Bone-marrow adipocyte — the fat cell that suppresses normal blood formation and feeds leukaemia a chemoresistance signal

also called marrow adipocyte · marrow adipose tissue · MAT · bone marrow adipose tissue

Bone marrow contains fat cells, and the more fat a region of marrow holds, the less blood it makes — fatty replacement of marrow is what a pathologist sees after chemotherapy or in marrow failure. Recent work shows these fat cells also release leptin, a hormone that makes leukaemia cells harder to kill with chemotherapy.

The same thing, for a scientist

Marrow adipocyte number correlates inversely with haematopoietic activity, and the relationship is causal: HSCs and short-term progenitors are reduced in the adipocyte-rich tail vertebrae relative to the adipocyte-free thoracic vertebrae of the mouse, and marrow engraftment after irradiation is **accelerated** in lipoatrophic A-ZIP/F1 'fatless' mice and in mice treated with the PPAR-gamma inhibitor bisphenol A diglycidyl ether, implicating adipocytes as **predominantly negative** regulators of the marrow microenvironment [E1]. The KMT2A-relevant finding is 2026 and specific: elevated marrow leptin and blast LEPR strongly associate with poor cytarabine clearance and reduced survival in newly diagnosed AML; in human AML lines, primary blasts and two syngeneic mouse models including **MLL-AF9**, exogenous leptin blunts cytarabine cytotoxicity while the high-affinity LEPR antagonist Allo-aca restores chemosensitivity without altering baseline growth; mechanistically leptin upregulates LEPR and triggers JAK2/STAT3 signalling that raises mitochondrial complex I activity, oxidative phosphorylation and mitochondrial ROS, and the mtROS surge activates a compensatory antioxidant programme that shields blasts from drug-induced oxidative damage [E2]. Note that LEPR is the same marker that defines the principal perivascular niche stromal cell (`cell-type-car-cell`), of which the adipocyte is a differentiated descendant.

Fat in the marrow is not inert filler

Anyone who has looked at a marrow biopsy after induction chemotherapy has seen fatty replacement: the cellular marrow is gone and adipocytes fill the space. The question Naveiras and colleagues asked was whether the fat is a consequence or a cause. They compared mouse skeletal regions that differ naturally in adiposity — the adipocyte-rich tail vertebrae against the adipocyte-free thoracic vertebrae — and found HSCs and short-term progenitors reduced in the fatty ones. Then they removed the variable: in genetically fatless A-ZIP/F1 mice, and in mice given a PPAR-gamma inhibitor to block adipogenesis, marrow engraftment after irradiation was accelerated 1.

So marrow fat actively restrains haematopoiesis, and the clinical inference the authors draw — that antagonising marrow adipogenesis might speed engraftment after transplant — is still, nearly two decades later, untested in children.

The leptin circuit, and the third appearance of a metabolic niche mechanism

The 2026 finding is the first adipocyte result with direct KMT2A relevance. Leptin is abundant in marrow adipose tissue. In newly diagnosed AML, high marrow leptin and high blast LEPR associate with poor cytarabine clearance and shorter survival. Causally: leptin blunts cytarabine cytotoxicity in human lines, primary blasts and two syngeneic mouse models, one of which is MLL-AF9; the LEPR antagonist Allo-aca restores chemosensitivity without changing baseline leukaemia growth — the signature of a genuine resistance mechanism rather than a growth effect. The mechanism is metabolic: LEPR/JAK2/STAT3 raises complex I activity and oxidative phosphorylation, the resulting mitochondrial ROS surge triggers a compensatory antioxidant programme, and that programme shields blasts from drug-induced oxidative damage 2.

This is now the third independent niche compartment in this beat whose protection of KMT2A-rearranged leukaemia is metabolic and redox-mediated: Prx1+ mesenchymal CXCL12 controlling LSC redox balance (cell-type-car-cell), nestin+ BMSCs transferring OXPHOS capacity and glutathione (cell-type-nestin-msc), and now adipocyte leptin driving complex I and an antioxidant response. Three different cells, three different ligands, one convergent mechanism. That convergence is the single most generalisable finding of this beat, and it makes the antioxidant/redox axis rather than the adhesion axis the place to look for a niche-directed intervention in this genotype.

It also sets up an uncomfortable prediction. If niche protection works by buffering oxidative stress, then agents that reduce oxidative stress — including some supportive-care measures — would be expected to phenocopy the niche and protect the leukaemia. Nobody appears to have tested that.

How this could bite us back
  • Reducing marrow adiposity would speed normal engraftment but the same manipulation removes a compartment whose relationship to leukaemic cells is supportive in one direction and suppressive in another; the net effect in a patient with residual disease is unknown.
  • Leptin is a systemic hormone central to appetite and growth. Antagonising LEPR in an infant with a cancer that already causes failure to thrive is a serious proposition, not a niche experiment.
  • If niche protection is oxidative-stress buffering, then any co-administered agent that reduces reactive oxygen species could phenocopy the niche and undo a chemotherapy dose.

  1. Infants have almost no marrow adipose tissue, which appears and expands through childhood. Does that make the leptin mechanism irrelevant in infant KMT2A-rearranged ALL, and could it contribute to why marrow biology differs so sharply between infant and older-child disease?
  2. Does LEPR antagonism improve cytarabine efficacy in a KMT2A-rearranged patient-derived xenograft, separated from the AML1-ETO9a arm that the source paper reports alongside it?
  3. If three independent niche compartments protect this leukaemia by buffering oxidative stress, do antioxidant supportive-care measures protect the leukaemia too? Nobody appears to have asked.
  4. Chemotherapy causes fatty marrow replacement. Does the marrow that leukaemia relapses into therefore contain more leptin than the marrow it was diagnosed in, making the second course of the same drug less effective for a niche reason?

Bone-marrow adipocytes are predominantly negative regulators of the haematopoietic microenvironment: HSCs and short-term progenitors are reduced in frequency in the adipocyte-rich tail vertebrae of the mouse relative to adipocyte-free thoracic vertebrae by flow cytometry, colony-forming activity and competitive repopulation, and marrow engraftment after irradiation is accelerated in lipoatrophic A-ZIP/F1 fatless mice and in mice treated with the PPAR-gamma inhibitor bisphenol A diglycidyl ether.

    What this evidence cannot show

Mouse. Tail and thoracic vertebrae differ in temperature, mechanical loading and vascular supply as well as in adiposity, so the regional comparison is confounded; the genetic and pharmacological arms are the load-bearing evidence. A-ZIP/F1 mice lack fat systemically, with metabolic consequences well beyond the marrow. BADGE has off-target activity. Read: abstract only.

Evidence that argues against this (1)
    • The adipocyte's effect on leukaemia is not the same as its effect on normal haematopoiesis, and in leukaemia the direction is supportive rather than suppressive: marrow leptin from adipose tissue blunts cytarabine cytotoxicity in AML including an MLL-AF9 model, and a LEPR antagonist restores chemosensitivity. A cell that suppresses normal blood formation while protecting leukaemic blasts is the worst possible combination, and it means 'reduce marrow adiposity' cannot be assumed beneficial in a patient with disease. Elevated bone-marrow leptin and blast-cell LEPR levels associate with poor cytarabine clearance and reduced survival in newly diagnosed AML patients; in human AML lines, primary blasts and two syngeneic mouse models (MLL-AF9 and AML1-ETO9a), exogenous leptin markedly blunts cytarabine cytotoxicity while the high-affinity LEPR antagonist Allo-aca restores chemosensitivity without altering baseline leukaemia growth; leptin upregulates LEPR and triggers JAK2/STAT3 signalling that boosts mitochondrial complex I activity, oxidative phosphorylation and mitochondrial ROS, and the mtROS surge activates a compensatory antioxidant programme that shields blasts from drug-induced oxidative damage.

    What this evidence cannot show

A 2026 paper with no independent replication. The patient association is correlative and the cohort size is not given in the abstract; 'poor cytarabine clearance' is a pharmacokinetic endpoint whose relation to outcome is indirect. MLL-AF9 is one of two mouse models and the abstract does not separate the KMT2A-rearranged results from the AML1-ETO9a ones, so the KMT2A-specific effect size is not established. Allo-aca is a research peptide with no clinical development. Adults, not infants. Read: abstract only. **Partly resolved:** the leukaemia model is confirmed **murine MLL-AF9**, but the arms remain unseparable - the GEO accessions the paper lists (GSE1159, GSE37642, GSE6891) are **re-analysed public patient cohorts** rather than this study's own data, and the proteomics deposition PXD066693 returns HTTP 404. Resolved 2026-09-19 by the Q18b unstated-systems sweep from the study's deposition, not from its text; bounded as a statement about the deposited data rather than about the whole study.

Evidence that argues against this (2)
    • A redox-buffering account of niche protection sits awkwardly with the one hypoxia experiment done in this genotype: in KMT2A-rearranged AML, hypoxia induction SENSITISED cells to venetoclax plus the menin inhibitor MI-503, and the combination acts partly by repressing HIF-1A and its target HDAC9. If a low-oxygen, oxidative-stress-adapted state made KMT2A-rearranged blasts harder to kill, that experiment should have gone the other way. Read: abstract only. Ling Q et al. 2023
    • A study that lists re-analysed public cohorts in its data-availability statement has deposited nothing that bounds its own experiments, so the deposition route cannot resolve it. This is a half-resolution and should not be read as 'the model is murine MLL-AF9' full stop. Oncofusion knowledge graph (internal analysis) 2026
