Nestin+ mesenchymal stem cell — the niche cell that MLL-AF9 leukaemia wakes up and then loses

also called Nestin-GFP+ MSC · nestin-positive mesenchymal stem and progenitor cell · Nes+ MSPC

These are the marrow's own stem cells for bone and fat, sitting right next to blood stem cells and next to nerve fibres, and they release the signals that keep blood stem cells healthy. Leukaemia disturbs them: it drives them out of their resting state and pushes them towards becoming bone-forming cells instead of blood-supporting ones.

The same thing, for a scientist

Nestin-GFP-positive mesenchymal stem cells were defined as an essential HSC niche component: they contain all marrow CFU-fibroblast activity, can be propagated as self-renewing mesenspheres, are spatially associated with both HSCs and adrenergic nerve fibres, highly express HSC-maintenance genes that are selectively downregulated on enforced mobilization or beta-3 adrenoreceptor activation, and their in vivo depletion rapidly reduces marrow HSC content and progenitor homing [E1]. The KMT2A-relevant finding is that **MLL-AF9 AML abolishes the quiescence of Nestin+ niche cells** and expands phenotypic MSPCs primed for osteoblastic differentiation at the expense of HSC-maintaining NG2+ periarteriolar cells, with the leukaemogenic adrenergic signal running through the **beta-2** receptor on stromal cells [E2]. A later study of the same driver is the most consequential: in MLL-AF9 AML, **nestin+ BMSC numbers are not reduced** (unlike bulk stroma) but their **function switches** to supporting AML at the expense of non-mutated HSCs; **nestin+ cell depletion delays leukaemogenesis and selectively decreases AML but not normal cells**; and the support runs through increased oxidative phosphorylation, TCA cycle activity and glutathione-mediated antioxidant defence [E3]. That makes this the one niche cell in this beat whose removal is reported to hurt the leukaemia more than the host. The identity of the population is contested — conditional Scf deletion from Nestin-cre cells does not affect HSCs [E4] — and this entry does not resolve that dispute.

What the cell is, and the dispute that will not go away

Nestin is an intermediate filament, originally a neural stem-cell marker. Méndez-Ferrer and colleagues found that a Nestin-GFP reporter labels a rare perivascular mesenchymal population in marrow that carries all the colony-forming fibroblast activity, self-renews in serial transplantation as 'mesenspheres', and sits physically beside both HSCs and sympathetic nerve fibres. Depleting them in vivo drops marrow HSC content quickly 1.

The unresolved problem: conditional deletion of Scf from Nestin-cre cells had no effect on HSCs, while deletion from LEPR+ perivascular cells did 4. The usual explanation is that Nestin-GFP and Nestin-cre label different cells — the reporter labelling a periarteriolar-enriched subset and the cre recombining more broadly or not at all in the relevant population. That explanation is plausible and unproven. This entry therefore treats 'Nestin+ MSC' as a reporter-defined population with demonstrated depletion phenotypes, not as a molecularly defined cell type, and flags anywhere a conclusion depends on which reading is correct.

What the leukaemia does to it

In MLL-AF9 AML the Nestin+ compartment does not simply shrink. It is activated out of quiescence, and the mesenchymal compartment expands towards an osteoblast-primed phenotype while the NG2+ periarteriolar cells that maintain HSC quiescence are lost 2. The healthy niche and the leukaemic niche are therefore different niches, not the same niche in different amounts — which is the point the authors make explicitly, proposing 'separate mesenchymal niche activities for malignant and healthy hematopoietic stem cells'.

A practical corollary the graph should not lose: in leukaemic marrow, the HSC-supporting compartment is depleted while the leukaemia-supporting compartment is expanded. That is a candidate explanation for why the normal haematopoiesis of a child with KMT2A-rearranged leukaemia recovers poorly even at low blast burden, and it is measurable in a marrow biopsy.

The functional experiment matters more than the descriptive one. Forte and colleagues report that in MLL-AF9 AML, nestin+ BMSC numbers are not reduced while bulk stroma is, and that what changes is their function: they begin supporting AML cells at the expense of non-mutated HSCs. Depleting nestin+ cells delays leukaemogenesis and, in chimeric mice, selectively decreases AML cells and not normal ones; the mechanism is increased oxidative phosphorylation, TCA cycle activity and glutathione-mediated antioxidant defence transferred from stroma to blast 3.

Hold that beside the Ebf2+ result: deleting Ebf2+ MSPCs accelerated AML [see mechanism-leukaemia-remodels-its-own-niche E3], while deleting nestin+ BMSCs delayed it. Two mesenchymal depletions in the same driver, opposite directions. This is the single most important caution in this beat: 'the stroma' is not one thing and does not have one sign. Any proposal to target the mesenchymal niche in KMT2A-rearranged disease has to name which subset, and the two subsets that have been tested disagree.

The metabolic theme is consistent across three independent studies — Prx1+ CXCL12 controlling LSC redox balance (cell-type-car-cell), nestin+ BMSCs supplying OXPHOS and glutathione 3, and marrow leptin driving complex-I activity (cell-type-bone-marrow-adipocyte). The mesenchymal niche's contribution to leukaemic survival in this genotype is repeatedly metabolic rather than a simple anti-apoptotic signal.

How this could bite us back
  • Restoring the Nestin+ niche would restore a compartment that supports normal HSCs, but the same compartment has been shown to support AML bioenergetics and chemotherapy escape, so the intervention could cut both ways in a patient who still has disease.
  • Beta-adrenergic manipulation in an infant carries cardiovascular consequences that are not acceptable for a speculative niche indication.
  • The Ebf2+ mesenchymal deletion result shows that removing mesenchymal cells can accelerate leukaemia, so 'target the stroma' is not a safe default direction.

  1. Is the Nestin-GFP population the same as the LEPR+/CAR population? Every functional statement in this entry is conditional on the answer and the field has been arguing about cre drivers instead of settling it.
  2. Does the osteoblast-primed MSPC expansion seen in MLL-AF9 AML occur in KMT2A::AFF1 infant B-ALL, and is it visible in a diagnostic human trephine as increased osteoblastic lining or altered CD271+ stroma?
  3. If leukaemic marrow has fewer HSC-supporting niche cells, does that explain the slow count recovery seen in these children, and could it be a measurable marker of niche damage independent of blast burden?
  4. Is mirabegron, a licensed beta-3 agonist reported to restore Nestin+ cells in myeloproliferative neoplasm patients, of any use here, given that the leukaemogenic signal in MLL-AF9 runs through beta-2 rather than beta-3?

Mesenchymal stem cells identified by Nestin expression constitute an essential HSC niche component: they contain all bone-marrow CFU-fibroblastic activity, propagate as self-renewing mesenspheres, are spatially associated with HSCs and adrenergic nerve fibres, highly express HSC-maintenance genes that are selectively downregulated during enforced mobilization or beta-3 adrenoreceptor activation, and their in vivo depletion rapidly reduces marrow HSC content and progenitor homing; parathormone doubles bone-marrow Nestin+ cell number and favours their osteoblastic differentiation.

In MLL-AF9-driven AML in vivo, nestin-positive bone-marrow mesenchymal stem cell numbers are NOT reduced (unlike bulk stroma) but their function changes to support AML cells at the expense of non-mutated HSCs; nestin+ cell depletion delays leukaemogenesis in primary AML mice and selectively decreases AML but not normal cells in chimeric mice; and nestin+ BMSCs support AML survival and chemotherapy relapse through increased oxidative phosphorylation, TCA cycle activity and glutathione-mediated antioxidant defence, so AML cells co-opt energy sources and antioxidant defences from the stroma.
