Hypothesis: the same drug is protective in the islet and harmful in the parathyroid and the growth plate

also called tissue-divergent menin inhibition · islet benefit parathyroid harm

We think blocking menin helps insulin-making cells while harming the parathyroid glands and the growing skeleton, because those tissues use menin for different jobs on different timescales. If that is right, the same child can benefit and be harmed at the same time, in different organs.

The same thing, for a scientist

The synthesis this beat needs, stated so it can be killed. Claim: menin inhibition produces **tissue-divergent** outcomes because the menin-dependent restraint being released differs by tissue in both its target and its recovery kinetics. In the beta cell, released restraint drives compensatory proliferation that is physiologically bounded and reverses when the stimulus ends, giving net benefit over months [E1][E2]. In the parathyroid chief cell, the released restraint is on **secretion** as well as growth, so PTH rises within weeks and there is no physiological counter-regulation to bound it [E3][E4]. In the skeleton, the released restraint lands on a developmental window that does not reopen, so even a short exposure can be permanent [E5]. The prediction is not that the drug is safe or unsafe but that **its sign depends on the tissue and on whether that tissue has a bounded recovery**.

Discriminating predictions

P1. In revumenib-treated patients, fasting glucose falls or is unchanged while intact PTH rises, in the same individuals. The two should be uncorrelated or inversely correlated, not co-directional. Testable today on existing trial chemistry data.

P2. Intact PTH rises with drug exposure (AUC or trough) after adjustment for creatinine and phosphate; glucose does not track creatinine. Testable today on existing PK and laboratory data.

P3. After the last dose, PTH returns towards baseline within weeks (secretory disinhibition, reversible), whereas any height-velocity deficit does not recover (developmental window, irreversible). Requires the off-treatment arm that exp-endocrine-surveillance-panel-menin-treated-children proposes and that FDA's PMR does not compel.

P4. In an animal given a clinical menin inhibitor for 12 weeks and then withdrawn, islet proliferation normalises while parathyroid chief-cell hyperplasia and growth-plate closure do not.

P5. A menin inhibitor applied to primary human parathyroid cells abolishes TGF-beta suppression of PTH secretion at concentrations at or below those that displace KMT2A - i.e. the endocrine effect is not a high-dose artefact.

What would kill it

See data.falsifiers. The cleanest single kill is P1 failing in the co-directional sense: if glucose rises on treatment in the same patients whose PTH rises, then the beta-cell benefit does not translate at clinical exposures and the whole divergence framing collapses into a simpler one-sided toxicity story - which is a legitimate and useful outcome, and one this beat should be equally willing to reach.

Prior and why it is not higher

Set at 0.35. It is not higher because (a) no clinical menin inhibitor has ever been tested on an islet, a parathyroid cell or a chondrocyte, so the entire tissue-divergence argument is assembled from tool-compound and genetic data; (b) the human biochemical pattern on revumenib - phosphate up, creatinine up - resembles renal secondary hyperparathyroidism more than primary parathyroid autonomy, which is a direct strike against the parathyroid arm [see mechanism-parathyroid-autonomous-menin-loss-causes-hypercalcaemia]; and (c) the growth-plate arm has no mechanism at all, only a toxicology finding and an absence of literature.

Facts and figures
statementMenin inhibition is net protective in the pancreatic islet and net harmful in the parathyroid and the growing skeleton, because the restraint released differs by tissue in target and in recovery kinetics.

How this could bite us back
  • If the hypothesis is right, a benefit in one organ cannot be traded against harm in another, because the child gets both and only one of them is reversible.
  • A tissue-divergent drug cannot be dose-optimised for safety: the concentration that spares the parathyroid also spares the leukaemia.
  • The hypothesis's own benefit arm depends entirely on tool compounds; if it fails to translate, the graph is left with the harms and no offset.
Still open
  1. Is there a tissue in which menin inhibition is neutral, and what distinguishes it?
  2. Could intermittent dosing preserve the leukaemic effect while allowing endocrine tissues to recover between cycles?
  3. Does the divergence hold in a child, whose islet, parathyroid and growth plate are all in different developmental states from an adult's?
