CSPG4 / NG2 - the flow-cytometry surrogate for a broken KMT2A, and a direct target gene of the fusion that makes it
also called CSPG4 · NG2 · neuron-glial antigen 2 · MCSP · melanoma chondroitin sulfate proteoglycan · 7.1 antigen · Chondroitin sulfate proteoglycan 4 · Q6UVK1
NG2 is a large sugar-coated protein that appears on the surface of leukaemia cells when the KMT2A gene is broken, and a flow cytometer can see it in hours where a genetic test takes days. The catch is that NG2 is not reading the broken gene - it is reading what the broken gene switches on, which is why it can be present without the break and absent with it.
The same thing, for a scientist
CSPG4 (UniProt **Q6UVK1**, CSPG4_HUMAN, **2322 aa**, single reviewed human record, synonym MCSP) encodes the chondroitin sulfate proteoglycan called **NG2** in the leukaemia literature and detected by monoclonal antibody **7.1**. It is the single most informative immunophenotypic surrogate for 11q23/KMT2A rearrangement, and this graph has cited its performance numbers in `concept-flow-cytometry-immunophenotype-kmt2a-r` without ever writing the molecule. **The measured performance, in four independent designs.** Behm 1996 screened 104 consecutive children with ALL prospectively: 9 were NG2-positive (8.6%), all 9 carried t(4;11) or t(11;19) with MLL rearrangement, and **no NG2-negative case carried an MLL rearrangement** - including 7 with del(11)(q23) or inv(11)(p13q23) [E1]. Wuchter 2000 took it to 533 samples and got the opposite kind of answer: **58 of 72 (81%) MLL-rearranged samples were 7.1-positive** at a 20% cut-off, but **11 adult AML cases were 7.1-positive with germline MLL**, so the marker is *"sensitive but not entirely specific"* in the authors' own words [E2]. Schwartz 2003 measured it as a diagnostic test in 313 adult ALL: **sensitivity 0.89, specificity 0.89, PPV 0.93** against MLL rearrangement in B-precursor ALL, with 3 NG2-positive MLL-germline cases and **8 NG2-negative cases carrying MLL-AF4 transcripts** [E3]. Bisegna 2024 ran the mirror-image design on 1,382 newly diagnosed B-ALL: of 88 NG2-positive cases at a >10% threshold, **85 (96.6%) carried KMT2A-r**, 91% of them pro-B [E4]. **The honest single sentence: sensitivity 0.81-0.89 and positive predictive value 0.93-0.97 in B-lineage, with specificity that decays badly in myeloid and adult disease.** And the cut-off is not one number: ROC analysis on 505 NG2-positive paediatric cases put the optimum at **7% in infants with ALL, 12% in older children with ALL, 28% in infants with AML and 14% in older children with AML** [E5]. **NG2 is a direct target gene of the fusion, not a passive correlate.** Lopez-Millan 2024 showed NG2 is an **epigenetically regulated direct target gene of MLL-AF4**, that it negatively regulates the glucocorticoid receptor *NR3C1* through FLT3/AP-1, and that it **confers glucocorticoid resistance** [E6]. It sits downstream of the fusion's transcriptional programme - which is why it is a good surrogate, and why it should be expected to move when that programme is interrupted. **And it is malleable.** Prieto 2018 found NG2 does **not** enrich for leukaemia-initiating or CNS-initiating cells on serial xenotransplantation - it is *"a malleable marker"* - yet it is strongly upregulated on blasts infiltrating extramedullary sites and CNS, and NG2 blockade almost abolished engraftment [E7].
The name collision in this graph: two cells, one molecule
cell-type-ng2-periarteriolar-cell and this entry are the same gene product on two completely different cells.
- On the blast (this entry): NG2/CSPG4 is an aberrantly expressed surface antigen driven by the KMT2A fusion's own transcriptional programme, present on 81-89% of KMT2A-rearranged acute leukaemias, and used diagnostically.
- On the stroma (
cell-type-ng2-periarteriolar-cell): NG2/CSPG4 marks the pericytes wrapped around marrow arterioles that hold haematopoietic stem cells quiescent, and that are lost in MLL-AF9 AML.
A reader meeting the bare string "NG2" anywhere in this graph cannot currently tell which is meant. The disambiguation rule for this graph: gene-cspg4 is the molecule and the blast marker; cell-type-ng2-periarteriolar-cell is the niche cell. Where text says "NG2-positive blasts" it means the first; where it says "NG2+ pericyte" or "NG2+ periarteriolar" it means the second.
This is not pedantry, it is a therapeutic hazard. The niche entry already records it in its own data.naming_hazard: an NG2-directed therapeutic - a CSPG4 CAR-T, an anti-NG2 antibody - would engage both, hitting the quiescence-enforcing stromal compartment as well as the blast. Schwartz's argument that NG2 is negative in non-malignant haematopoietic cells and could therefore serve for MRD 3 is true of haematopoietic cells and says nothing about non-haematopoietic marrow stroma, which is where the pericyte lives. A safety claim bounded to one lineage is not a safety claim.
Does NG2 mark the rearrangement or the lineage state it produces?
Both, and the evidence separates cleanly.
It marks the rearrangement's output. NG2 is a direct, epigenetically regulated MLL-AF4 target gene 6. That is why the correlation with genotype is as tight as it is, and it is a stronger statement than "associated with": the fusion acts at the locus.
It is not a fixed property of the clone. Serial xenotransplantation shows NG2 is malleable and does not mark a stable initiating population; it is upregulated on blasts entering CNS and extramedullary sites 7. Steroid exposure before the flow sample can push it below threshold - Bisegna's two false-negative KMT2A-r cases were attributed to exactly that 4.
It is not exclusive to the genotype. In 967 paediatric AML samples, CSPG4 expression correlated with KMT2A-r but also occurred in patients without KMT2A-r, and had no significant prognostic effect on EFS or OS 8. That is the cleanest available demonstration that the marker tracks a lineage/differentiation state which a KMT2A rearrangement usually but not always produces.
The consequence, which is the point of the beat. If NG2 is a read-out of the fusion's transcriptional programme rather than of the DNA lesion, then interrupting that programme should move NG2 while the rearrangement stays exactly where it is. A menin inhibitor is the interruption this graph cares about, and that prediction is written as hyp-ng2-tracks-the-transcriptional-programme-not-the-rearrangement. Nobody has measured it: NG2[tiab] AND (menin[tiab] OR revumenib[tiab] OR SNDX-5613[tiab] OR VTP-50469[tiab]) returns 0 records and CSPG4[tiab] AND (menin[tiab] OR "menin inhibitor"[tiab]) returns 0 9.
Cell type, cell state, localisation, epigenetics
- Cell type. B-lineage: 57% of pro-B, 20% of pre-B, 2% of common ALL, and absent in T-ALL and mature B-ALL 3. Myeloid: 7.1-positive AML is myelomonocytic with CD56 co-expression in 33 of 38 cases 2.
- Cell state. Not a stable clonal property. Upregulated in the migratory/extramedullary state, suppressed by prior steroid 47.
- Localisation. Type I transmembrane proteoglycan at the plasma membrane; 2,322 residues with a very large ectodomain 10. No subnuclear address, which is worth saying because most gene entries in this graph are nuclear proteins and a reader scanning for that line should find the reason it is absent.
- Epigenetics. Direct, epigenetically regulated MLL-AF4 target 6. This is the molecular link between the graph's chromatin layer and its diagnostic layer.
- Function on the blast. Not inert: it interacts with FLT3 to give ligand-independent FLT3 signalling and downregulates NR3C1 via AP-1 transrepression, producing glucocorticoid resistance 6. It also carries a migratory/CNS-infiltration signature 7.

| name collision | NG2/CSPG4 is ALSO the marker of the periarteriolar pericyte in cell-type-ng2-periarteriolar-cell. gene-cspg4 = the molecule and the blast marker; cell-type-ng2-periarteriolar-cell = the niche cell. Same gene product, two cells, opposite therapeutic implications. |

| subnuclear localization | not applicable - CSPG4/NG2 is a single-pass plasma membrane proteoglycan with no reported subnuclear pool |
| shown for fusion | YES for the plasma-membrane compartment, and this is the only YES in this tranche. NG2 surface positivity is measured on unpermeabilised KMT2A-rearranged blasts by diagnostic flow cytometry (PMID 33135273, PMID 38468826), and NG2 is a direct transcriptional target of the KMT2A fusion (PMID 39093982). NO for any finer localisation: raft residence, polarity, internalisation rate and the membrane-bound-versus-shed ratio have not been measured in this disease. |
| epigenetic readout | NOT APPLICABLE AS AN ACTIVITY - AND NOT AN ABSENCE AS A LOCUS, which is the same two-part answer gene-clec12a reached for the same reason, and the reason is domain architecture rather than a failure to find a paper. WHY IT CANNOT WRITE, ERASE OR READ A MARK. UniProt Q6UVK1 (CSPG4_HUMAN), FETCHED LIVE 2026-09-21: 2,322 aa, a single-pass type I membrane protein with an EXTRACELLULAR topological domain at 30-2224, a helical TRANSMEMBRANE segment at 2225-2245 and a CYTOPLASMIC tail at 2246-2322. Its only annotated domains are Laminin G-like 1 at 30-192 and Laminin G-like 2 at 202-380, and its Pfam complement is exactly PF16184 (Cadherin_3) and PF02210 (Laminin_G_2). There is no SET, JmjC, HAT, HDAC, bromodomain, chromodomain, Tudor, PHD, CXXC, YTH, DNMT or MT-A70 module anywhere in the sequence, 96% of the protein is outside the cell, and no nuclear pool is reported. A surface proteoglycan with a 77-residue cytoplasmic tail has no mechanism by which to modify or read chromatin. WHAT IS NOT NOT-APPLICABLE, AND IT IS THIS ENTRY'S OWN HEADLINE. CSPG4/NG2 is a DIRECT TARGET GENE of the KMT2A fusion that makes it - this entry's title says so - and it is the single most informative immunophenotypic surrogate for an 11q23 rearrangement, detected by antibody 7.1. So the LOCUS is under fusion control and the chromatin state at that locus is a real, open question. What is missing is any measurement of it: CSPG4[tiab] AND H3K79[tiab] returns 0, NG2[tiab] AND H3K79[tiab] returns 0, and NG2[tiab] AND MLL[tiab] AND chromatin[tiab] returns 0. H3K79me2 is the mark this graph's DOT1L arm predicts at every direct KMT2A-fusion target, and the best flow-cytometry surrogate for the fusion is a locus at which nobody has measured it. THE DISCRIMINATING EXPERIMENT IS CHEAP AND THE ENTRY SHOULD NAME IT: H3K79me2 and menin CUT&RUN at CSPG4 in a KMT2A::AFF1 line, plus CSPG4 surface expression under a menin inhibitor. If the locus carries fusion-dependent H3K79me2 and loses CSPG4 on menin inhibition, then the diagnostic marker is a pharmacodynamic marker - and this graph already holds the cause-or-consequence warning for the analogous CLEC12A case, where menin inhibition induces the antigen and differentiation is an equally good explanation. A surface antigen that a drug moves is a flow-cytometry readout that could stop meaning what the diagnostic label says it means. |
| epigenetic readout rule91 audit | RULE 91 CHECK ON THE RECORD THIS VERDICT LEANS ON, AND IT WEAKENS ONE HALF OF IT. The epigenetic_readout above argues NOT-APPLICABLE partly from 'no nuclear pool is reported'. Fetched live 2026-09-21, UniProt Q6UVK1's SUBCELLULAR LOCATION comment has FOUR entries - Cell membrane, Apical cell membrane, Cell projection/lamellipodium membrane, Cell surface - and ALL FOUR carry ECO:0000250, by similarity, sourced to UniProtKB Q00657. Q00657 is CSPG4_RAT, the RAT orthologue (2,326 aa, Rattus norvegicus, verified live). SO THE HUMAN RECORD CONTAINS NO EXPERIMENTAL LOCALISATION ANNOTATION AT ALL - not a defective experimental citation, but the absence of one, with a non-human orthologue standing in. WHICH WAY THIS CUTS. It does NOT overturn the not-applicable, because the verdict rests primarily on TOPOLOGY - 2,195 extracellular residues, 20 transmembrane, 77 cytoplasmic, and two Laminin G-like domains - which is a sequence fact rather than a curator inference, and on the search bound in epigenetic_bounded_absence. But it DOES mean 'no nuclear pool is reported' must be read as 'UniProt annotates no nuclear pool, and UniProt annotates nothing experimentally for this protein's location in humans'. A rat-derived by-similarity call is the weakest form of support this graph accepts, and it is the second time in two cycles that the localisation half of a UniProt record has failed an audit here (cycle 52: 13 of 16 localisation records defective, including TRIM27 citing a nuclear-matrix paper for an endosome). REOPENING CONDITION, SHARPENED: any human experimental localisation for CSPG4 - immunofluorescence, fractionation or proteomics - that reports a nuclear or nucleoplasmic pool, or any report of a cleaved CSPG4 intracellular fragment, reopens the activity verdict. |

Four aliases were not searched.",
"reopening_condition": "The ACTIVITY claim reopens only if a nuclear pool or a chromatin-binding module is reported for CSPG4 - a proteolytic intracellular fragment reaching the nucleus would do it, which is not far-fetched for a shed surface proteoglycan and is worth watching. The LOCUS claim reopens on any H3K79me2, H3K4me3, menin or fusion occupancy track at CSPG4 in a KMT2A-rearranged sample, or any report of CSPG4 surface expression changing under a menin or DOT1L inhibitor. GSE127508 and the published KMT2A::AFF1 occupancy datasets already in this project's queue would answer it without new experiments."
}- An NG2/CSPG4-directed therapeutic would hit the marrow niche as well as the blast. `cell-type-ng2-periarteriolar-cell` is the NG2-positive pericyte that enforces HSC quiescence, and it is not haematopoietic - so the repeated claim that NG2 is absent from normal haematopoietic cells is true and does not bound the toxicity. The same laboratory that proposes NG2 blockade published on-target off-tumour myeloablation with a CD133 CAR in this disease.
- NG2 is a glucocorticoid-resistance effector, so lowering it is not obviously safe either. If NG2 suppresses NR3C1, an anti-NG2 agent would be expected to resensitise to steroid - but the same axis runs through FLT3, which this graph already carries as a cooperating lesion. Perturbing NG2 perturbs FLT3 signalling and the in vivo direction is not established.
- Using NG2 to triage who gets fusion testing will systematically miss t(4;11). Sensitivity is 0.81-0.89: roughly one in six to one in nine KMT2A-rearranged patients is NG2-negative, and eight MLL-AF4-positive patients were NG2-negative in a single adult series. Any protocol using NG2 to decide whether to run FISH must state what happens to those patients.
- The cut-off is a per-laboratory, per-lineage, per-age decision and it has already failed in the field. A multi-centre study found accuracy varying greatly per centre; ROC-optimal thresholds range from 7% to 28%. A published sensitivity from an expert laboratory is not the performance a trial network will achieve.
- If NG2 is a read-out of the fusion's transcriptional programme, a menin inhibitor may make it disappear while the leukaemia is still there. An MRD panel anchored on NG2 would then over-call remission, exactly as B-lineage panels do at lineage switch. Nobody has measured NG2 under menin inhibition.
- A shed ectodomain would act as an antigen sink. NG2 is cleaved from the surface by MMP-13 in other tissues (PMID 25166220) and the shed fraction has never been measured in KMT2A-rearranged B-ALL; circulating NG2 would absorb an anti-NG2 antibody or CAR before it reached a blast. One plasma ELISA on banked KMT2A-r samples would settle it.
- NG2 is a direct MLL-AF4 target gene (PMID 39093982), so a menin inhibitor or any fusion-directed agent is expected to LOWER surface NG2. Sequencing an NG2-directed modality after a menin inhibitor could therefore remove its own target, and the combination has not been tested against that prediction.
- CSPG4/NG2 is an active CAR-T and antibody target as well as a diagnostic marker, and this entry now records that its locus is under fusion control with the chromatin state unmeasured. If a menin or DOT1L inhibitor lowers CSPG4 - which is untested and plausible for a direct fusion target gene - then sequencing a menin inhibitor before a CSPG4-directed immunotherapy could remove the antigen the immunotherapy needs. The reverse of the CLEC12A case, where menin inhibition INDUCES the antigen, and neither direction has been measured.
- A diagnostic threshold set on untreated blasts does not transfer to treated ones. This entry carries ROC thresholds for NG2 positivity; if the antigen is fusion-transcription-dependent, those thresholds are properties of the untreated state and any post-treatment reassessment of KMT2A status by flow would be measuring the drug as much as the lesion.
- Does NG2 surface expression fall under menin inhibition while the KMT2A rearrangement persists? Zero published records intersect the two, and the answer decides whether NG2 can be used for MRD in the menin-inhibitor era.
- Is NG2 on the blast and NG2 on the periarteriolar pericyte the same epitope for antibody 7.1? Nobody has stained both with the same reagent, and the answer determines whether an NG2-directed therapeutic ablates the quiescence-enforcing niche.
- Why do NG2-negative KMT2A::AFF1 cases exist, and are they a biological subgroup or an assay artefact? Eight such cases sit in one adult series and two in a paediatric one, and steroid pre-treatment has been proposed but never tested prospectively.
- What drives CSPG4 expression in the KMT2A-germline paediatric AML cases? A fusion-independent route to the same surface protein exists and is uncharacterised.
- Does the order of NG2 loss relative to CD19 loss distinguish lineage switch from epitope loss early enough to act on?
- Does the CSPG4 locus carry fusion-dependent H3K79me2 in a KMT2A::AFF1 cell? CSPG4/NG2 is described in this entry as a direct target gene of the fusion and is the best immunophenotypic surrogate for it, and H3K79me2 has never been measured there - CSPG4 AND H3K79 returns 0 and NG2 AND H3K79 returns 0, against a control of 3 for CSPG4 AND KMT2A.
- Does CSPG4 surface expression fall under a menin or DOT1L inhibitor? If it does, the most-used diagnostic flow marker for 11q23 rearrangement is also a pharmacodynamic marker, and a treated patient's immunophenotype would stop meaning what the diagnostic label says - the same cause-or-consequence problem this graph records for menin-inhibitor-induced CLEC12A.
- Is there a nuclear CSPG4 fragment? The architecture argument for not-applicable rests on 77 cytoplasmic residues out of 2,322; a cleaved intracellular domain reaching the nucleus is the one route by which this verdict would have to be reopened, and nothing read here excludes it.

Monoclonal antibody 7.1, which recognises NG2, was used to screen 104 consecutive children at initial presentation with ALL; 9 cases (8.6%) were reactive, of whom 5 had t(4;11)(q21;q23) and 4 had t(11;19), and none of the NG2-negative cases had either translocation. MLL gene rearrangement was disclosed only in those 9 cases and was not detected in 89 NG2-negative samples, including 7 with del(11)(q23) or inv(11)(p13q23). NG2-positive cases were significantly more likely to be infants, to have hyperleukocytosis and CNS involvement, to be CD10-negative and to express the myeloid-associated antigens CD15 and CD65.

In 533 acute leukaemia samples from 215 adults and 318 children (271 AML, 217 B-lineage ALL, 37 T-lineage ALL, 8 CD7+CD56+ myeloid/natural killer cell precursor acute leukaemias), 58 of 72 (81%) samples with MLL rearrangements were monoclonal antibody 7.1-positive at a 20% cut-off, comprising 28 of 31 t(4;11), 16 of 17 t(9;11), 3 of 5 t(11;19) and 2 of 6 del(11)(q23). All 7.1-positive ALL (n=34) and childhood AML (n=17) cases had MLL rearrangements, but 11 adults with AML and one adult with 7.1-positive CD7+CD56+ myeloid/natural killer cell precursor acute leukaemia were MLL-germline by Southern blot. The authors conclude 7.1 is a sensitive but not entirely specific marker.

Evidence that argues against this (1)
- That the optimal cut-off differs fourfold between infant ALL (7%) and infant AML (28%) is itself evidence against reading NG2 as a direct marker of the DNA lesion: a marker of the rearrangement should not need a different threshold depending on which lineage the rearranged cell happens to occupy. NG2 is an epigenetically regulated direct target gene of the MLL-AF4 fusion protein. NG2 negatively regulates expression of the glucocorticoid receptor NR3C1 and confers glucocorticoid resistance to MLLr B-ALL cells; mechanistically NG2 interacts with FLT3 to produce ligand-independent activation of FLT3 signalling and downregulation of NR3C1 via AP-1-mediated transrepression. NG2 expression is described by the authors as a hallmark of MLLr B-ALL and as minimally expressed in healthy haematopoietic cells.

Evidence that argues against this (1)
- If NG2 were purely a fusion-driven target gene, CSPG4 expression should not occur without the rearrangement. In 346 paediatric AML samples tested for CSPG4, expression correlated with KMT2A-r but also occurred in KMT2A-germline patients, so at least one fusion-independent route to the same surface protein exists. In MLL-rearranged infant B-ALL, Interfant cohort data showed that high NG2 expression associates with lower event-free survival, a higher number of circulating blasts and more frequent CNS disease or relapse. Serial xenotransplantation of primary MLL-AF4-positive leukaemias indicated that NG2 is a malleable marker that does not enrich for leukaemia-initiating cells or CNS-infiltrating cells; however, NG2 expression was highly upregulated in blasts infiltrating extramedullary haematopoietic sites and the CNS, specific blockade of NG2 resulted in almost complete loss of engraftment, and gene expression profiling revealed a migratory signature in NG2-positive blasts.

**A bounded absence, not a claim that the experiment has not been done.** Three symbol forms (NG2, CSPG4, and the unbounded OR) and four drug forms were run; the antibody clone name `7.1` was NOT run because it is unusable as a search token, and `MCSP` was not run. A menin-inhibitor trial reporting a flow panel including NG2 in a supplementary table would be invisible to all of these. The repository was also grepped and no entry in this graph cites such a measurement.

This is a diagnostic-performance study, not a cell-biology study: it establishes that the epitope is on the outer leaflet of intact blasts and nothing finer. It cannot distinguish membrane-anchored from surface-adsorbed shed protein, cannot measure copy number per cell in absolute terms, and says nothing about polarity, raft residence or internalisation. Brief rule 74 applies - the antibody clone is the identifier for what was seen, and the clone is not recorded in this entry.
Evidence that argues against this (1)
- NG2/CSPG4 is shed from the surface by MMP-13 in other systems, so a fraction of the protein an anti-NG2 reagent can bind is not membrane-anchored. Rule 91 applies and cuts BOTH ways here: an absent feature annotation is weaker evidence than a present one, because UniProt annotates what curators have seen - so 'no chromatin domain is annotated' is not identical to 'no chromatin domain exists'. For this protein the topology carries the argument independently: 2,195 of 2,322 residues are extracellular and only 77 are cytoplasmic, and a chromatin activity would have to live in those 77.
