Enumeration for the partner-outcome beat: what was searched, what was used, what was rejected, what could not be read
also called Q19 partner-outcome enumeration · partner-stratified outcome search record
This is the record of the search itself: which questions were typed into PubMed, how many papers came back, which ones were used, which ones were deliberately set aside and why, and which ones could not be opened at all.
The same thing, for a scientist
Search record for the question does clinical outcome in KMT2A-rearranged leukaemia differ by 3-prime fusion partner, run on 2026-09-19. Nine cohort datasets were located and written up: five reporting a partner effect (childhood AML n=1,130 and n=756, non-infant childhood ALL n=629, adult AML n=181 and n=125, single-centre paediatric AML n=150) and five reporting none (infant ALL n=115 and n=175, adult AML n=172 and n=205, post-transplant registry n=426 KMT2A-r). One cross-age B-ALL study (n=465) reports an adjusted partner effect on MRD clearance rather than survival. Five counted queries established that no menin-inhibitor trial reports response by 3-prime partner in the primary literature. Three papers were located, judged on-beat by title, and set aside for stated reasons; two protocol reports could not be read beyond their abstracts after the full PMC route failed. One premise in the dispatching brief could not be verified and is recorded as such.
Queries run, with counts
All PubMed, 2026-09-19. Multi-word [tiab] terms were run in singular and plural per the standing rule; karyotype terms were run in both parenthesised and bare-token form.
| Query | Count | Used |
|---|---|---|
(KMT2A[tiab] OR MLL[tiab]) AND ("fusion partner"[tiab] OR "fusion partners"[tiab] OR "translocation partner"[tiab] OR "translocation partners"[tiab]) AND (prognosis OR prognostic OR outcome OR outcomes OR survival)[tiab] | 115 | 9 cohorts |
infant[tiab] AND (leukemia OR leukaemia)[tiab] AND (KMT2A OR MLL)[tiab] AND ("translocation partner(s)" OR "fusion partner(s)")[tiab] | 50 | 2 |
11q23 MLL rearranged AML translocation partner prognostic outcome children | 4 | 2 |
KMT2A rearranged acute leukemia fusion partner survival adults | 19 | 5 |
menin-inhibitor partner queries A-E (see absence-partner-stratified-response-in-menin-inhibitor-trials) | 9, 8, 2, 2, 3 | 0 |
Interfant-06 infant ALL outcome Pieters | 10 | 2 |
10.1016/S0140-6736(02)08782-2[doi] (to resolve a reference, never from recall) | 1 | 1 |
One methodological note worth carrying forward: the parenthesised ("t(4;11)"[tiab]) and bare-token (t 4 11[tiab]) karyotype queries returned identical hit sets here, so the PubMed parenthesis trap did not bite in this beat. That is not a reason to stop running both.
Bounded rejections: on-beat by title, not used, with reasons
- PMID 37969120 (Mrózek, Transl Pediatr 2023), Prognostic importance of the fusion partners and measurable residual disease in patients with AML who harbor 11q23/KMT2A alterations. The title is the most on-beat in the whole search. It is an editorial commentary on the I-BFM study and carries no abstract and no primary data. Used as a pointer, not cited for any claim. Would change my mind: nothing, unless it contains an original reanalysis behind the paywall.
- PMID 38026790 (HemaSphere 2023), Integration of Transcriptomic Features to Improve Prognosis Prediction of Pediatric AML With KMT2A Rearrangement, n=351. Builds a 7-gene transcriptomic score that the authors position as a refinement of a stratification already based on treatment response and translocation partner. It does not report outcome by partner, so it cannot answer this beat's question - but it is the strongest evidence that the field considers the partner-based stratification improvable. Would change my mind: per-partner outcome in its supplement.
- PMID 36042201 (Isobe, Nat Commun 2022), Multi-omics analysis defines highly refractory RAS burdened immature subgroup of infant ALL. Squarely relevant to the maturity and RAS confounds that this beat identifies, and cited by the cross-age B-ALL study for exactly that. Set aside because its subgrouping is by transcriptional state and RAS burden, not by 3-prime partner. Would change my mind: a partner-by-subgroup table.
- PMID 29384595 (Eur J Haematol 2018), n=20 MLL-rearranged AML, reporting that MLLT1 and MLLT3 partners benefit more from allo-HSCT. Used only as counter-evidence; twenty patients across many partners cannot support a primary entry, and its direction for MLLT1 contradicts the 1,130-patient classification.
- PMID 42161656 (Zhonghua Xue Ye Xue Za Zhi 2026) and PMID 41918382 (Zhonghua Yi Xue Yi Chuan Xue Za Zhi 2026), both Chinese-language reports on adult KMT2A AML and on non-AFF1 11q23 variants (n=10) respectively. Not read; recorded so a future cycle knows they exist and were not assessed rather than not found.
- PMID 30280786 (Eur Rev Med Pharmacol Sci 2018), CCLG-ALL2008 MLL-rearranged paediatric ALL. Corrected 2026-09-20: the abstract WAS retrievable and so is the full text. Europe PMC carries the abstract, and resolving the DOI returns 12,787 characters (route 5,
ana-the-route-5-sweep-and-the-publisher-fingerprints). The venue caution stands and is joined by a sharper one: the KMT2A arm is 28 patients inside a 634-patient cohort, in a high-risk group of 217 split three ways, so it cannot carry a partner-stratified conclusion. Recorded as read-and-weighted-lightly rather than not-read.
A general rejection class: reviews. 39887730, 39201709, 39428967, 36979800, 42122138 and roughly a dozen others are review articles on KMT2A-rearranged leukaemia or menin inhibitors. All were title-checked, none cited, per the standing preference for primary sources.
Bounded unreadables: located, on topic, could not be read
Both were checked by the full route the standing brief requires - Europe PMC search?query=EXT_ID:<PMID>&resultType=core&format=json read for the pmcid field, not for abstractText - and both returned pmcid: null, so no fullTextXML fetch was possible.
| PMID | Paper | PMCID | Full text | What its title asserts |
|---|---|---|---|---|
| 17658395 | Interfant-99 primary report, Lancet 2007 | none | none | a hybrid ALL/AML protocol for infants under 1 year, with a randomised late-intensification question |

| 19528532 | Balgobind 11q23 paediatric AML, Blood 2009 | PMC2927031 | 0 chars | novel prognostic subgroups in childhood 11q23/MLL-rearranged AML |
For the last two a PMCID exists but fullTextXML returned zero characters, so the abstract is all that was read. No content beyond each abstract is asserted anywhere in this beat for any of these five papers, and in each case the entry says so in its caveats. In particular, the AUGMENT-101 Data Supplement Table S3, which contains the per-partner response rates, is not carried in that paper's PMC record (pmc-prop-has-supplement: no) and was not read.
A premise in the dispatching brief that could not be verified
The brief stated that Interfant-99 and Interfant-06 both report outcome by partner. From the retrievable record that could not be confirmed: neither abstract contains per-partner EFS, neither full text is open access, and Interfant-06's stated prognostic factor is KMT2A rearrangement as a binary term. The claim may still be true of the papers' tables. Per the standing instruction that a brief is a starting point and not a finding, it is recorded as unverified rather than repeated. See epi-interfant-protocols-partner-stratified-outcome-not-reported.

- This enumeration's negative claims are only as good as the phrase 'fusion partner' being present in an abstract; a study reporting per-partner outcome in a table without flagging it in its abstract is invisible to every query run here, and that is the most likely way this beat is wrong.
- Five of the most important papers in the beat were read as abstracts only, so every number taken from them is a number the authors chose to put in an abstract - a selection that systematically favours significant findings over null subgroups.
- The broadest query returned 115 records of which 40 were title-checked. What is in the remaining 75, and does any of them report per-partner outcome?
- Are there partner-outcome reports in the non-English literature, particularly the two Chinese-language records listed above and any Japanese reports from the JPLSG series?
- Do conference abstracts from ASH and EHA contain partner-stratified menin-inhibitor response data that never reached a journal? None was searched in this beat.
- E1moderate confidence 3 of 5: One strong study, or several smaller consistent ones.database
The broadest counted query for this beat, KMT2A or MLL combined with fusion/translocation partner terms in singular and plural and with prognosis/outcome/survival terms, returned 115 PubMed records on 2026-09-19, from which nine cohort datasets with partner-stratified outcome were identified.
PubMed esearch, query: (KMT2A[tiab] OR MLL[tiab]) AND ("fusion partner"[tiab] OR "translocation partner"[tiab] OR "fusion partners"[tiab] OR "translocation partners"[tiab]) AND (prognosis[tiab] OR prognostic[tiab] OR outcome[tiab] OR outcomes[tiab] OR survival[tiab]) · National Center for Biotechnology Information · PubMed (NCBI E-utilities esearch) 2026What this evidence cannot show
Only the first 40 titles of the 115 were inspected in the initial pass; the remaining 75 were not individually title-checked, so the enumeration is not exhaustive at the tail. Restricted to title/abstract fields, so a study reporting partner outcome only in tables without saying so in its abstract is invisible to this query. English-language bias in what was subsequently read. Counts on PubMed are not stable over time.
Evidence that argues against this (1)
- The standing retired-symbol rule warns that queries keyed to current gene symbols undercount by up to tenfold; this query uses the generic phrase 'fusion partner' rather than partner gene symbols, which avoids that trap but introduces the opposite one - a paper reporting per-partner outcome without using the phrase is missed entirely. The single most on-beat title located, a 2023 Translational Pediatrics article on the prognostic importance of fusion partners and MRD in 11q23/KMT2A AML, is an editorial commentary with no abstract and no primary data, and was rejected for that reason.
Prognostic importance of the fusion partners and measurable residual disease in patients with acute myeloid leukemia who harbor 11q23/KMT2A alterations. · Mrózek K · Transl Pediatr 2023What this evidence cannot show
The NCBI efetch record returns no AbstractText for this PMID. Its status as a commentary is inferred from the venue, the single-author byline and the absence of an abstract; the full text was not retrieved, so this classification could be wrong.
Evidence that argues against this (1)
- The study it comments on, the 1,130-patient I-BFM analysis, is cited extensively throughout this beat, so nothing is lost by rejecting the commentary unless it contains an original reanalysis. van Weelderen RE et al. 2023

A 351-patient paediatric KMT2A-r AML transcriptomic study explicitly describes the existing risk stratification as established on treatment response and the translocation partner of KMT2A, and proposes a 7-gene score that outperformed LSC17 - evidence that the field regards partner-based stratification as real but improvable, without itself reporting outcome by partner.
Integration of Transcriptomic Features to Improve Prognosis Prediction of Pediatric Acute Myeloid Leukemia With KMT2A Rearrangement. · Li J, Zong S, et al. · Hemasphere 2023What this evidence cannot show
Abstract only. The comparison is against LSC17 and against the authors' own clinical model, not head to head against partner-based stratification, so it does not establish that a transcriptomic score beats the partner. Derived and validated in cohorts whose overlap with the I-BFM cohort is not stated.
Evidence that argues against this (1)
- The I-BFM study found the partner risk group independent of end-of-induction MRD in multivariable analysis, so a transcriptomic score that correlates with treatment response may be capturing the same axis rather than a better one. van Weelderen RE et al. 2023

A multi-omics study of infant ALL defining a highly refractory RAS-burdened immature subgroup is on-beat for the maturity and comutation confounds but subgroups by transcriptional state and RAS burden rather than by 3-prime partner, and was therefore set aside.

Abstract-level assessment; the paper may contain a partner-by-subgroup table not reflected in its abstract, in which case this rejection is wrong and should be revisited. **REJECTION OVERTURNED 2026-09-19.** The condition this caveat set for its own reversal was met. The full text (PMC9427775) does partition by fusion partner, and strongly: three of the five integrative clusters correspond to MLLT1 (P = 3.7e-8), MLLT3 (P = 5.6e-6) and AFF1 (P = 5.7e-6), while no survival difference was seen across partners. Written up as `epi-infant-all-molecular-subgroups-track-partner-but-not-outcome`. Surfaced by `scripts/reading-queue.py` on low claim-to-abstract overlap.
Evidence that argues against this (2)
- The cross-age B-ALL study cites this work for the developmental-maturity axis and then shows that fusion partner contributes independently of maturity (P=0.03), so the two framings are complementary rather than alternatives and a combined analysis would be more informative than either. Hartmann AM et al. 2026
- The rejection was reversible only because it named what would overturn it. An agent writing 'set aside as off-beat' with no condition attached produces a decision nobody can audit, and this project's rejection register would then be a record of conclusions rather than of reasoning. The practice is worth the extra sentence. Nature Communications 2022
