Is KMT2A::AFF1 leukemia epithelial or endothelial? No — it is haematopoietic. But the question is a good one
also called blood not epithelium · cell of origin KMT2A-AFF1 · haematopoietic vs epithelial vs endothelial · why the endothelial question is not silly · lineage of infant ALL
KMT2A::AFF1 leukemia is a cancer of blood cells, not of the skin-like cells that line organs (epithelium) or the cells that line blood vessels (endothelium). The question is still worth asking, because in the embryo blood cells really are born out of a vessel wall, and because the very same kinds of gene fusion do cause epithelial and connective-tissue cancers when they land in a different cell.
The same thing, for a scientist
KMT2A::AFF1 (MLL-AF4) B-ALL is a haematopoietic malignancy: the malignant compartment is a CD19+ pro-B/pro-B-like blast, and the disease is classified, staged and treated as a lymphoid leukemia. The confusion is understandable and partly legitimate. Definitive haematopoiesis arises by an endothelial-to-haematopoietic transition from haemogenic endothelium in the dorsal aorta, demonstrated by live imaging and by conditional Runx1 deletion in VE-cadherin+ endothelium [E3][E4], and the earliest blood and endothelial cells share CD34, CDH5/VE-cadherin, TEK/TIE2 and RUNX1. In human embryonic models, MLL-AF4 does not behave lineage-neutrally at that stage: it enhances specification of hemogenic precursors while skewing their further maturation toward an endothelial fate [E2], and co-expression with the reciprocal AF4-MLL strongly promotes emergence of both endothelial- and hemogenic-primed hemato-endothelial precursors [E1]. Separately, the fusion-class argument is decisive against lineage determinism: KMT2A::MAML2 is a recurrent fusion in type B2/B3 thymoma, an epithelial neoplasm [E5]; ALK fusions occur in lymphoma [E6], lung adenocarcinoma [E7] and inflammatory myofibroblastic tumour [E8]; ETV6::NTRK3 occurs in infantile fibrosarcoma [E9] and in secretory breast carcinoma, where it transforms mammary epithelium into gland-forming, epithelial-antigen-positive tumours [E10]; NTRK1 rearrangements occur in papillary thyroid carcinoma [E11]. The lineage is set by the cell that receives the fusion, not by the fusion.
The short answer, for a newcomer
KMT2A::AFF1 leukemia is haematopoietic — it is a blood cancer.
The cell that goes wrong is a very early blood cell. In infant t(4;11) disease the blasts carry B-lymphoid markers (CD19, CD10-low/negative, often co-expressing myeloid antigens such as CD15/CD65), and the disease sits in bone marrow, blood, spleen, liver and central nervous system — the places blood cells travel to. It is not a carcinoma (epithelial) and not an angiosarcoma or haemangioendothelioma (endothelial).
So why did the question come up? Three reasons, all of them real, and each is a section below:
- Development. Blood genuinely comes out of a vessel wall in the embryo. There is a cell type called haemogenic endothelium that is an endothelial cell by appearance and marker profile and that turns into a blood stem cell. So an "endothelial precursor of leukemia" is not a crank idea; it is a real developmental stage — see
cell-type-haemogenic-endothelium. - Markers overlap. CD34, VE-cadherin (CDH5), TIE2 (TEK) and RUNX1 are shared between early blood and early endothelium, so flow-cytometry or immunohistochemistry panels that were designed for one lineage light up in the other.
- The fusions themselves are lineage-agnostic. KMT2A, ALK and NTRK fusions all occur outside blood. Same fusion class, completely different tumour, because it landed in a different cell.
Why the endothelial question is not silly: haemogenic endothelium and the EHT2 deeper
In the mammalian embryo, the first true haematopoietic stem cells appear in the wall of the dorsal aorta in the aorta-gonad-mesonephros region. They do not migrate in from somewhere else; they are made there, out of the endothelium.
The decisive experiments:
- Direct visualisation. Time-lapse confocal imaging of the intact mouse embryonic aorta captured the de novo emergence of phenotypically defined HSCs (Sca1+, c-Kit+, CD41+) directly from ventral aortic haemogenic endothelial cells 3. Before this, the haemogenic-endothelium model rested on retrospective lineage tracing and in vitro ESC work; after it, the transition is something people have watched happen.
- Genetic requirement, in the endothelium. Conditional deletion showed that RUNX1 activity in VE-cadherin-positive endothelial cells is essential for intra-arterial cluster, haematopoietic progenitor and HSC formation, whereas RUNX1 is not required in cells already expressing the pan-haematopoietic gene Vav1 4. The requirement is precisely at the endothelium-to-blood step and ends once the cell is committed blood.
The process even has an ontology term of its own: GO:0098508, "endothelial to hematopoietic transition" — "the generation of hematopoietic stem cells from hemogenic endothelial cells by a process that includes tight-junction dissolution and loss of cell polarity followed by delamination from the endothelium."
So what? If a leukemic fusion is acquired in utero — and KMT2A::AFF1 is, from concordant-twin and neonatal-blood-spot work — then there is a window in which the cell holding it genuinely has an endothelial-like identity. Asking whether infant ALL has an endothelial-flavoured origin is asking a developmental-timing question, not a pathology-classification error.
And there is real data on MLL-AF4 at that exact stage
This is not hand-waving. Two studies from the Menendez/Bueno group put MLL-AF4 into human embryonic stem cells and watched the hemato-endothelial branch point.
- MLL-AF4 alone acts as a global transcriptional activator and a positive regulator of homeobox genes in hESCs, and functionally it enhances the specification of hemogenic precursors but strongly impairs further haematopoietic commitment in favour of an endothelial cell fate. The MLL-AF4 cells are transcriptionally primed toward hemogenic precursors prone to endothelial maturation, with upregulation of vascular-endothelial master genes. Importantly, MLL-AF4 expression was not sufficient to transform hESC-derived haematopoietic cells 2.
- MLL-AF4 plus the reciprocal AF4-MLL (the fusion expressed in only about half of t(4;11) patients) strongly promotes the emergence of hemato-endothelial precursors, both endothelial- and hemogenic-primed, and the double-fusion precursors go on to make significantly more of both haematopoietic and endothelial committed cells, without simply proliferating or surviving better. H3K79me3 ChIP-seq showed enrichment specifically over the HOXA cluster in the differentiating double-fusion cells 1.
The honest reading: MLL-AF4 measurably perturbs the hemato-endothelial branch point in human embryonic differentiation. That is an effect on a bipotent precursor, not evidence that the leukemia is endothelial. The leukemia that patients get is B-lymphoid. But the fusion is demonstrably active in a cell that has not yet chosen between blood and vessel — and that is the lab-relevant version of the PI's question.
The markers that cause the confusion
Early blood and early endothelium are not just developmentally adjacent, they are phenotypically overlapping:
| Marker | Endothelium | Early haematopoietic |
|---|---|---|
| CD34 | yes (pan-endothelial) | yes (HSPC standard) |
| CDH5 / VE-cadherin | yes (defining) | yes, on haemogenic endothelium and transitional cells 4 |
| TEK / TIE2 | yes (angiopoietin receptor) | yes, on HSCs |
| RUNX1 | only in haemogenic endothelium | yes (master haematopoietic TF) 4 |
| PTPRC / CD45 | no | yes, acquired late |
| SPN / CD43 | no | yes, acquired at the transition |
The two markers that actually separate them are the last two. CD43 (leukosialin/SPN) is detected on all emerging clonogenic haematopoietic progenitors before CD45 and reliably separates the haematopoietic CD34+ population from CD34+CD43-CD31+KDR+ endothelial cells in human differentiation cultures 12. This is why a CD34+ panel alone can never answer "blood or vessel?", and why the historical "endothelial precursor" literature was so easy to misread.
The killer argument: the same fusion class shows up in epithelium, mesenchyme and blood1 deeper
If lineage were dictated by the fusion, each fusion would give one tumour type. It does not. This is the cleanest available demonstration that cell of origin is the variable, not the oncogene.
KMT2A itself is not restricted to blood. A pan-cancer analysis of 255,008 sequenced advanced cancers, including 242 thymomas, identified a recurrent KMT2A::MAML2 fusion in 11 thymoma patients — breakpoints between KMT2A exon 8, 9, 10 or 11 and MAML2 exon 2 — at a frequency of 4% of all thymomas and 6% of B2/B3-histology thymomas, specifically in the most aggressive type B2 and B3 subtypes, one with foci of thymic carcinoma 5. Thymoma is a thymic epithelial neoplasm. So a KMT2A fusion, with a KMT2A N-terminal breakpoint in the same intronic region that generates leukemic fusions, produces an epithelial tumour when it occurs in a thymic epithelial cell.
ALK — one kinase, three lineages.
- Haematopoietic/lymphoid: the t(2;5) fuses the NPM nucleolar phosphoprotein gene to a then-new tyrosine kinase, ALK, in anaplastic large-cell non-Hodgkin lymphoma arising from activated T lymphocytes 6.
- Epithelial: a small inversion in chromosome 2p creates EML4-ALK in non-small-cell lung cancer — a lung adenocarcinoma, i.e. an epithelial carcinoma — detected in 6.7% (5/75) of NSCLC patients in the discovery cohort, mutually exclusive with EGFR mutation 7.
- Mesenchymal: clonal 2p23 rearrangements involving ALK, with ALK protein expression, recur in inflammatory myofibroblastic tumour, a soft-tissue (myofibroblastic) tumour — the authors explicitly noted "ALK may have a novel role outside its previously recognized realm of lymphoid neoplasms" 8.
NTRK — mesenchymal, epithelial, and thyroid.
- Mesenchymal: t(12;15)(p13;q25) fusing ETV6 to NTRK3 in congenital/infantile fibrosarcoma, a malignant tumour of fibroblasts, absent from the histologically similar adult-type fibrosarcoma 9.
- Epithelial — and this one is mechanistically decisive: the identical ETV6-NTRK3 fusion is expressed in 12 of 13 (92%) human secretory breast carcinomas, a subtype of infiltrating ductal carcinoma. Retroviral transfer of ETV6-NTRK3 into murine mammary epithelial cells produced transformed cells that formed tumours in nude mice, and those tumours produced glands and expressed epithelial antigens, confirming that ETV6-NTRK3 transformation is compatible with epithelial differentiation 10. The same fusion, put in an epithelial cell, makes an epithelial cancer.
- Thyroid: chromosome 1 rearrangements juxtaposing TPR and the NTRK1 tyrosine kinase domain generate thyroid-specific TRK oncogenes (TRK-T1, TRK-T2) recurrently activated in papillary thyroid carcinoma 11.
The so-what. The oncogene supplies a program — sustained kinase signalling for ALK/NTRK, aberrant transcriptional elongation and HOXA activation for KMT2A. The cell supplies the enhancer landscape, the accessible chromatin, the available transcription-factor partners, and therefore the tumour's identity and its vulnerabilities. That is exactly why the lab cares which cell KMT2A::AFF1 lands in: it determines what the fusion can read, and therefore what it can do, and therefore what will kill it.
The honest caveat on the KMT2A solid-tumour point
Do not oversell this. KMT2A rearrangements are overwhelmingly a haematopoietic phenomenon; the thymoma finding 5 is a genuine but rare and recently described exception, and thymic epithelium is an unusual tissue that sits in an immunological organ. This node deliberately does not claim the broader statement that KMT2A fusions are common in sarcoma or other solid tumours; searches for a verifiable recurrent KMT2A fusion in sarcoma returned nothing citable, so it is not asserted. The ALK and NTRK examples carry the argument, and they carry it fully.
Putting it together: what the PI should take away
- Classification: KMT2A::AFF1 disease is a haematopoietic (lymphoid) leukemia. Nothing in the literature makes it epithelial or endothelial.
- Development: the precursor of blood is endothelial-like, and that stage is real, named, imaged and genetically defined 34. A prenatally acquired fusion can in principle be present in it.
- Evidence at that stage: MLL-AF4 is not inert in the hemato-endothelial precursor. It expands hemogenic precursors while skewing their maturation toward endothelium 2, and with its reciprocal partner it expands both endothelial- and hemogenic-primed precursors 1. That is a lineage-plasticity effect in a bipotent cell, and it is the most concrete published link between this fusion and endothelium.
- Not sufficient: MLL-AF4 alone did not transform hESC-derived haematopoietic cells 2. Something else — cellular context, a second hit, the right developmental window — is required. That is the real experimental problem, and it is why cell-of-origin nodes matter more than they look.
- Lineage is contextual: KMT2A::MAML2 in epithelial thymoma 5, ALK in lymphoma/lung/soft tissue 678, ETV6::NTRK3 in fibrosarcoma and in gland-forming epithelial breast carcinoma 910, NTRK1 in papillary thyroid carcinoma 11. The fusion is the verb; the cell is the subject.

]- If lineage is set by the recipient cell rather than the fusion, then models built in the wrong cell (hESC, 3T3, HEK293) can generate confident mechanistic conclusions that do not transfer to a pro-B blast — and much of the MLL-AF4 literature is exactly that.
- Lineage plasticity is a clinical bite-back, not just a conceptual one: KMT2A-rearranged leukemia is notorious for lineage switch under pressure (B-ALL to AML after blinatumomab or CAR-T). A fusion that sits above the lineage decision can escape a lineage-directed therapy by changing lineage.
- The menin dependency may be lineage-contingent rather than fusion-contingent. If so, a menin inhibitor could fail in a non-haematopoietic KMT2A-fusion tumour even though the interface is intact — and, more worryingly, could fail in leukemic cells that have shifted lineage.
- The 'endothelial precursor' framing is seductive and largely unsupported for the actual disease. Building a programme on it risks chasing a developmental analogy instead of the blast that kills the patient.
- Is KMT2A::AFF1 ever present in the endothelial progeny of a fusion-bearing hemato-endothelial precursor in a patient? Nobody has looked for the fusion in non-haematopoietic tissue — endothelium, vascular biopsy, or single-cell data from a t(4;11) infant. A negative would close the question; a positive would reframe the disease.
- Why does MLL-AF4 skew hESC hemato-endothelial precursors toward endothelium in vitro [E2] when the disease that results is B-lymphoid? Is the endothelial skew an artefact of constitutive expression, or a real early effect that is later overridden?
- Does the thymic epithelial KMT2A::MAML2 thymoma [E5] respond to menin inhibition? If the menin-MLL interface is retained (KMT2A exon 8-11 breakpoints suggest it is), this is a direct test of whether the menin dependency travels with the fusion across lineages — and it would be a genuinely new indication.
- What makes a cell permissive for a given fusion? For ETV6::NTRK3 the same fusion works in fibroblast and mammary epithelium [E9][E10]; for KMT2A::AFF1 no non-haematopoietic tumour has been reported. Is that biology, or is it just that the translocation only happens in cells with high KMT2A/AFF1 transcription?
- Which lineage-specific cofactors does KMT2A::AFF1 require? If it is the B-lineage enhancer landscape that makes the fusion oncogenic, those cofactors are targets that a solid-tumour KMT2A fusion would not share.

Co-expression of MLL-AF4 and the reciprocal AF4-MLL fusion in human embryonic stem cell hematopoietic differentiation strongly promotes the emergence of hemato-endothelial precursors, both endothelial- and hemogenic-primed, which specify into significantly higher numbers of both hematopoietic and endothelial committed cells without changes in survival or proliferation, with H3K79me3 enrichment specifically over HOXA cluster genes.

An hESC-based in vitro differentiation model with enforced (lentiviral/transgenic) fusion expression, which is a very imperfect proxy for prenatal human haematopoiesis: expression level, timing and cellular context are all artificial. The readout is precursor emergence and lineage output, not leukemia; no leukemic transformation is reported. The reciprocal AF4-MLL fusion is expressed in only about half of t(4;11) patients, so the cooperativity described applies to a patient subset at best.

Ectopic constitutive expression in hESCs, a system with no immune microenvironment, no niche and a differentiation trajectory that only approximates human embryonic haematopoiesis. The endothelial skewing is a differentiation-output phenotype in vitro and has never been recapitulated in a human embryo or in a patient sample. The negative result (no transformation) is the most robust part of this paper and is the part most often left out when it is cited.
Evidence that argues against this (2)
- Explaining lineage or cell-type specificity by the fusion's DNA-binding module is not supported. Replacing the MLL CXXC domain in MLL-AF9 with CXXC or MBD domains that cannot bind the same DNA still targeted the fusion to the Hoxa9 locus in primary bone-marrow progenitors; what differed was whether specific CpGs were protected from methylation, and hence whether leukaemia developed. Locus addressing and leukaemogenic function separate, so an endothelial-versus-haematopoietic argument built on where the fusion can bind is arguing from the wrong property. Read: abstract only. The two classes are not evidence about each other, and using the kinase-fusion literature to argue about tissue restriction of KMT2A fusions crosses that boundary. The EML4-ALK transformation assay is also 3T3 fibroblasts and nude mice, a system in which many oncogenes transform. Nature 2012

Clonal chromosome aberrations involving 2p23 with rearrangement of the ALK locus and ALK protein expression recur in inflammatory myofibroblastic tumour, a soft-tissue myofibroblastic neoplasm, indicating a role for ALK outside lymphoid neoplasms.
Recurrent involvement of 2p23 in inflammatory myofibroblastic tumors · Griffin CA, Hawkins AL, Dvorak C, Henkle C, Ellingham T, Perlman EJ · Cancer Research 1999What this evidence cannot show
Three cases with FISH and immunohistochemistry plus four previously reported cases with 2p22-24 involvement — a very small series, and the specific 5' fusion partners (later shown to include TPM3, TPM4, CLTC and others) were not identified here. 'Recurrent' rests on a handful of cases.

A recurrent t(12;15)(p13;q25) in congenital/infantile fibrosarcoma fuses ETV6 (TEL) at 12p13 to the NTRK3 neurotrophin-3 receptor gene at 15q25, producing a chimera of the ETV6 helix-loop-helix dimerisation domain and the NTRK3 tyrosine kinase domain; transcripts were found in all three fibrosarcomas analysed and were absent from adult-type fibrosarcoma and infantile fibromatosis.
A novel ETV6-NTRK3 gene fusion in congenital fibrosarcoma · Knezevich SR, McFadden DE, Tao W, Lim JF, Sorensen PHB · Nature Genetics 1998What this evidence cannot show
Three tumours in the original series; frequency and specificity were established by later work. The paper proposes rather than demonstrates a transforming mechanism.

The same ETV6-NTRK3 fusion found in paediatric mesenchymal cancers is expressed in 12 of 13 (92%) human secretory breast carcinomas but not in other ductal carcinomas, and retroviral transfer of ETV6-NTRK3 into murine mammary epithelial cells produced transformed cells that formed tumours in nude mice which produced glands and expressed epithelial antigens, confirming that ETV6-NTRK3 transformation is compatible with epithelial differentiation.

Secretory breast carcinoma is rare (13 cases here), so this is the strongest available series but still small. The mouse mammary epithelial transformation used retroviral overexpression, which does not reproduce endogenous fusion stoichiometry. This is the single most useful citation in this node for the cell-of-origin argument, because the identical fusion is shown to produce both a mesenchymal and an epithelial tumour.

The breast result is the strongest form of the entry's argument and the weakest analogy for it: a kinase fusion crossing from mesenchymal to epithelial tissue is expected, because catalysis does not require a lineage-specific environment. Chromosome 1 rearrangements juxtaposing TPR with the NTRK1 tyrosine kinase domain generate structurally distinct thyroid-specific TRK oncogenes (TRK-T1 and TRK-T2) that are recurrently activated in papillary thyroid carcinoma, alongside rearrangements with tropomyosin and with TFG on chromosome 3.

Molecular characterisation of a small number of tumours; NTRK1 rearrangement is a minority event in papillary thyroid carcinoma (RET/PTC and BRAF V600E dominate). No DOI is indexed for this 1997 article; the record was verified by PMID.

In human embryonic stem cell differentiation cultures, CD43 (leukosialin/SPN) is detected on all emerging clonogenic haematopoietic progenitors before CD45 appears and reliably separates the haematopoietic CD34+ population from CD34+CD43-CD31+KDR+ endothelial cells and CD34+CD43-CD31-KDR- mesenchymal cells.
Leukosialin (CD43) defines hematopoietic progenitors in human embryonic stem cell differentiation cultures · Vodyanik MA, Thomson JA, Slukvin II · Blood 2006What this evidence cannot show
An in vitro hESC/OP9 co-culture system; marker boundaries defined there do not automatically transfer to the human embryo, and CD43 is not used this way in routine clinical flow cytometry. Useful for interpreting hemato-endothelial experiments, not for diagnosing a patient's leukemia.

- CD43 separating haematopoietic from endothelial and mesenchymal populations was established in human embryonic stem cell differentiation cultures, so it is a marker validated in a model of the transition. The separation it achieves is operational for that culture and does not establish a sharp lineage boundary in an embryo. Oncofusion knowledge graph (internal analysis) 2026
