Cell identities considered and not written: what the search actually returned for basophils, mast cells, eosinophils, NK cells and the rest

also called cell type rejection ledger · cells saturation rejected candidates

Before writing entries for the cell types this disease talks about, every plausible candidate was searched in PubMed and the results counted. This records the ones that were deliberately not written and why, so that nobody spends a day rediscovering that there are zero papers connecting this leukaemia to mast cells.

The same thing, for a scientist

A rejection ledger for the cells-saturation beat of cycle 39, following the model of `ana-kmt2a-interactome-systematic-enumeration` and `ana-histone-mark-audit-rejected-candidates`. Every candidate cell type or state was scored by a PubMed eutils intersection with **both halves controlled**: the disease half `(KMT2A OR MLL-rearranged OR MLL-AF4 OR MLL-AF9)[Title/Abstract]` returns **2,919** records, and each cell half was counted separately so that a small intersection could be distinguished from a small literature [E1]. Nine cell types and four cell states were written. **Five candidates are rejected outright** for having essentially no KMT2A-specific literature: basophil (0 records), mast cell (0), plasma cell (2), Langerhans cell (4) and osteoblast (5). **Three are rejected as not lineage-relevant** despite non-zero counts, because inspection of the records shows that none describes a KMT2A-driven leukaemia of that lineage: eosinophil (10 records, of which two are real KMT2A biology — a KMT2A partial tandem duplication in hypereosinophilic syndrome and a YAP1::KMT2A sarcoma — but neither is an eosinophil-lineage leukaemia), natural killer (14, NK biology incidental in each) and neutrophil (13, concerning neutropenia as a complication). **Four are deferred rather than rejected**, with the highest-priority being the bone-marrow mesenchymal and neural niche, where the intersection is 86 records and includes a primary finding that MLL-AF9 AML disrupts sympathetic nerves and Nestin-positive niche-cell quiescence [E2].

Method, and the rule this beat followed

The standing instruction that governs this ledger is the one about controlled intersections: a small intersection means nothing unless both halves are counted. A query returning 9 records could mean the association is absent or could mean the literature on either side is tiny. All counts below were run against NCBI eutils esearch on 2026-09-19 with [Title/Abstract] field restriction and no other filters, and every non-zero intersection under 20 records had its titles inspected before a disposition was assigned 1.

The disease half is (KMT2A[Title/Abstract] OR MLL-rearranged[Title/Abstract] OR MLL-AF4[Title/Abstract] OR MLL-AF9[Title/Abstract]) = 2,919 records. Using KMT2A alone returns 1,651 and misses the older literature, which matters for cell types whose association would have been described in the 1990s if it existed at all.

Basophil and mast cell are the strongest rejections in this ledger and are worth stating positively rather than as an absence of evidence: against a basophil literature of 20,779 records and a mast cell literature of 51,465, the intersection with a 2,919-record disease literature is exactly zero in both cases. That is not an under-studied association; it is the absence of one. Both are granulocytic/mast lineages downstream of the GMP or of a separate basophil-mast progenitor, and their exclusion sharpens the point made in cell-type-cmp that KMT2A fusions occupy a narrow slice of the myeloid branch — monocytic, not granulocytic, not basophil-mast, not erythroid, not megakaryocytic.

Plasma cells and Langerhans cells were checked because both appear in the differential of childhood histiocytic and lymphoid neoplasia; osteoblasts because they are a classical HSC niche cell.

Rejected as not lineage-relevant: non-zero counts that do not survive reading
candidatecell-half recordsintersectionwhat the records actually are
eosinophil102,80810a KMT2A partial tandem duplication in hypereosinophilic syndrome (case report), a YAP1::KMT2A sarcoma, and a severe-asthma sputum biomarker paper
natural killer59,61114includes an attenuated Salmonella typhimurium leukaemia-inhibition study and a CD244/SHP-2 leukaemia-initiating-cell paper in which NK cells appear only as assay components
neutrophil214,97013shares the same severe-asthma sputum paper; the remainder concern neutropenia as a treatment complication rather than neutrophilic leukaemia

The disposition here is narrower than the one above and the wording matters. These three are not rejected on the grounds that the co-occurrences are all accidental — two of the ten eosinophil records are real KMT2A biology, namely a KMT2A partial tandem duplication in hypereosinophilic syndrome and a YAP1::KMT2A sarcoma. They are rejected because none of the records describes a KMT2A-driven leukaemia of that lineage, which is what a cell-type entry in this beat would have to be about 3.

The lesson the ledger exists for still holds, and the severe-asthma sputum paper appearing in both the eosinophil and neutrophil intersections makes it: a raw intersection of 10-14 records looks like a thin but real literature, and a common reason for co-occurrence is that both terms appear in a paper about something else entirely. A count is not a finding, and reading the titles is what separates the two cases.

Note also what the eosinophil search surfaced: YAP1::KMT2A sarcoma. KMT2A fusions exist outside haematopoiesis. That is out of scope for a cells beat but is a genuine and probably under-represented fact in this graph.

Deferred, not rejected — these have real literature and no entry

1. The bone-marrow mesenchymal and neural niche — the highest-priority remaining gap. The intersection of the disease half with (mesenchymal OR niche OR stromal)[Title/Abstract] returns 86 records. It contains at least one substantial primary finding: neuropathy of the sympathetic nervous system promotes leukaemic bone-marrow infiltration in an MLL-AF9 AML model, with AML disrupting sympathetic nerves and the quiescence of Nestin-positive niche cells, expanding mesenchymal stem and progenitor cells primed for osteoblastic differentiation at the expense of HSC-maintaining NG2-positive periarteriolar niche cells, and with the leukaemogenic adrenergic signal transduced by the beta-2 but not beta-3 adrenergic receptor on stromal cells 2. That is a cell-type story (Nestin+ MSPC, NG2+ periarteriolar cells), it is KMT2A-specific, and the graph has no entry for any of it. It is deferred here only because a niche beat is a different beat, and it is flagged as the single best target for the next cells cycle.

2. The MPAL blast. Deliberately not written as a cell_type, because mechanism-kmt2a-mpal-mixed-phenotype already covers the WHO lineage-assignment criteria, the one-plastic-cell-versus-two-clones question, the roughly 10 percent KMT2A share of ambiguous-lineage leukaemia and the ALL-type-induction treatment finding, at cell-identity depth. Writing a parallel cell-type-mpal-blast would violate the one-entry-per-thing rule in the direction of duplication rather than granularity. The one piece of new material found this cycle — that ETP-ALL and T/myeloid MPAL have overlapping immunophenotypes and both carry KMT2A rearrangements, separated definitively only by cytoplasmic MPO — has been placed in cell-type-early-t-precursor instead 4.

3. The erythroid progenitor and proerythroblast. Intersection 14 records. The erythroid question that mattered to this project — whether erythroid character explains the haem-biosynthesis dependency — is answered in cell-type-mep, which records that none of the 11 KMT2A-rearranged DepMap lines is erythroid or megakaryocytic while eight such lines sit in the comparison group. A separate erythroid entry would add the normal biology without adding a KMT2A-specific fact.

4. Senescence as a distinct cell state. Folded into cell-state-quiescence rather than written separately, because the only KMT2A-adjacent paediatric-leukaemia report describing a 'senescence-associated stem-like' dormant subpopulation is in Ph-like ALL and uses 'senescent' and 'dormant' together, which conflicts with the standard distinction between reversible quiescence and irreversible senescence. The quiescence entry records the contradiction as an open question rather than asserting a state the literature does not cleanly define.

What was written

For completeness, the positive side of the ledger. Cell types: cell-type-monocyte, cell-type-monoblast, cell-type-gmp, cell-type-cmp, cell-type-mep, cell-type-megakaryocyte, cell-type-clp, cell-type-early-t-precursor, cell-type-plasmacytoid-dendritic-cell. Cell states: cell-state-quiescence, cell-state-gmp-like, cell-state-cell-cycle-g1-cdk6, cell-state-fetal-program-retention.

One of these, cell-type-plasmacytoid-dendritic-cell, sits close to the rejection threshold: its intersection is 9 records and the evidence is case-report-level, with one published KMT2A-rearranged 'BPDCN-like' case turning out to be acute monocytic leukaemia once MPO was assessed. It was written rather than rejected because the neonatal-leukaemia literature lists BPDCN among presentations of the population in which KMT2A-AFF1 is commonest, because CD123 is a live therapeutic target shared with AML blasts and the GMP, and because one case documents pDC and monocytic programmes in a single blast population. Its confidence_overall is set to weak and its caveats say so.

How this could bite us back
  • A rejection ledger built on title/abstract counts will systematically miss associations reported only in full text or under obsolete gene synonyms, so a future finding could overturn any of these dispositions without any of them having been unreasonable.
  • Deferring the niche means the graph currently has no account of the compartment that imposes quiescence, which is the compartment cell-state-quiescence identifies as the cause of relapse; the deferral is defensible but it leaves the causal chain broken at its most therapeutically relevant link.
Still open
  1. Would repeating these searches with the older synonyms ALL-1, HRX, Htrx and 11q23 change any disposition, particularly for basophil and mast cell where the 1990s cytogenetic literature would have reported a lineage association if one existed?
  2. Is the bone-marrow niche a cells-beat topic or its own beat? It has 86 records, a primary mouse finding in MLL-AF9, and no entry anywhere in this graph, which makes it the largest identified gap in the cell domain.
  3. Should the MPAL blast get a cell_type entry after all, given that a reader searching by node type will not find it under mechanism?
  4. KMT2A fusions occur in sarcoma (YAP1::KMT2A). How much non-haematopoietic KMT2A biology is this graph missing, and does any of it bear on the menin interface?

    What this evidence cannot show

Title/Abstract field restriction misses associations described only in full text, in figures or under partner-gene names such as AF4, ENL or AF10 without the MLL prefix. Older literature using 'ALL-1', 'HRX' or 'Htrx' for KMT2A is not captured by this disease half. Counts are a screening instrument, not a measure of biological association, which is why every non-zero intersection under 20 had its titles read.

Evidence that argues against this (1)
    • A zero intersection under one synonym set is not a zero literature. The disease half used here omits 'ALL-1', 'HRX', 'Htrx' and '11q23', the last of which is the term under which much of the 1990s cytogenetic literature on lineage associations was published; a basophil or mast-cell association reported cytogenetically in 1995 as '11q23 abnormality' would be invisible to this search. The EU Concerted Action 11q23 Workshop series, for example, reported lineage distributions across 550 patients under the 11q23 heading rather than under MLL. Read: abstract only. In an MLL-AF9 AML model, neuropathy of the sympathetic nervous system promotes leukaemic bone-marrow infiltration: development of AML disrupts sympathetic nerves and the quiescence of Nestin-positive niche cells, leading to expansion of phenotypic mesenchymal stem and progenitor cells primed for osteoblastic differentiation at the expense of HSC-maintaining NG2-positive periarteriolar niche cells, with the leukaemogenic adrenergic signal transduced by the beta-2 but not the beta-3 adrenergic receptor expressed on stromal cells.

    What this evidence cannot show

Mouse MLL-AF9 model; the niche cell populations are defined by mouse reporter alleles (Nestin-GFP, NG2) that have no exact human equivalent, and no human KMT2A-rearranged marrow has been shown to carry the same sympathetic neuropathy. Cited here to justify deferring rather than rejecting the niche, not as an established feature of human disease. Read: abstract only.

Evidence that argues against this (1)
    • The niche's influence on quiescence is contested even within normal haematopoiesis, with megakaryocytes assigned the quiescence-maintaining role through CXCL4 in one study and through TGF-beta1 in a simultaneous one, and with the direction of the effect inverting after chemotherapeutic stress. A niche beat entering this area would inherit an unresolved mechanistic literature, which is a reason the deferral is defensible rather than merely convenient. Read: abstract only. Zhao M et al. 2014

Inspection of the small non-zero intersections shows that none describes a KMT2A-driven leukaemia of the lineage in question: the eosinophil and neutrophil intersections both include a severe-asthma sputum biomarker paper, the natural killer intersection includes an attenuated Salmonella typhimurium leukaemia-inhibition study and a CD244/SHP-2 leukaemia-initiating-cell paper in which NK cells appear only as assay components, while the two eosinophil records that are genuine KMT2A biology are a KMT2A partial tandem duplication in hypereosinophilic syndrome and a YAP1::KMT2A sarcoma.

This is an example record cited to document one kind of false-positive co-occurrence, not a source for any biological claim. Only the title was read; the record appears in both the eosinophil and neutrophil intersections. Author list taken from the eutils record.

Evidence that argues against this (1)
    • A count-based rejection can still be too quick, and the eosinophil row is where this ledger came closest to being so. Two of its ten records are genuine KMT2A biology: a KMT2A partial tandem duplication in a patient with hypereosinophilic syndrome, and a YAP1::KMT2A-rearranged sarcoma showing that KMT2A fusions occur outside haematopoiesis altogether. Neither is an eosinophil-lineage leukaemia, which is why the disposition stands as written, but a reader should know that the eosinophil intersection is not empty of KMT2A content and that the second of those records points at a whole domain this graph barely covers. Evidence that argues against this (1)
    • The decision not to write a cell-type entry for the MPAL blast could be wrong if the existing mechanism entry is read as being about the disease rather than about the cell. mechanism-kmt2a-mpal-mixed-phenotype does cover WHO lineage-assignment criteria and the one-cell-versus-two-clones question, but a future reader looking for 'what is an MPAL blast' under cell_type will not find it, and the graph's own granularity rule favours many precise entries. This deferral should be revisited if the cells folder is extended again. Read: this graph's own entry. Khoury JD et al. 2022
