LIN28B-dependent fetal B- and NK-lymphopoiesis - the NORMAL state the leukaemia requirement is parasitic on, and the reason the therapeutic window is narrow by construction

also called fetal B-lymphopoiesis · LIN28B-dependent fetal lymphoid output · the normal arm of Ling 2026

Before birth, blood stem cells need LIN28B in order to keep multiplying, stay stem-like, and make B cells and NK cells at all. That is normal biology, not cancer biology - which is why removing LIN28B stops infant leukaemia starting and also stops the baby's own immune system being built.

The same thing, for a scientist

Ling et al. report that in human fetal liver CD34+ cells, **LIN28B is essential to maintain proliferation and stem-like properties and to support B- and NK-lymphopoiesis** [E1]. This is the normal-development arm of the same paper that shows the KMT2A::AFF1 transformation requirement, and it is a distinct object from that requirement: it describes a **state of a normal cell**, not a property of a leukaemia. The graph holds it separately for two reasons. **First, it is the control the transformation result needs.** If a LIN28B-null fetal CD34+ cell is no longer a proliferating, stem-like, lymphoid-competent progenitor, then 'fails to transform on KMT2A::AFF1 induction' may be a statement about the competence of the target cell rather than a statement about the fusion. Nothing readable from the abstract separates the two. **Second, it is the entire bite-back.** Every LIN28B-directed intervention in an infant is aimed at a gene the infant's own lymphopoiesis is using. The mouse literature supports the normal-state claim independently, and in a different lineage: the Lin28b-let-7-Hmga2 axis is a bidirectional developmental clock for myeloerythroid output [E2], and enforced Lin28b reprograms adult haematopoiesis toward fetal-type lymphoid output in published work that this beat did not independently verify (see caveats).

The NK arm deserves a flag of its own. B- and NK-lymphopoiesis both descend from lymphoid progenitors, so a shared requirement is plausible, but this graph holds no other evidence for a LIN28B-NK link and the entry should not be read as establishing one. If it is real, it widens the toxicity concern - NK cells are the arm of immunity a leukaemic infant can least afford to lose, and are also the effector cells of several immunotherapies used in this disease.

The control this state implies, which is the most important experiment named anywhere in this beat

The transformation result and the normal-biology result are the same perturbation read two ways:

LIN28B knockout in human FL CD34+
   |-- normal arm: proliferation down, stem-like properties down, B/NK output down
   |-- leukaemia arm: KMT2A::AFF1 induction produces no transformation

Those two arms are not independent. A cell that has stopped proliferating and lost stem-like properties is a poor substrate for any oncogene. To claim a specific requirement for KMT2A::AFF1 transformation, one needs a second oncogene run in the same LIN28B-null cells - ideally one whose transformation is known not to depend on the fetal state.

Whether Ling et al. did this cannot be determined: the paper is paywalled and absent from PMC and Europe PMC (verified 2026-09-23). It is entirely possible they did and the abstract simply has no room. The honest position for this graph is that the specificity control is unread, not absent, and the experiment is written down as exp-lin28b-null-fetal-progenitor-second-oncogene-specificity-control so that a reader with access can either discharge it or run it.

Where this sits relative to the fetal entries already here
  • cell-type-fetal-liver-hsc is the cell - the compartment and its markers, and the case that it is the cell of origin.
  • cell-state-fetal-program-retention is the leukaemia's state - infant blasts keeping the fetal signature that childhood KMT2A::AFF1 blasts do not.
  • This entry is the normal cell's state - what a healthy fetal progenitor is doing with LIN28B before any fusion exists.

The three are easy to merge and should not be. The retention entry's claim is that the leukaemia keeps a fetal setting; this entry's claim is that the setting itself is a LIN28B-dependent functional programme in normal development. The first is a description of blasts; the second is a requirement in healthy tissue, and it is the one with the toxicity consequence.

Facts and figures
state definitionThe functional programme of a normal human fetal liver CD34+ haematopoietic stem/progenitor cell in which LIN28B is required to maintain proliferation and stem-like properties and to support B- and NK-lymphoid output. It is a state of healthy tissue, not of leukaemia.

relationship to transformationThe SAME perturbation that abolishes this normal state abolishes KMT2A::AFF1 transformation in the same cells. Whether the transformation block is specific to the fusion or secondary to loss of progenitor competence is UNRESOLVED from the abstract and is the key missing control.
therapeutic consequenceA LIN28B-directed agent in an infant targets a gene the infant's own B- and NK-lymphopoiesis requires. The window is narrow by construction, not by accident.

How this could bite us back
  • This is the toxicity entry for the whole beat: the target gene is required for normal human fetal B- and NK-lymphopoiesis, in the same experiment that makes it interesting.
  • If LIN28B is needed for progenitor proliferation generally, a LIN28B inhibitor would be myelosuppressive on top of chemotherapy in patients who are already cytopenic.
  • Infants with KMT2A::AFF1 ALL are treated with blinatumomab and CAR-T approaches whose efficacy depends on lymphoid and NK compartments. Suppressing lymphoid output to block a fetal programme could directly antagonise the immunotherapy backbone.
  • The state is defined by a fetal restriction, which means it is also self-limiting: it disappears with age. A therapy aimed at it has a shrinking target in a growing patient, and a relapse after the programme has lapsed would be untouched by it.

  1. Is the KMT2A::AFF1 transformation block specific to the fusion, or is it downstream of losing a competent proliferating progenitor? A second oncogene in the same LIN28B-null cells is the control, and it is unread rather than known-absent.
  2. How severe is the normal phenotype - a shift in lineage output, as in mouse, or a loss of progenitor function? The two imply very different toxicity ceilings for a LIN28B-directed agent.
  3. Is the NK requirement real and does it persist? NK cells matter both for infant immunity and as effectors of immunotherapies used in KMT2A-rearranged ALL, so a LIN28B agent that suppresses NK output could antagonise the rest of the regimen.
  4. When exactly does LIN28B fall in HUMAN haematopoiesis, and does the fall coincide with the postnatal window in which KMT2A::AFF1 infants actually present? The mouse timing is documented; the human timing is what an epidemiological argument would need.

Evidence that argues against this (1)
    • In mouse, deleting Lin28b in fetal progenitors shifts haematopoietic output toward adult-like myeloid dominance rather than abolishing progenitor function, and conditional Lin28b deletion in fetal progenitors was compatible with normal survival in a leukaemia model - so a complete loss of proliferative and stem-like capacity is not the mouse phenotype, and the human severity claim rests on this one paper. Lin28b expression decreases in mouse common myeloid progenitors as haematopoiesis matures, with mature let-7 accumulating in step; inhibiting let-7 in adults recapitulates fetal erythroid-dominant haematopoiesis, and deleting Lin28b or activating let-7 in the fetal state shifts output toward adult-like myeloid dominance, with Hmga2 as an effector.

Abstract read; full text not opened. MOUSE, MYELOERYTHROID output, normal haematopoiesis. It establishes Lin28b as a causal developmental-timing factor, which corroborates the general shape of the human claim, but it measures a different lineage from the B/NK one and reports a SHIFT in output rather than a loss of progenitor function.

Evidence that argues against this (1)
    • This is a MYELOERYTHROID result used to corroborate a B- and NK-lymphoid claim. The Lin28b-let-7-Hmga2 axis setting developmental timing in common myeloid progenitors does not establish that LIN28B is required for B- or NK-lymphopoiesis, and the one paper that claims the lymphoid requirement is abstract-only here. Corroboration across lineages is weaker than it looks. Blood 2026

FULL TEXT READ from PMC10912017. Both statements are supported in the paper by supplementary figures that were not themselves inspected here (the full text refers to 'Supplementary Fig.' without the figure being reproduced in the text stream retrieved). The human paediatric AML expression claim is a re-analysis of external data, not a primary measurement in this paper. Note the scope: it is about AML, not about infant B-ALL, and it is the postnatal/adult side of the axis - it corroborates that LIN28B is fetal-restricted, not that it is required in fetal cells.

Evidence that argues against this (1)
    • This is the postnatal/adult side of the axis and it corroborates only that LIN28B is fetal-restricted. It says nothing about whether LIN28B is REQUIRED in fetal cells, which is this entry's actual claim, and the expression statements rest on supplementary figures and on re-analysis of external data rather than on primary measurement.
