property of the cell of origin rather than of the fusion's transcriptional output.
That predicts the dependency should be strongest in the most fetal-like models — infant KMT2A::AFF1 lines and patient-derived xenografts — and weakest in adult-derived AML lines. The four B-lineage lines in this project's index arm are the closest thing available to that test, and they are indeed the stronger half for seven of the eight genes 2. That is a suggestive coincidence and nothing more: four lines, and B-lineage is confounded with everything else about them.

- If the dependency is aldehyde-driven, the correct patient selection marker is ALDH2 methylation and not KMT2A rearrangement, and a trial enrolled on fusion status would dilute its own effect.
- ALDH2 loss-of-function is common in East Asian populations. A therapy that exploits an FA dependency created by ALDH2 deficiency has an obvious and unusually large pharmacogenomic safety question attached to it in exactly those patients' normal tissue.
- Does ALDH2 expression predict FA-gene dependency across the DepMap leukaemia panel better than KMT2A rearrangement does?
- Are the eleven KMT2A-rearranged lines in this index arm ALDH2-silenced, and if so is that a property of the fusion or a coincidence of which lines exist?
- Does ADH5 (formaldehyde) behave like ALDH2 (acetaldehyde) in these lines?
- Is the fetal origin of KMT2A-rearranged infant leukaemia relevant, given that the aldehyde–FA axis has its sharpest phenotype in fetal and early-postnatal haematopoietic stem cells?

Domain-focused CRISPR screening identified UBE2T and FANCL as selective dependencies in AML; the dependency is a synthetic lethal interaction with ALDH2, which acts in a parallel pathway against endogenous aldehyde genotoxicity, and recurrent DNA hypermethylation silences ALDH2 in human AML sufficiently to confer FA-pathway dependency.

Cancer cell lines, with AML compared against other cancer lineages rather than against other leukaemias. The screen's library was focused on ubiquitination domains, so it could only have nominated E2s and E3s. KMT2A rearrangement status of the dependent lines is not reported in the abstract.

- This project's non-KMT2A AML arm shows the FA set depleted in the opposite direction (+0.095, p = 0.999) against other leukaemia lines, and the KMT2A increment survives with lineage held constant. With lineage held exactly constant the KMT2A increment persists: KMT2A-rearranged AML against non-KMT2A AML gives headline-eight mean d -0.334 (p = 0.0001) and KMT2A-rearranged B-ALL against non-KMT2A B-ALL gives -0.308 (p = 5e-05), while non-KMT2A AML against other leukaemia gives +0.095 (p = 0.999).

Seven KMT2A-rearranged AML lines against 25, and four KMT2A-rearranged B-ALL lines against 9. ALDH2 expression and methylation were not measured or conditioned on, which is the analysis that would actually decide between the two models.
Evidence that argues against this (1)
- A contrast between two arms that both silence ALDH2 cannot see a dependency they share. The +0.095 in non-KMT2A AML is therefore equally consistent with 'no AML FA dependency' and with 'an AML FA dependency shared by the comparator'. Yang Z et al. 2021

No published work connects KMT2A or MLL to ALDH2 or aldehyde dehydrogenase: a PubMed title/abstract search returns four records, all of which are unrelated on title-checking (fish ecotoxicology, vinegar biotransformation, and two alcohol-metabolism studies where MLL matched an unrelated abbreviation).
PubMed counted-absence search for KMT2A/MLL against ALDH2 and aldehyde dehydrogenase, run 2026-09-20 and title-checked · PubMed (NCBI) 2026What this evidence cannot show
`MLL` is a short abbreviation and matched unrelated usages, which is why every hit was title-checked; a four-record raw count that reduces to zero is a textbook alias-precision failure in the making. The absence concerns titles and abstracts, not DepMap-scale analyses that would not name either term.
