MECOM (EVI1) - a cell-of-origin memory that MLL-AF9 maintains but cannot install, present in about 43% of MLL-rearranged AML and marking the chemoresistant half

also called EVI1 · MDS1 · MDS1-EVI1 · PRDM3 · KMT8E · EVI-1 · Ecotropic virus integration site 1 protein homolog · MDS1 and EVI1 complex locus protein

EVI1 is a stem-cell gene that the MLL-AF9 fusion can keep switched on but cannot switch on by itself: whether a leukaemia has it depends on which cell the fusion landed in. The leukaemias that carry it are the harder, more chemotherapy-resistant ones, and turning it down makes mouse leukaemias easier to treat.

The same thing, for a scientist

MECOM (Q03112, 3q26.2) is the MDS1-and-EVI1 complex locus. In this disease its central result is a **cell-of-origin** one rather than a downstream-output one. Roughly **43% of MLL-rearranged AMLs are EVI1-positive**, across MLL-AF6, -AF9, -AF10, -ENL and -ELL, and EVI1-positive cases differ molecularly, morphologically and immunophenotypically from EVI1-negative ones [E1]. Mechanistically, **MLL-AF9 maintains Evi1 expression when it transforms an Evi1-positive HSC and does not activate Evi1 in an Evi1-negative GMP** [E1] - MLL-AF9 is a keeper, not an installer, and the paper is titled for that partner even though its 43% cohort figure is MLL-rearranged broadly. Krivtsov and colleagues made the same point from the other direction: leukaemia stem cells derived from HSCs inherit high Evi-1 from the cell of origin, their signature is enriched in poor-prognosis human MLL-rearranged AML in three independent data sets, they carry globally elevated 5-methylcytosine mirroring the EVI1-positive/negative split in human MLL-rearranged leukaemia, and they are **more chemoresistant** [E3]. Where it is on, it is load-bearing: shRNA knockdown of Evi1 in an Evi1-positive MLL-AF9 mouse model inhibits leukaemia growth in vitro and in vivo and increases sensitivity to chemotherapy [E1], and MLL-ENL directly activates the *Evi-1* promoter, with the up-regulation occurring exclusively in the undifferentiated haematopoietic population [E2]. The best-characterised EVI1 effector is **ERG**, a direct transcriptional target at a conserved intragenic enhancer whose suppression differentiates EVI1-driven AML and whose ectopic expression abolishes the EVI1 dependence [E4] - which puts two of this beat's six genes on one axis. **DepMap sees none of this** (z +0.08 myeloid, -0.57 B-lineage, rank 4,664 of 17,728) and that is expected rather than contradictory: the panel's KMT2A-rearranged lines are largely EVI1-negative and a dependency present in 43% of a disease is averaged away by a screen that does not stratify on it [E5].

This is a prediction, not a measurement. No one has seen this protein in this shape. The colours say how much the software trusts itself, and that is worth reading before anything else — the orange stretches are places the model is effectively saying this part has no fixed shape.

A protein is one long chain of amino acids that folds up. The ribbon traces that chain from one end to the other; it is not a wire, it is a simplification of where the backbone runs. Drag to rotate, scroll to zoom.

Domains are the stretches of chain that do a particular job, and they are what the coloured blocks above the picture mark out. Click one and the view flies to it. In this project the important question about any domain is usually whether the fusion still has it — the labels say lost where it does not.

Anything in yellow sticks is a bound small molecule, which is usually the drug.

Expressed in / required in / maintains a state
  • Expressed in. ~43% of MLL-rearranged AMLs, across five fusion partners 1; up-regulated in murine bone-marrow cells transformed by MLL-ENL or MLL-AF9 2; high in HSC-derived but not GMP-derived MLL-AF9 leukaemia stem cells 3. The expression is conditional on the cell of origin, which is the single most important fact on this entry. In this graph's own material MECOM also appears as an overexpression event in the KOPB-26 KMT2A::MLLT3 infant B-ALL line, where it sits beside KRAS G12A as a confounder for anything attributed to the fusion.
  • Required in. Evi1-positive MLL-AF9 murine leukaemia - knockdown inhibits growth in vitro and in vivo 1; propagation of MLL-ENL-immortalised cells 2. Not required in Evi1-negative MLL-rearranged leukaemia, by the same paper's logic, and not a CRISPR fitness dependency in any leukaemia arm of DepMap 5.
  • Maintains a state. Two distinct states. (i) The HSC-like, 5mC-high, chemoresistant leukaemia-stem-cell state inherited from the cell of origin 3. (ii) The undifferentiated compartment specifically - MLL-ENL's up-regulation of Evi-1 occurs exclusively there 2. EVI1 is therefore less a downstream output of the fusion than a marker and maintainer of which cell the fusion started in.
Why the fusion is a keeper and not an installer, and why that changes the question

Bindels and colleagues transduced murine bone marrow with MLL-AF9 and compared two starting populations 1:

starting cellEvi1 beforeEvi1 after MLL-AF9 transformation
Evi1-positive HSConmaintained on
Evi1-negative GMPoffstays off

That is a clean statement that MLL-AF9 cannot activate Evi1 de novo. Krivtsov and colleagues reach the same conclusion from single-cell transduction and transplantation: HSC-derived and GMP-derived leukaemia stem cells have the same immunophenotype and different gene-expression programmes, including high Evi-1 only in the HSC-derived ones, and that difference tracks 5mC levels and chemoresistance 3.

The consequence for this graph. EVI1 does not belong in the list of things MLL-AF9 - or, on present evidence, any KMT2A fusion - drives. It belongs in the list of things that determine what kind of leukaemia the fusion produces, alongside hyp-kmt2a-fusion-insufficient-without-permissive-cell and mechanism-lineage-switch-kmt2a. It is a property of the soil, retained through transformation, that then becomes load-bearing.

A caveat that matters and is rarely stated: both papers are murine MLL-AF9/MLL-ENL myeloid systems (rule 73). Nothing here is B-lineage and nothing here is KMT2A::AFF1. The human arm of 1 is expression and immunophenotype in an AML cohort with 11q23 translocations.

The EVI1-to-ERG axis, which puts two of this beat's genes in one sentence

Schmoellerl and colleagues built tractable human and murine EVI1-driven AML models, withdrew EVI1 in vitro and in vivo, and ran the first genome-wide CRISPR screens in EVI1-dependent AML 4. Integrating conserved transcriptional targets with genetic dependency data identified ERG as a direct transcriptional target of EVI1, occupied at a conserved intragenic enhancer in AML cell lines and primary patient samples, aberrantly expressed and selectively required in both human and murine EVI1-driven AML. Suppressing ERG induces terminal differentiation; ectopic ERG abrogates the dependence on EVI1 - an epistasis result, and the strongest statement in this whole beat that one of these factors sits downstream of another.

This is EVI1-driven (3q26-rearranged) AML, not KMT2A-rearranged AML. Whether the EVI1-to-ERG axis operates in the EVI1-positive 43% of MLL-rearranged AML is untested, and it is the most obvious experiment this beat can name.

Identifier hazards - MECOM is the worst on this beat

Four traps, three of them capable of producing a confident wrong answer:

  1. EVI1 and MDS1 are PREVIOUS OFFICIAL HGNC SYMBOLS, not aliases (HGNC:3498 prev_symbol = [MDS1, EVI1], fetched live 2026-09-20). Rule 69 part 1 applies: a previous official symbol is where the older literature lives, and essentially all the pre-2015 work publishes under EVI1. Measured: MECOM[tiab] AND (MLL OR KMT2A)[tiab] returns 38; widening to (MECOM OR EVI1 OR "MDS1-EVI1" OR PRDM3)[tiab] returns 112 - a 2.9-fold undercount from the current symbol alone.
  2. The canonical accession is the wrong molecule for 'EVI1'. Q03112's displayed sequence is isoform 7 at 1,230 aa and carries a SET domain at 78-190 - the MDS1 PR domain. The EVI1 protein does not have it. UniProt lists eight named isoforms including 'Long, Evi-1a' and 'Evi-1c, Mds1/Evi1'. Asserting Q03112 + 1,230 aa for a claim about EVI1 is the PKM2 trap: the query succeeds, accession and length verify, and the object verified is the parent locus rather than the protein meant. The PR/SET domain's presence is not cosmetic - MDS1-EVI1 and EVI1 behave differently in transformation assays, so the isoform is the claim.
  3. PRDM3 and KMT8E are aliases from the methyltransferase nomenclature, and UniProt's recommended protein name is Histone-lysine N-methyltransferase MECOM - a reader checking 'is this the EVI1 oncogene?' gets a methyltransferase and may reject a correct record (the DPF2 failure shape).
  4. EVI1 is also the murine Evi-1 retroviral integration site name, and spellings EVI1, Evi-1 and EVI-1 are three different PubMed strings.

what this entry does not claimThat the KMT2A fusion drives MECOM/EVI1. The published result is the opposite, and it is specifically an MLL-AF9 result: MLL-AF9 maintains Evi1 where it is already on and cannot install it where it is off. No other partner has been tested this way.

How this could bite us back
  • EVI1 is a zinc-finger transcription factor with no ligandable pocket and a decade of failed attempts at PRDM-family chemistry. The only routes are protein-protein interfaces or degraders, and the degrader route is complicated by eight isoforms.
  • EVI1 is essential for normal HSC regulation, so an EVI1-directed agent has a marrow-failure toxicity built into its mechanism - the same shape as the essential-function problem this graph records for menin.
  • The therapeutic logic that works is INVERSE: knocking EVI1 down sensitises to chemotherapy [E1]. That means EVI1 status is a stratification biomarker before it is a target, and the immediate clinical action is to measure it, not to drug it.
  • If EVI1 is a cell-of-origin memory the fusion cannot install, then an agent blocking the fusion does not remove EVI1 - and the chemoresistant, hypermethylated programme survives menin inhibition by construction. Nobody has tested this and it is the most consequential prediction on this entry.
  • 43% is a cohort statistic from one series under one threshold. A stratification biomarker whose prevalence has been measured once is not ready to select patients.
Still open
  1. Does the EVI1-to-ERG axis operate in the EVI1-positive 43% of MLL-rearranged AML? [E4] establishes it in 3q26-driven AML with an epistasis result; the transfer experiment has never been run, and if it transfers then ERG becomes a drug target for a defined subset of this disease rather than a bystander.
  2. What is EVI1 status in KMT2A::AFF1 infant B-ALL? Every EVI1 datum in this disease is myeloid. If the cell of origin for infant t(4;11) is a fetal-liver HSC or LMPP rather than a committed pro-B cell - which this graph argues elsewhere - then the cell-of-origin logic predicts EVI1-positivity, and nobody has looked.
  3. Is DepMap's KMT2A-rearranged arm stratifiable by EVI1 expression? The data exist publicly. A CRISPR effect computed within the EVI1-high half is the one number that would test E1 in human cells, and this beat did not have the budget to run it.
  4. Does menin inhibition move EVI1? If EVI1 is a cell-of-origin memory rather than a fusion output, it should be MENIN-INDEPENDENT - and a menin inhibitor that collapses HOXA9/MEIS1 while leaving EVI1 standing would leave the chemoresistant programme intact, which is a testable resistance prediction.
  5. Which isoform is measured in the MLL-rearranged cohorts - EVI1 or MDS1-EVI1? The papers say EVI1, and the PR/SET domain difference is functionally decisive, but no source read this cycle distinguishes them at the protein level in MLL-rearranged material.

Evi-1 is frequently up-regulated in murine bone-marrow cells transformed by the MLL chimeric genes MLL-ENL or MLL-AF9; analysis of the Evi-1 promoter region showed that MLL-ENL activates transcription of Evi-1; the up-regulation occurs exclusively in the undifferentiated haematopoietic population, in which Evi-1 particularly contributes to the propagation of MLL-ENL-immortalised cells; and human AML cases with MLL rearrangement and high Evi-1 show a stem-cell-like gene-expression signature.

Abstract fetched from PubMed efetch XML on 2026-09-20 and read in full; body not retrieved, so the promoter region tested and whether MLL-ENL occupancy was shown by ChIP (as opposed to reporter activation) are NOT asserted - the abstract says 'analysis of the Evi-1 promoter region revealed that MLL-ENL activates transcription', which is compatible with a reporter assay. MURINE, MYELOID, MLL-ENL and MLL-AF9. THIS SITS IN TENSION WITH E1, which finds MLL-AF9 unable to activate Evi1 in an Evi1-negative GMP; the reconciliation offered by both papers is that the activation is restricted to the undifferentiated compartment, but the two results are not identical and the entry does not pretend they are.

Evidence that argues against this (1)
    • E1 shows MLL-AF9 does NOT activate Evi1 in initially Evi1-negative GMPs, which is hard to square with a straightforward 'MLL fusion activates the Evi-1 promoter' reading of this paper. The compartment restriction both papers report is the likeliest reconciliation, but a promoter-reporter activation in a permissive cell and a failure to activate an endogenous silent locus in a committed progenitor are different experiments, and the graph should not merge them. Abstract fetched from PubMed efetch XML on 2026-09-20 and read in full; body (PMC4693300) not retrieved, so the chemotherapy agent, the 5mC assay and the three validation data sets are NOT named here. MURINE MLL-AF9 MYELOID for the causal arms; the human arms are gene-expression and methylation comparisons in existing MLL-rearranged AML cohorts. This paper does not itself test whether Evi-1 is REQUIRED - it establishes that Evi-1 status is inherited from the cell of origin and that it tracks methylation and drug response. The requirement statement is E1's.

Evidence that argues against this (1)
    • An inherited gene-expression programme that correlates with chemoresistance need not be causal for it: HSC-derived and GMP-derived leukaemias differ in many genes and in global 5mC, and attributing the drug-response difference to Evi-1 specifically requires the knockdown, which is done in E1 in a different model and shows chemosensitisation. The two together make the case; neither does alone. Bindels EM et al. 2012

In experimentally tractable human and murine EVI1-driven AML models with genome-wide CRISPR screening, the ETS transcription factor ERG is a direct transcriptional target of EVI1 that is aberrantly expressed and selectively required in both human and murine EVI1-driven AML; EVI1 controls ERG expression and occupies a conserved intragenic enhancer region in AML cell lines and primary patient samples; suppression of ERG induces terminal differentiation of EVI1-driven AML cells, and ectopic expression of ERG abrogates their dependence on EVI1.

    What this evidence cannot show

Abstract fetched from PubMed efetch XML on 2026-09-20 and read in full; body not retrieved, so the models' genotypes, the enhancer coordinates and the screen's hit list are NOT asserted. THE SYSTEM IS 3q26/EVI1-REARRANGED AML, NOT KMT2A-REARRANGED AML. Whether this axis operates in the EVI1-positive subset of MLL-rearranged AML is untested and is written here as an open question rather than as an inference.

Evidence that argues against this (1)
    • The epistasis result - ectopic ERG abrogating EVI1 dependence - is a strong claim about EVI1-driven AML and says nothing about a leukaemia driven by a KMT2A fusion where EVI1 is a retained cell-of-origin marker rather than the driver. In KMT2A-rearranged lines DepMap finds ERG is if anything LESS required than in other leukaemia lines (z +0.48 myeloid), so the axis does not visibly transfer. Fetched live on 2026-09-20, DepMap Chronos gene effect for MECOM has mean +0.0199 across 1,257 cell lines with 2.07% of lines at or below -0.5; among leukaemia lines the KMT2A-rearranged myeloid half sits at z +0.08 and the B-lineage half at z -0.57 relative to 44 other leukaemia lines, and this project's genome-wide scan ranks MECOM 4,664 of 17,728 on KMT2A-selectivity with a difference of -0.0365. MEN1 through the identical query shape returned z -2.41 / -2.43.

    What this evidence cannot show

A bounded negative with a date, a route and a positive control through the identical query shape (rule 67). THE BOUND IS THE WHOLE POINT: the published dependency is confined to the EVI1-POSITIVE subset, which is about 43% of MLL-rearranged AML [E1], and none of the 11 KMT2A-rearranged DepMap lines is annotated for EVI1 status in this query. A screen that pools an EVI1-positive minority with an EVI1-negative majority returns the null it returned here, and that null is uninterpretable as evidence against E1. The only informative row is B-lineage z -0.57, driven largely by RS4;11 at -0.433 on four lines - one line, not a result. Ensembl id from HGNC primary (HGNC:3498); approvedSymbol read back as MECOM.

Evidence that argues against this (1)
    • MECOM is the clearest case on this beat where a clean DepMap null costs the reader nothing and tells them nothing. The measurement that would be informative - CRISPR effect stratified by EVI1 expression within the KMT2A-rearranged arm - is computable from public data and has not been run by anyone, including this project.
